What happened?The incident occurred while a carpenter working for Al Turki was putting back the railings on the trailer after unloading. His right little finger got caught between the railing and the railing slot on the trailer causing the injury.

What happened?The incident occurred while L&T (Larson and Tubro Electromech) driver (Subcontracted to IECC) was travelling from Yibal to Muscat in a Mitsubishi pickup with three passengers. The pickup collided with a Galfar shovel that was entering the main road from the contractor’s camp at a T junction. The driver suffered a broken leg. One of the passengers sustained only slight injuries while the other two passengers escaped without any injuries.

What happened?The incident occurred while a welder working for Shaleem Petroleum Co. was loosening the spring leaf centre bolt of a water tanker with spanner/pipe wrench in the Shaleem Workshop. While rigging up jacking frames, the Schlumberger Field Supervisor was giving signal to lower the top frame onto the bottom one, and his arm got trapped in between the two frames causing crush injuries to his forearm

What happened?The incident occurred when Al-Maqshan & Athwa Haima Company (Subcontracted to TISCO) crew were off loading the waste from a closed skip box. The helper opened the skip door upwards which suddenly closed down again hitting his thumb resulting in crush injuries and fractured thumb.

What happened?The incident occurred when the crew were running cement stinger. While making the connection, the 2” wash down hose disconnected above the low torque 2” hammer union and fell down from a height of 10m. The hose hit the Shiv-Vani Drilling Co. Assistant Driller on the shoulder. He fell to the floor hitting his head against the slip handle. He sustained cut wounds to his forehead.

Fatality (23 Oct 06)

What happened?

Mr. Donald McPherson, a 39 year old British Fishing Engineer employed by United Engineering Services (UES) was driving from Nimr to Rig 88 in Harweel. At about 47.1 km from Marmul roundabout on the Marmul -Harweel blacktop road, he lost control of the vehicle and rolled over 6 times. He suffered severe head injuries and died on the spot. Our thoughts are with the family and friends of the deceased.

Fatality (17 Oct 06)

What happened?

A Bahwan Exel Heavy Goods Truck driven by Mr Faisal Bin Mohammed Al Maskery, was transporting casings from Jabel Ali Dubai to Muscat. His vehicle was stopped by the police for routine roadside checks. As he was waiting to return to the cab, Mr Maskery was struck by a third party Canter truck which collided with the rear of his vehicle. He sustained fatal head injuries and died on the spot. The policeman managed to jump to his safety. The 3rd party driver is hospitalised. Our thoughts are with the family and friends of the deceased.

What happened?The incident occurred when fire broke out decommissioning the mud tanks prior to the rig move. The Precision Drilling Mud Checker and Roustabout both sustained burns when they were removing the suction line in between the suction tank and reserve tank.

off UBD Valves prior to completing the Rig flush.

• Always make sure that a gas test is done, all enclosed spaces/ lines are opened and vented, and PTW is obtained before initiating any hot work.

• All Welding on Tanks must be done after Rig Move has completed (i.e. when the tank is in COMPLETE isolation and is out of the vicinity of any source of crude)

What happened?The incident occurred while the crew were working on a prototype pipe testing fixture. The Equipment Operator caught the sleeve of his coveralls on the cycle/rpm counter arm which is attached to a rotating shaft. The counter arm wrapped his sleeve around the shaft ultimately pulling him violently around the equipment stand.

What happened?

The incident occurred during a BBQ organized by Schlumberger in Fahud (outdoors). Mr. Ahmad Bakti, a 30 year old Indonesian was trying to take photos with a colleague. He was positioning himself for a better view and fell off the cliff.

A group of colleagues went down to search and rescue him. He was taken to Fahud clinic where he was pronounced dead.

Our thoughts are with his family, colleagues and friends.

What happened?A group of six employees subcontracted to SOCAT (a PDO Contractor) were travelling in a Land-Rover on the black top road from Lekhwair to Muscat. The right rear tyre burst and the vehicle rolled over. Mr. Surendran died on the spot.Our thoughts are with his family, colleagues and friends.

What happened?An FMC service engineer had worked most of the night, rested for 2-3 hrs and then drove his car from Saih-Rawl to Muscat. Just before Nizwa he fell asleep, hit a street light and rolled-over. There were several heavy objects kept in the passenger compartment. The injuries were all caused by those flying objects.

What happened?A Galfar mechanic tried to stop a welding generator. The button was difficult to move so he used pliers. The pliers slipped and his hand jerked forward into the rotating radiator fan.

What happened?An Al-Hassan canter had a head-on collision with a third party vehicle on the Muscat-Salalah main road 115 km south of Qarn Alam just after mid-day (12.30). The driver was sleepy. He veered slowly towards the right hand side of the road. When he realised this he overcorrected to the left straight into the path of the oncoming third party saloon car.

What happened?A PDO employee was cutting metal strips using a hydraulic cutting machine. While setting up the metal strip that had to be cut, he accidentally operated the machine with his foot and trapped his finger which was placed under the shear.
