Sunnybrook’s MyChart™ A Brief Overview

Sunnybrook’s MyChart™ program is a patient-driven personal health record solution that was developed in-house and launched in February 2006. At the time, MyChart™ was one of Canada’s first PHR solutions and over the course of the past 10 years it has evolved into a mature, credible, and proven platform.

MyChart™ enables patients, families, caregivers, and clinicians across the continuum of care to participate in personal health management.

Our patients can view and trend their test results (labs, radiology images, and pathology), view clinical reports, request and schedule appointments, submit consult assessment forms and questionnaires, and document their personal self-entered health information all in one record view.

Data available is not limited to Sunnybrook – our users can view data from any participating hospital or community care site such as Mount Sinai Hospital, Michael Garron Hospital, William Osler Health System, etc.

MyChart™ Users are empowered with sharing capabilities – they can delegate access from anywhere, at any time and the process is completely controlled and managed by the User.

Through MyChart™, patients are able to self-manage and take “ownership” of their health management needs – patients become more knowledgeable of their overall health, manage conditions, and follow treatment which helps keep them connected to their health status.

The program is continuously expanding, adding features, and developing new solutions in close partnership with our clinicians.

Sunnybrook’s MyChart™ Changing the hospital landscape

Many organizations across the province use their own electronic systems that are not capable of integrating with one another - MyChart™ is able to integrate with various data sources to provide patients a consolidated view of their medical record in addition to the patient’s own personal information which is essential to make informed care decisions

MyChart has significantly increased the level of engagement and participation of patients in health care management

There are ~45,000 logins to the system every month. Patients are able to access information that typically would have been retrieved in person through the release-of-information (ROI) process

When patients visit Health Records in person, they have the opportunity to enroll in MyChart instead of waiting for hospital staff to make physical copies of their record, which may take up to 30 days to process.

A Patient Story

• Judith Morley, a Sunnybrook patient, was aboard a plane on her way to Mexico when she began to experience intense abdominal pains

• A physician on board the flight strongly suggested to make an emergency landing and she was rushed to a local hospital

• Doctors at the hospital learned she had a history of cancer and had undergone multiple surgeries

• Judith informed the doctors that she was a Sunnybrook patient and had an online service called MyChart™ that allowed her to access her medical record from the hospital in Canada.

• Doctors were able to make decisions quickly about her care, avoiding surgery and the potential complications that could have arisen. Judith credits MyChart™ with saving her life.
