E7tension1 and Production. That 'CE 5ould maBe as an effecti!e reliable measure for

faculty ranBing among the public tertiary institution.

'ualitati!e Contribution E!aluation is a !alidating factor of CCE 5ith t5o le!els> 6irst

is 'CE for instructors1 assistant Professors and associate Professors is focused on

Step +. The faculty shall secure the 'CE forms for E7tension from theChairman of the College?:ni!ersity 'CE Team?Committee

Step -. The faculty shall be responsible for the distribution of the forms to the

clients.

Step 0. A 5itness should sign the 'CE forms1 preferably the head of the

organiation?agencies affirming the authenticity of the ans5ers of the

respondents and the !alidity of the e7tension program

proect?acti!ities.

Step 2. The duly accomplished 'CE 6orms1 together 5ith the authenticated

supporting documents1 should be submitted to the College?:ni!ersity

'CE Team?Committee.

Step *. The College?:ni!ersity 'CE Team?Committee shall e!aluate and

re!ie5 the documents submitted1 specially the signatures of the

clients and the 5itnesses. Specimens of their signatures should be

found in order and authentic.
