Why do we need to improve?
• Reducing measurement error
• Improving accuracy of usual
dietary intake
• Strengthening associations
between diet & disease

Dietary assessment – the basics:
what was eaten?
how much was eaten?
how often was this eaten?
Methods: retrospective v. prospective

Sources of measurement error:
• Different assessment methods
• recall problems & cognitive complexity, food tables,
cultural, embarrassment (underreporting), etc
• Different designs produce different results eg, CC
and cohort
• Homogeneity of diet within individual population
groups
• Nutrient intakes highly correlated
• Heterogeneity eg, dietary pattern definitions

Is this good enough?
Can we do better?!!
• use of new technology
• Web-based; mobile phone app.; camera.
• portion size estimation
• DietDay, IPSAS – allows individual portion
size grading; portion size atlas; mobile
phone - portion size recognition….
• more independent biomarkers
• better measures of cooking style
• expand scope of food composition
tables

In UK, 92% of population
owns/uses a mobile phone
(2012, OFCOM)
39% owned a smartphone
in 2011 (OFCOM)
50% in January 2012
(Kantar)
A smartphone which allows
self-monitoring of food and
activity could be a
convenient weight
management tool.

Adherence to intervention:
Self monitoring declined in all 3 arms over 6 months but total days usage
was highest in the MMM group (p=<0.0001) .

Distribution of days of dietary recording on MMM
for each participant over the course of the 6
month MMM pilot trial
Duration (days)
Figure :Plot of daily use of MMM for dietary self monitoring in a randomised, 3 armed (smartphone app,
website, diary) 6 month pilot trial of MMM (N=128). The X axis is the MMM ID number that was
automatically assigned to the participant and the y axis is dietary self monitoring in days. Each green
shaded box is a day with ≥500 and ≤5000 kcal energy recorded on MMM.

► At 6 weeks, 91% of smartphone group “satisfied” or “very satisfied” with
group allocation compared to 23% in the diary group and 50% in the web group
(p=0.02).
► At 6 months, 63% of smartphone group “satisfied” or “very satisfied” with
group allocation compared to 50% in the diary group and 42% in the web group
(p=0.045).
► 65% found the smartphone “convenient”, (compared to 35% in the diary
group and 53% in the website group) (p=0.006)
► 76% of smartphone group felt “comfortable using study equipment in
social settings”, (compared to 40% in the diary group and 21% in the website
group) (p=0.002).

Advantages of an automated 24 hour
recall
Potential to reduce underreporting
compared to food frequency
questionnaires.
Extensive food composition database
can be incorporated.
Up-front coding and cleaning of
responses reduces the researcher’s
workload.
Can relatively easily be adapted for
different population groups.
There is no online 24 hour dietary recall assessment tool for the
UK adult population.
Traditional methods of
dietary assessment are
limited.

Focus groups – to inform development
16
Focus groups in 3 age groups: adults (n=24); adolescents/11-18 yrs (n=29) and
older people/65+yrs (n=7)
Adolescent key findings
•Must be quick – max time to complete 15 minutes
•Would not read help – some suggested video or avatar
•Food portion images important
•Would require incentive to complete
•Lay-out – clear and simple, not too much text on page

Focus group findings
17
Adult key findings – similar to
adolescents but also:
• Feared endless list of foods
• Desire to search by keyword and
category.
•Able to navigate site quickly, no pop-
ups
•Design – ‘health neutral’, clear, no
elaborate fonts or colours
•Wanted feedback on intake
Older people key findings;
• Time to complete (20-30 mins
acceptable)
•Incentive to take part
not a priority
•Mistrust of entering
personal details online

Development of a new branded food
database
• Brandbank – ‘Big 8’ back of pack nutrition only = initially 50,000 items
+ UK supermarket brands (e.g. Tesco, Morrisons)
+ Major fast food chains in the UK
+ Generic items from McCance and Widdowson
To create a database with full micronutrient data, McCance
&Widdowson values have been mapped to Brandbank macronutrient
data.
• Currently 46,000 food and drink items have been mapped

Mapping portion size images
21
• Portion size images mapped to food and
drink items. 6000 foods have images
assigned.
• Users can also estimate portion size as:
fraction of purchased pack; suggested
average portion or manually entering
amount.

Future investigation
• Usability testing (3 age groups)
• Validation study (reference measure= 2 x dietitian
administered 24 hr diet recalls over 6 months, 2x blood and
urine collection biomarker assessment).
• Pilot in a sample of the UK women’s cohort study (35,000
women) and Clinical Biobank cohort at Imperial College.
• Dissemination for use by researchers
