Public Education Coordinators offer dementia specific education to professionals who work with people who have dementia. Programs are flexible and geared toward the learning needs of different groups including personal support workers, activationists, nursing, dietary and housekeeping staff, students, volunteers, and visitors. Topics include an overview of AD, communicating with people who have ADRD, responding to responsive behaviours, working with families, an aging simulation, activities of daily living, and therapeutic activities. Conducting an assessment, planning and coordinating services to meet a senior’s needs, goals and expected outcomes. Our comprehensive social work program provides practical support to caregivers coping with Alzheimer’s diseases and related disorders. In supporting the family caregivers we are providing education about the disease, coping techniques for the caregivers and providing therapeutic counselling. The Alzheimer Society of York Region’s social workers play an integral role in assisting families in navigating throughout the course of the disease. Mental Health Diversion is defined as a pre-trial procedure where crown attorneys can, at their discretion, decide not to prosecute when the charge is a minor offense and major mental illness is determined as the underlying cause. Instead, the accused is referred to appropriate mental health services in the community, professional treatment and support to reduce the chances of re-offending. Furthermore, if the individual meets specific criteria he or she may also qualify for short-term case-management. A Court Worker or Case Manager will work with the client to develop an individual service plan. The plan will outline the treatments and services that the individual will be expected to utilize. All referral for Mental Health Diversion must go through a 6-week screening process to

The program supports individuals who are suffering from serious mental illness and who live or work in York Region. Individual, couple, family and group therapy are provided. The Mood Disorders Clinic (MDC) provides multidisciplinary and multi-modal clinical care and education for patients, families and professionals with diagnoses of depression or bipolar disorder. Services include psychiatric assessment, diagnosis, treatment recommendations and brief follow-up where appropriate. Where patients are referred back to the referring physician with recommendations, the clinic physicians will function as a back up and resource to the treating physician in the community and a repeat consultation can be requested. Involvement of non-medical clinical staff in treatment plans is designed to best meet the needs of individual patients. If direct patient care, particular research interests include early intervention and exploring the gene/environment interaction.

Problem Gambling Community Treatment Service- Etobicoke

Provides group and limited one-to-one counselling for problem gamblers, and support for family members, who may be seen with or without the individual with the gambling problem. Counselling is available in person and by phone, and is generally solution-focused, using cognitive-behavioural interventions, relapse prevention and harm reduction strategies. The services include open-cycle Preparation groups, closed-cycle 8-week Skills for Change groups, open Aftercare groups, and other groups designed for specific issues and concerns. Couple and family counselling is available. Some staff members specialize in specific populations: women, youth, older adults and ethnocultural communities. Staff also provide professional training, outreach and public information sessions on problem gambling issues, treatment and prevention. Problem gambling treatment services are provided in downtown Toronto at 33 Russell Street, and at two satellite offices in Scarborough and Etobicoke (limited service). Psychogeriatric Assessment Consultation and Education (PACE)

Community-based outreach teams that provide a range of services to seniors. The clinics provide assessment and psychogeriatric consultation in the client's own environment. For registered clients, counselling, support, medication monitoring and home visits are all components of the psychiatric follow up. The clinics also provide crisis services during clinic hours, consultation to community agencies, advocacy, education and support to the clients¿ family members and community education. Community Treatment Orders (CTO) provides a legal mechanism for people with serious mental illness to receive treatment, support and supervision in the community. In partnership with the client and service providers, a comprehensive plan is developed that is less restrictive than being hospitalized. This program is for people who experience recurrent hospitalizations, benefit from treatment and usually do not engage in outpatient follow-up. Only physicians can initiate a community treatment order, which is limited to six months in duration. CMHA/Peel Resource Center

Information and Referral-Mental Health/Mental Illness CMHA/Peel offers a Mental Health Resource Centre, which serves as a welcoming point of entry to the mental health system for people living and/or working in Brampton, Caledon, and Mississauga. CMHA/Peel –Resource Center hours of operations is 9am to 4:30pm, Mondays to Fridays. CMHA/Peel will be able to help individuals from Etobicoke, Woodbridge and Dufferin by providing them with information of services and if need make referrals to services in there area. Mental Health is defined as knowing and accepting yourself, building meaningful relationships, coping with problems day to day living, and maintaining a sense of humor. It also means striking a balance in all aspects of your life: social, physical, spiritual, economic and mental Mental Illness is a generic term for a variety of diagnosable illnesses, based on set criteria of symptoms. Mental illness affects all aspects of a person’s life: work, family, relationships, school, and recreation. It affects everyday life. The primary functions of the Certified Information and Referral Specialist at CMHA/Peel is to provide information around Mental Health and/or Addictions Inquiries. CMHA staff are educated and trained to provide information, appropriate referrals to local Mental Health and Addiction Services, brief services or interventions, follow-ups and can begin the intake process for CMHA/Peel and other programs. CMHA/Peel Certified Information and Referral Specialist are equipped to take calls from individuals whom may have an enquiry of a family member or friend with a mental illness, or they have been diagnosed with a Mental Illness. This includes Anxiety Disorders, Mood Disorders, Personality Disorders, Psychosis, Schizophrenia,

self injury and Concurrent Disorders (Addictions and Mental Health). In regards to Dementia, Alzheimer’s CMHA/Peel is familiar with programs in the Peel Region and would link the caller to the various services. Should an individual call in and is enquiring about information in the area of Mental Health/Mental Illness regarding services for Mental Health , CW CCAC can connect/link the client or family to a CMHA Certified Information and Referral Specialist at (905) 451-2123. There is always a live person to take these calls but from time to time all lines may be busy. There is the ability to leave a message and a call back will happen within the business day. CMHA/Peel Certified Information and Referral Specialist are trained to work with individuals whom may be encountering a situation or a series of problems that feel overwhelming and out of control; or the individual has a mental illness and are overwhelmed in coping with there activities of daily living; or the individual expresses an inability to see options, choices that may be helpful. CW CCAC may link the client through a warm transfer to CMHA/Peel Resource Center to de-escalate the individual and then assist the individual in putting a safety plan in place. When an individual is in acute distress, CMHA/Peel will involve the services of Peel Mobile Crisis or 911. Similar practice is in place for CW CCAC. With client’s/ family verbal consent, CMHA/Peel Certified Information and Referral Specialist will do a follow up call back to inform a CW CCAC Case Manager as to what information was provided and/or what referrals were made
