ON 111300 I RESPONDED TO THE INTERSECTION OF HAFFEN RANCH RD. AND MARY LOU
PERTAINING TO A HELICOPTER CRASH INVOLVING FLIGHT FOR LIFE, LIFEGUARD 2.
UPON MY ARRIVAL AT APPROXIMATELY 2112 HRS. DEPUTY J. MOORE, DEPUTY R. ROBERT
DEPUTY S. ARRENDALE, DEPUTY SGT. B. BALDING AND ASSISTANT SHERIFF R. MARSHALL WERE
AT THE SCENE ALONG WITH PAHRUMP FIRE AND RESCUE AND AMBULANCE SERVICE.
I FIRST OBSERVED A FLIGHT FOR LIFE HELICOPTER ON ITS RIGHT SIDE TO THE EAST OF HAFFEN RANCH RD. PAHRUMP FIRE AND RESCUE AND AMBULANCE SERVICE PERSONNEL WERE
AROUND THE HELICOPTER ASSISTING CREW MEMBER OUT OF THE HELICOPTER AND THE FIRE
CREW WAS MONITORING THE AREA AROUND THE HELICOPTER FOR POSSIBLE FIRE.
THE SCENE WAS SECURED ONCE THE CREW WAS REMOVED AND THE NYE COUNTY SHERIFF'S

AT APPROXIMATELY 2116 HRS. I WAS ABLE TO CONTACT RICHARD LOUGHRY WHO WAS A
PASSENGER IN THE HELICOPTER. HE AGREED TO AN INTERVIEW WITH ME.

MR. LOUGHRY STATED HE WAS THE FORWARD FACING CREW MEMBER AS A PARAMEDIC. HE GAVE
ME A TAPED STATEMENT PERTAINING TO HIS OBSERVATIONS AS THE FLIGHT WENT DOWN.

A WRITTEN STATEMENT WAS THEN OBTAINED FROM JAMES S. PERRY WHO WAS PART OF THE
AMBULANCE CREW WHO WAS ON THE SCENE AND HAD REQUESTED LIFEGUARD 2 TO RESPOND TO
THERE LOCATION PRIOR TO THE ACCIDENT. MR. PERRY HAD SET UP THE LANDING ZONE FOR 1 ,TFEGIlARn 7.
ACCORDING TO MR. PERRY, HE HAD THE LANDING ZONE SET UP AND THE HELICOPTER WAS
EXPECTED TO LAND ON MARY LOU EAST OF THE AMBULANCE. AS THE LANDING ZONE WAS
ESTABLISHED, A SMALL CAR DROVE ONTO MARY LOU FROM THE EAST IN A WEST BOUND
DIRECTION. MR. PERRY STOPPED THE VEHICLE IN FRONT OF THE SECOND RESIDENCE WEST OF
HAFFEN RANCH AND PRIOR TO THE LANDING ZONE. THERE WAS COMMUNICATION WITH THE
PILOT WHO WISHED THE VEHICLE BE MOVED AND WHEN MR. PERRY WAS RELAYING THIS
INFORMATION TO THE DRIVER OF THE VEHICLE, HE OBSERVED LIFEGUARD 2 TURN HARD TO THE
EAST FROM ITS POSITION OVER THE HOUSES, POINTING SOUTH. THE HELICOPTER APPEARED
TO GAIN IN ELEVATION WHILE MAKING THE TURN THEN DROPPING PAST THE HOMES TOWARDS
THE POWER LINES A N D HE LOST SIGHT OF IT. THE HELICOPTER THEN CAME BACK UP INTO
HIS SIGHT AND IT APPEARED TO BE POINTING SOUTH AGAIN AND THEN IT DROPPED OUT OF
SIGHT AGAIN. MR. PERRY THEN HEARD THE CRASH AND SAW DIRT A N D DEBRIS FLYING UPWARD L. V I

5960 MARY LOU BLOCKING TRAFFIC ON THE EAST END OF THE LANDING ZONE. THE TRACKS ON
THE SHOULDER OF THE ROAD FOR THE VEHICLE IN QUESTION ARE IN FRONT OF 5960 MARY LOU
AND APPROXIMATELY 201'4" EAST OF THE AMBULANCE WHICH WOULD ALLOW A SPACE EXCEEDING
200' FOR THE FLIGHT FOR LIFE HELICOPTER TO LAND. THE HELICOPTER WAS IN THE DESERT
TO THE EAST OF HAFFEN RANCH ROAD AND TRIAGULATION MEASUREMENTS FROM MARY LOU AND
GRAIN MILL ROAD (FIRST STREET NORTH OF MARY LOU AND INTERSECTING WITH HAFFEN RANCH
RD.IARE APPROXIMATELY 296'4" SOUTH/EAST OF GRAIN MILL RD. AND APPROXIMATELY 324'1"
NORTH/EAST OF MARY LOU. THE FARTHEST DISPLACED PIECES OF HELICOPTER WERE A SMALL
SECTION OF ROTOR BLADE APPROXIMATELY 133'4" TO THE SOUTH OF THE TOP CENTER PORTION
OF THE HELICOPTER AND A SECTION OF PLEXIGLAS WAS APPROXIMATELY 137'5" WEST OF THE
FRONT OF THE HELICOPTER. THERE ARE POWER LINES POSITIONED PARALLEL TO HAFFEN
RANCH ROAD CONSISTING OF FOUR ( 4 ) WIRES OF WHICH THE CENTER ONE IS APPROXIMATELY
33'8" FROM THE EAST EDGE OF THE ASPHALT OF HAFFEN RANCH RD.
I OBTAINED A DIAGRAM FROM SCOTT PERRY DESCRIBING WHAT HE HAD OBSERVED.
ON 112000 I LOCATED THE DRIVER OF THE VEHICLE THAT WAS ON MARY LOU THE NIGHT OF THE ACCIDENT. THE NAME OF THE DRIVER IS PEGGY TUDOR. MS. TUDOR AGREED TO GIVE ME
A WRITTEN STATEMENT OF HER ACTIONS AND OBSERVATIONS.
ACCORDING TO MS. TUDOR, ON MONDAY NOVEMBER 13, 2000 AT APPROXIMATELY 2030 HRS. SHE
RECEIVED A TELEPHONE CALL AT HER RESIDENCE FROM HER STEPFATHER THAT HER MOTHER WAS
GOING TO GO BY AMBULANCE DUE TO A MEDICAL PROBLEM. SHE LEFT HER RESIDENCE AND
STOPPED TO GET HER DAUGHTER AND THEN WAS ENROUTE TO 5920 MARY LOU. WHEN SHE
TURNED HER VEHICLE ONTO MARY LOU, SHE SAW A "MAN IN THE STREET" AND SHE STOPPED
THE VEHICLE IN FRONT OF THE SECOND RESIDENCE WEST OF HAFFEN RANCH RD. THE MAN
APPROACHED HER VEHICLE WHILE TALKING ON A PORTABLE RADIO. THE MAN TOLD ME "I
THINK YOU MIGHT HAVE TO MOVE". HE WAS TALKING ON THE RADIO SO SHE WAITED. MS.
TUDOR OBSERVED THE MAN LOOKING UP IN THE SKY AND SHE COULD HEAR A HELICOPTER OVERHEAD, THEN SHE HEARD A CRASH. MS. TUDOR ADVISED SHE NEVER DID SEE THE
HELICOPTER UNTIL AFTER THE CRASH. SHE RAN TO THE HELICOPTER AND SHE OBSERVED
& CONTINUATION FOLLOW-UP SUPPLEMENT
ACCORDING TO MS. PETERSON, SHE HAD BEEN PICKED UP BY HER MOTHER THE NIGHT OF THE
INCIDENT AND WAS ENROUTE TO HER GRANDMOTHER'S RESIDENCE ON MARY LOU. THEY WERE
AWARE THAT FLIGHT FOR LIFE WAS ENROUTE SO SHE WAS WATCHING FOR THE HELICOPTER. AS
THEY WERE TRAVELING SOUTH ON S.R. 160 SHE COULD SEE THE HELICOPTER TO HER WEST AND
AT THAT TIME IT WAS A "BIT BEHIND US". ONCE THEY TURNED ONTO HAFFEN RANCH, SHE
LOST SIGHT OF THE HELICOPTER. THEY CONTINUED SOUTH ON HAFFEN RANCH RD. AND TURNED
RIGHT (WEST) ONTO MARY LOU AND SHE SAW THE AMBULANCE IN FRONT OF HER GRANDMOTHER'S
RESIDENCE (5920 MARY LOU). THEY PULLED THE VEHICLE OFF TO THE SIDE OF THE ROAD TO
ASK THE MAN WHO WAS STANDING THERE IN THE ROADWAY IF THEY WERE OUT OF THE WAY. HE
WAS AT THE DRIVER'S SIDE WINDOW TALKING ON HIS RADIO. "ALL OF A SUDDEN" SHE HEARD
THE HELICOPTER CRASH BEHIND THE VEHICLE. MS. PETERSON ADVISED THAT SHE DID NOT
HEAR OF SEE ANY HELICOPTER ATTEMPT TO LAND ANYWHERE IN THEIR AREA PRIOR TO THE
CRASH. SHE DID NOT FEEL ANY DOWN DRAFT OR DIRT MOVING IN FRONT OF THEM WHICH SHE
KNOWS IN COMMON IN THIS AREA WHEN HELICOPTERS LAND.

ON 112200 I HAD CONVERSATION WITH VIC SPANKOWSKI WHO IS A COMMERCIAL HELICOPTER
PILOT. I EXPLAINED TO HIM THAT I HAD SOME TECHNICAL QUESTIONS PERTAINING TO THIS
ACCIDENT BECAUSE MY KNOWLEDGE WAS LIMITED PERTAINING TO AIRCRAFT ACCIDENTS. HE
ADVISED ME THAT HE HAS DONE INVESTIGATIONS FOR THE FAA AND AGREED TO ADDRESS MY
CONCERNS. AFTER REVIEWING THE STATEMENTS OF SCOTT PERRY AND LOOKING AT THE
PHOTOGRAPHS OF THE INSTRUMENT PANEL OF THE HELICOPTER HE AGREED TO SUBMIT A REPORT

THE AFOREMENTIONED MALE, WHO WAS IDENTIFIED AS THE PILOT, STARTED TO FALL TO
THE GROUND. I ASSISTED HIM INTO A SITTING POSITION AND ASKED WHAT HAPPENED.
THE PILOT STATED THAT HE WAS APPROACHING THE LANDING ZONE NEAR MARY LOU AND HAFEN RANCH, WHEN A CAR PULLED IN FRONT OF HIM. HE SAID WHILE HE WAS ATTEMPTING TO AVOID THE CAR, HE SAW THE ELECTRIC LINES ON THE EAST SIDE OF THE ROADWAY. HE SAID
HE AVOIDED THEM BUT COULD NOT AVOID THE CRASH.
THE PILOT, FLIGHT NURSE, AND FLIGHT MEDIC WERE TRANSPORTED TO LAS VEGAS, NAMES
ARE UNKNOWN.
I SPOKE WITH PARAMEDIC BELLUOMINI AND FIREFIGHTER/ EMT SCOTT PERRY WHO WERE ON
THE AMBULANCE WHICH REQUESTED THE HELICOPTER. MR. BELLUOMINI STATED HE SAW A
VEHICLE NEAR THE AFOREMENTIONED INTERSECTION BUT COULD NOT DESCRIBE IT DUE TO WHAT
OCCURRED WITH FLIGHT FOR LIFE.
UGH, SOP SHE CONTINUED NORTH. SHE
SAID SHE SAW THE HELICOPTER MAKE A RIGHT TURN AS IF GOING TO CIRCLE THEN CRASH INTO THE FIELD. I ASKED IF A FIRE DEPARTMENT MEMBER STOPPED HER ALONG THE ROADWAY.
