post/manual till 14.9.2016 time 5.30 p.m. in the Office of by The Executive Engineer, M.P.

State Agriculture Board, Technical Division No.2, Bhopal for providing comprehensive

Annual Maintenance Of EPABX, Telephone Intercom System Of Mandi Board Complex At

26, Kisan Bhawan, Arera Hills, Bhopal. The tender form can be purchased from the Office

of by The Executive Engineer, M.P. State Agriculture Board, Technical Division No.2,

Bhopal till 13.9.2016 time 5.30 p.m. (excluding holidays). The tender form can also be

viewed and downloaded from the website of the mandi board

www.mpmandiboard.gov.in. In case, tender form is downloaded from the website of

Mandi Board than demand draft of the cost of the tender document in the name of The

Executive Engineer, M.P. State Agriculture Board, Technical Division No.2, Bhopal will

have to be sent separately in sealed envelope.

DOCUMENTS COMPRISING THE BID

The bid submitted by the bidder shall be in the following parts:

Part 1 – This shall be known as Envelope A and would apply for all bids. Envelop A shall contain the following as per details given below:

i) Registration of proprietary or firm or company and organizational details.

ii) Payment of the cost of Bid Document; iii) Earnest Money; and iv) An affidavit duly notarized.

Part 2 – This shall be known as Envelope B and it should contain the

following documents:

i) PAN number. ii) TIN number iii) Gumasta license. iv) Commercial tax certificate. v) Experience certificate.

Part 3 – This shall be known as Envelope C and would apply to all bids. Envelop C shall contain financial offer in the prescribed format.

This contract is meant for ANNUAL MAINTENANCE OF EPABX, TELEPHONE

INTERCOM SYSTEM with all labour and material services as given below. Within

installation testing and commissioning of new parts /equipments /accessories in

place of damage /broken /worn-out pans /equipments for existing system.

NOTE:

The above list has been given only to given general idea to the contractor to

assess the quantum of work. This does not mean that contractor's scope of work

will be limited to maintain and attend above equipments and operation of complete

installation from EPABX ROOM to Distribution panel/ Officers chamber and

Employee Chamber Labour and Material for changing of faulty cables, faulty

Telephone Instrument.

SECTION - 3

SCOPE OF WORK:

Contractor shall do periodical preventive maintenance for the equipments as

and when required by inspection, testing, cleaning etc. For perfect trouble* free

and fault free smooth operation. The periodical preventive maintenance work

should be maintained in the register and to be got verified and signed by Mandi

Board Authorities, Periodical preventive maintenance will have to be done as per

IS:CODE, i.e. RULES OR as per prescribed in maintenance manuals.

1. CHECKING OF WIRING "WEEKLY OR AS REQUIRED.

2. Examine visually WIRE connection WITH.. Telephone Instrument "WEEKLY.

3. Examine OPERATOR CONSOL EQUIPMENT "WEEKLY OR AS REQUIRED.

4. Checking cable/wiring connection in Battery, M.D.F., Telephone Instrument etc.

5. Operation and maintenance of all equipment/ panels. "WEEKLY

6. Replacement work of defective parts and spare "WEEKLY

Apart from the above points, the contractor shall also do ALL the Telephone

Related work as per instructions/ schedule given by the Engineer In-charge time to

time.

SECTION - 4

TERMS AND CONDITIONS:

1. The Contractor should have valid Coral Company Authorized Service Dealer.

2. The Contractor must have at least (one) 01 year experience within last 03

years with similar Nature of Work and similar cost of work in Govt./ Semi Govt.

Department.

3. Contractor will be responsible to maintain the record in separate register.

4. Contactor will be responsible for taking all sorts of safety measures for his

workers during duty hours to avoid any type of incident.

5. Contractor should have his own tools and plants, helmets, including hand

gloves, other safety measures in good working condition for his working staff.

6. Contractor will be responsible for any causality of his workers if occurred due

to electrical fault or by any accident during duty or by any accident during duty

hours. Mandi Board will not be responsible in any way for the same.

7. In case of emergency contractor must provide the labour And Material for

Telephone intercom Maintenance work as per requirement and no Extra

payment shall be for the same.

8. Unauthorized person should not be allowed to enter in the EPABX control

room.

9. A fine of Rs.100/- per day will be imposed. if working staff of contractor is not

attend and rectify the Complaint within 24 hours.

10. The Contractor must do the group Insurance of the Workers on duty. He will

be fully responsible for complying labour laws as per Government rules and

regulations.

11. Contractor will be responsible to come at site on call at any time during 24

hours (even Sunday and holidays) with their technical staff if needed at the

time of Repair and Maintenance of intercom System apart from duty hours. No

extra payment will be made for the same.

12. In case of damages/ losses occurred due to negligent of contractor or his

working staff, said damages will be recovered from the contractor's bill.

13. In case of any dispute between the Employer and the Contractor, the decision

of the Managing Director, Mandi board shall be final and binding on both the

parties.

14. Any type of conveyance/ transportation etc. will be borne by the contractor.

15. The Engineer in-charge will be at liberty to terminate the contract by giving 15

days notice in writing, if the contractor fails to comply with the provisions of the

contract or if the services rendered by him are found to be unsatisfactorily.

SECTION - 5

TENDER BASIS AND MODE OF PAYMENTS:

1. The Contractor will be for a period of twelve months from the date of

commencing work.

2. Rates should be quoted on yearly basis.

3. M.P.S.A.M. Board will not be responsible for any tax or duties Income tax,

Service tax or any tax levied on contractor by other department or agencies.

The contactor will be entirely responsible for all kind of taxes.

4. The rates quoted by the contractor will not be enhanced during the period of

contract in any case.

5. The Engineer in-charge reserves the right to reject any/all quotations or

incomplete quotations without assigning any reason.

6. Successful contractor will have to deposit Rs.5000/- as performance guarantee

for due performance of the contract. The performance guarantee will be

refunded without any interests only after expire of the entire contract period in

case of satisfactory performance by the contractor.

7. Payment will be made to the contractor in equal installment every 03 month on

producing the bill in triplicate copy through the concerned division.

8. In case of satisfactory performance by the contractor the contract can be

renewed/ extended for a period of 12 months if both parties to the contract

mutually agree for the same. In case of renewal/ extension of the contract, the

rates will remain the same.

AGREEMENT FORM AGREEMENT

This agreement, made on the _________________ day of ________________ between ___________________ (name and address of Employer) (hereinafter called " the

contractor) hereinafter called "the Contractor" of the other part.

Whereas the Employer is desirous that the Contractor execute
