Welcome to Issue 7

Firstly, a big congratulations goes to Myleth Satane for submitting the wining photo for the front cover competition this issue. According to Myleth, “this is our photo of my workmates and volunteers celebrating St. Patrick’s Day at Geriatric Evaluation Management Uni (Gem Unit) in Modbury Hospital, South Australia.”Looks like everyone had a good time celebrating in green that day. Thanks and congratulations once again Myleth for submitting the winning photo.

For this issue’s feature, we took a closer look at Education for Healthcare Professionals (page 28), which takes a look at different nursing specializations while discussing the career paths and courses of study you need to attain a specialised role. Developing Critical Thinking SkillsCritical thinking is a vital part of nursing care, particularly as the profession becomes ever-more complex and accountable.

Critical thinking encourages us to use reflection and intellectual reasoning to make relevant, focussed

and creative decisions in our practice. Nurses who employ critical thinking are continually

questioning actions and analysing outcomes rather than practising their craft by rote.

An article by Lunney (2003) discusses seven

cognitive skills of critical thinkers in nursing. These

were originally described by Scheffer and Rubenfeld (2000)

after studying the opinions of a number of nurses who had written

about critical thinking topics.

These seven cognitive skills are identified as:

1. Analysing - i.e. breaking down a situation into its parts in order to

determine possible meanings.

2. Applying standards - the nurse selects specific standards that he/she

believes fits a particular situation, and then considers how these standards can be applied and whether they are applicable to the situation.

3. Discriminating - in discrimination, the nurse considers how a situation is similar to, or different from, other similar situations faced in the past.

4. Information seeking - this is used to decide whether further information is needed and where further information can be obtained, as well as the practice of obtaining needed information.

5. Logical reasoning - this step involves clustering data into meaningful groups or patterns and deciding on the relevance of individual pieces of data.

6. Predicting - In this step, the nurse anticipates different outcomes that may arise from using data in different ways and considers diagnoses in relation to interventions.

7. Transforming knowledge - the nurse uses knowledge gained from past experiences and adapts that

knowledge to use in new situations.

The use of these seven cognitive skills together with 10 habits of the mind also described by Sheffer & Rubenfeld (2000) involves a level of critical thinking that goes beyond the simple application of the nursing process. In using these skills, nurses will be more effective problem solvers, advocates and teachers.

What does an aged care nurse do?Aged care nurses are responsible for taking care of the ageing population of the community, from over 65 years of age and older. This includes assistance with day-to-day activities, health promotion, pain management, wound care, palliative care and family education. As an aged care nurse, you need to have well developed diagnosis, assessment, treatment and evaluation skills, specifically for your elderly patients, as well as have the appropriate knowledge and skills necessary within your chosen area of practice. Here are a few fields of aged care nursing that might interest you:

Acute Care: Within this field of practice, you are responsible for liaising closely with community nurses and other health care professionals to better develop an appropriate plan of care for the patients within hospitals, for when they return to their homes.

Community care: You will find many great opportunities to work independently, or in partnerships with families, other healthcare providers or small multidisciplinary teams. You may be required to visit patients within their home residences, or community centres where they may be based.

Residency Care: This is where the most current and innovative nursing homes provide their patients with good quality of life and health. This is specifically for the elderly unable to care for themselves on a daily basis.

Rehabilitation: This is for nurses who are interested in looking after patients with a variety of physical health problems, ranging from strokes, amputations or severe fractures, to serious surgical recoveries, debilitation of complex health issues and acute medical episodes.

An aged care nurse must also have excellent communication skills as they will be required to communicate to their elderly patients,

coordinate teams and counsel or support family members of their patients.

How do I become an aged care nurse?Firstly, you must be a registered nurse to apply for a post graduate certificate, diploma or masters in aged care at the available colleges and universities.Alternatively, you can also work as an assistant nurse or an enrolled nurse, if not already a registered nurse, depending on your field of study. There are a number of different fields in which you can enter, such as; residential, acute care, community and rehabilitation. Depending on your interest, you can enroll to study any of the following as previously mentioned ; health education, management, administration, acute nursing, advanced clinical nursing, and so on, just to name a few.

What career opportunities exist?As Australia’s population continues to age, nurses within the field of aged care will soon enter great demand, if not already. Nurses within this area, who are experienced enough can move into education, management roles and researching to further their careers. You can even expand into various different fields of nursing and health, which includes audiology, optometry, psychology and mental health. Currently, aged care nursing is a relatively new and developing field of nursing with many exciting opportunities available.

The University of Notre Dame offers a wide range of postgraduate courses ranging from graduate certificates to doctoral programs. The graduate courses provide further and continuing education to qualified and experienced registered nurses.

These courses are aimed at enabling the registered nurse to develop knowledge and consolidate skills to function as specialists within their chosen discipline. They are also aimed at providing currently practicing specialised nurses with opportunities to advance their own professional practice.

These courses also critically question research evidence in order to make informed judgment about the robustness and utility of findings related to nursing practice. The courses will provide an important link between research and professional practice and will encourage the development of a culture of collaborative

research between education and practice.Students are required to have a previous

nursing degree or equivalent and who have accumulated appropriate experience in a relevant discipline area and who are currently registered with the Nurses and Midwives Board of Western Australia, to gain admission into these courses.

Registered nurses not holding an undergraduate degree must first complete a Graduate Certificate to proceed to higher Postgraduate studies.

Postgraduate Nursing at Notre Dame

HEALnet (Health Education And Learning Network) is an Australian internet-based online learning package for the nursing profession.

The package offers short, accessible online learning sessions that keep nurses up to date with specific medical practices and, consequently, up to date with their professional development requirements.

Karyn O’Reilly, Head Teacher, Nursing at Coffs Harbour Education Campus, who has led a team from North Coast TAFE that developed the package, says that she is delighted to see the offering steadily becoming more popular.

“Having been a busy, time-poor nurse, and nursing educator, I know what it is like to keep up with professional education requirements,” says Ms O’Reilly.

“I believe that HEALnet is the ideal solution - for many reasons. Each unit takes only around 60 minutes, it fulfils continuing education requirements, it delivers the most up-to-date information in nursing procedures and theory and you don’t have to spend time going to a classroom. Being online, it fits around each

individual’s schedule,” says Ms O’Reilly.The HEALnet program is available via

the HEALnet website www.healnet.edu.au and features a range of innovative online learning approaches.

“For example, after reading a professional written article, you can test your knowledge with an online quiz, which might include click-and-drag tests,” says Ms O’Reilly. “You can also get immediate results, and opportunities to re-do the evaluation.”

52 units are now available, with many more coming online in the future. One unit is free so you can trial HEALnet. Annual subscriptions are available for an introductory price for individuals at $160 (Normally $260), which can be paid online by the student giving them unlimited access to all units, vodcasts, presentations, the journal club and the Joanna Briggs Institute. MidwiferyWhat does a midwife do?Midwives take care of mothers before, during and after a baby is born. They help in the assistance of normal births, as well as being able to recognize and give emergency medical assistance with any problems that may arise until a doctor is present. Midwives are trained to recognize abnormal factors, as well as apply preventative measures in the baby and mother throughout their entire pregnancy and birthing experience. They also provide support, education and safety advice for mothers, as well as assisting in caring for the newborn infant after birth.

How do I become a midwife?You must be a registered nurse to be able to qualify for entry to a midwifery education program in Australia. Many universities offer midwifery education programs at a graduate certificate, diploma and masters level, which you need to complete to be able to practice within this specialized field of nursing.

What career opportunities exist for a midwife?Midwives can work in hospitals within women’s’ health or community settings. You can also work in private practices, or independently by offering care for

women within their own homes. As a midwife, you are equipped to give supportive advice in relation to family planning, women’s’ health during and after pregnancy and care of the infant. A midwife can become an educator or hold a management position within healthcare and health advisory facilities.

What does a Mental Health nurse do?Mental Health nurses monitor the biological dimensions of mental illness of their patients through medication administration, behavioural observations and interpretation. They must protect the patients’ rights and support their families and carers in line with current trends and reforms. There’s a variety of sub-specialties within mental health nursing, which include; community based rehabilitation, adolescent mental health, alcohol and drug abuse, as well as psycho-geriatrics. All mental health nurses do work with a variety of patients suffering from different illnesses. However, particular hospitals or health facilities may run specialized programs to focus on particular illness groups, such as; bipolar disorder, schizophrenia, depression, eating disorders, personality problems, traumatic reactions as well as patients with multiple diagnoses, such as mental and physical illnesses. A mental health nurse needs to have good communication, listening and interpreting skills, to be able to understand a patient’s condition and to be able to recognize what it is they are not saying, as well as what they are.

How do I become a Mental nurse?Firstly, an undergraduate degree in nursing needs to be completed, from which you can apply for a post graduate course of study in mental health nursing. These programs are available at select universities and some hospitals or health

care facilities may offer traineeships or clinical placements that you can complete with your post graduate degree.

What career opportunities exists?A career in mental health nursing is truly rewarding and varied. You could work in a community facility, in a ward, care home or secure setting. With each new setting comes a slightly varied set of skills and knowledge. For instance, if you were to work in the community or with the elderly, you would have additional training in relation to dementia to better your care for this particular group of patients.

What does a nurse practitioner do?A nurse practitioner is a registered nurse by qualification. This is a registered nurse who has undertaken a master’s level degree and is authorized to practise in Australia by the Australian Nursing and Midwifery Council. This role is an advanced and extended clinical role, which includes assessment and management of clients, prescribing medications and ordering diagnostics. A nurse practitioner must provide innovative and flexible knowledge and skills to produce flexible health care delivery that complements other health care providers. A nurse practitioner has the knowledge and expertise to make complex and critical decisions about their patients’ care. They are qualified to examine their patients, order tests, interpret test results to further decide the best treatment or therapy options and prescribe medications. The nurse practitioner’s role is based on the nursing profession’s values, knowledge, theories and practice.

How do I become a nurse practitioner?To become a nurse practitioner, you must already have a nursing degree qualification, be a registered

nurse, hold a current practising certificate, have a minimum of three years post-registration experience within a specialty area as well as be recognized as an advanced practice nurse within a specialized field with a demonstrated excellence achievement within that role. Once you have all the above criteria, you can qualify to study to become a nurse practitioner at universities that offer this course as a post graduate master’s degree.

What career opportunities exist?The clinical areas where nurse practitioners can practice are many and varied. You may prefer to practice in mental health, community health, rural and remote areas, neonatal, cardiac and cardiology, endocrine, paediatrics, the emergency department, chronic pain management and midwifery, just to name a few. Nurse practitioners already practice at an advanced level, and it is one of the highest qualifications you can have as a registered nurse.

EDITORIALin demand as Australians go on $1 Billion spree

COSMETIC SURGERYThe latest non-surgical procedures, laser

and botox treatments are steadily growing in demand, as heard at a global conference in Hobart.

It seems that Australians can’t get enough of cosmetic surgery treatments, as we are now splurging about $1 billion alone, with this figure expected to grow with the continuing popularity of cosmetic surgery.

The primary clientele for the surgeries includes the baby boomer generation, who are willing to do what it takes to reverse the ageing process, according to the Australian College of Cosmetic Surgeons, Dr Russell Knudsen.

Dr Knudsen believes that “they’re affluent, they’re educated and they’re living longer than ever”

The most popular surgeries include breast augmentation and liposuctions, but Dr Knudsen is saying that it’s the non-surgical treatments that can be performed in a lunch hour are really flourishing.

“Less invasive techniques have taken an increasingly prominent role in managing cosmetic surgical outcomes. In fact, there’s an enormous amount of work being done now in the face using fillers which used to be performed as surgical techniques,” he said.

The Vocational Graduate Certificate in Cosmetic Nursing is a government-accredited course, which provides Nurses with the knowledge and hands-on skills to work within a cosmetic medical practice performing cosmetic nursing, dermal therapies and injectables procedures.

The qualification is delivered through the Australasian Academy of Cosmetic Dermal Science (AACDS), which have been delivering education in the field of cosmetic medicine since 2004. The college and its qualifications have gained significant recognition from the key societies/organisations in cosmetic medicine.

AACDS Medical Director, Dr Jayson Oates reports, “Although Nurses have the foundation skills as a health care

professional, they lack the specific skills and knowledge to apply to the field of cosmetic medicine and dermal therapies. The Vocational Graduate Certificate in Cosmetic Nursing focuses on the role of the RN or EN in a cosmetic medical practice and/or accredited day hospital.”

The course is delivered online over one semester full-time or two semesters part- time. Practical competencies in dermal therapies (chemical peels, IPL, laser, micro dermabrasion, skin needling) and cosmetic injectable procedures (botulinum toxin and dermal fillers) are completed on-campus in Perth, Sydney or the Gold Coast.

Aged care nurses now have the opportunity to apply for one of the 130 scholarships for a postgraduate or continuing education within their field, on offer by the Federal Government.

The Minister for Mental Health and Ageing, Mark Butler, announced that this new round of scholarships will allow aged care nurses and health professionals to develop the critical skills required, which will include professional development to the value of $30,000

Mr Butler stated that “Eligible study areas include clinical gerontology, behavioural management, dementia care, wound care, continence care, palliative care, infection control and leadership and management. Registered nurses can apply for scholarships of up to $15,000 a year for two years for postgraduate studies.

Enrolled and registered nurses may receive continuing education scholarships of up to $3,000 to attend conferences, $5,000 for short courses or workshops and up to $6,000 over two years for re-entry courses.”

These scholarships became available through the Australian Government’s $232 million ‘Supporting Professional Aged Care Workforce Program’.

“These scholarships encourage aged care nurses to extend their skills so they can improve the clinical care they provide, or to further their career paths in aged care. Having access to further training and career advancement makes aged care a better career option and makes it easier to attract and keep quality staff, which in turn means better care for our increasing population of older Australians,” Mr Butler stated.

Mr Butler also added that a crucial element of the Gillard Government’s far-reaching reforms to the health system was to actually support the health and aged care workforce.

“Last month, all states and territories agreed to join the national reform partnership. These reforms will build on our current strengths and reshape the system to better meet the needs of patients and health care in the 21st century.”
