Cardiorespiratory fitness (CRF) is not only an objective measure of habitual physical activity, but also a useful diagnostic and prognostic health indicator
for patients in clinical settings.
Although compelling evidence has shown that CRF is a strong and independent predictor of all-cause and cardiovascular disease mortality, the importance of CRF is often overlooked from a clinical perspective compared with other risk factors such as hypertension, diabetes, smoking, or obesity. Several prospective studies indicate that CRF is at least as important as the traditional risk factors, and is often more strongly associated with mortality. In addition, previous studies report that CRF appears to attenuate the increased risk of death associated with obesity. Most individuals can improve their CRF through regular physical activity. Several biological mechanisms suggest that CRF improves insulin sensitivity, blood lipid profile, body composition, inflammation, and blood pressure. Based on the evidence, health professionals should encourage their patients to improve CRF through regular physical activity.

AbstractOBJECTIVE: To quantify the association between walking and the risk of cardiovascular disease (CVD) and all-cause mortality in healthy men
and women.DATA SOURCES: Medline, Cochrane Database of Systematic Reviews, and Web of Science databases were searched to May 2007. Study selection:
Prospective epidemiological studies of walking and CVD and all-cause mortality.RESULTS: 18 prospective studies were included in the overall analysis, which incorporated 459 833 participants free from CVD at baseline
with 19 249 cases at follow-up. From the meta-analysis the pooled hazard ratio of CVD in the highest walking category compared with the lowest was 0.69, (95% CI 0.61 to 0.77, p<0.001), and 0.68 (0.59 to 0.78, p<0.001) for all-cause mortality. These effects were robust among men and women, although there was evidence of publication biases for the associations with CVD risk. Walking pace was a stronger independent predictor of overall risk compared with walking volume (48% versus 26% risk reductions, respectively). There was also evidence of a dose-response relationship across the highest, intermediate, and lowest walking categories in relation to the outcome measures.

AbstractBACKGROUND: Current recommendations prescribe that every adult should accumulate 30 min or more of moderate-intensity physical activity in
leisure time, preferably all days of the week. To further support these recommendations we examined the impact of walking intensity and walking duration on all-cause mortality.
DESIGN: Relative intensity and duration of walking were recorded in 7308 healthy women and men aged 20-93 at the third examination
(1991-1994) of the Copenhagen City Heart Study. During an average of 12 years of follow-up 1391 deaths were recorded.RESULTS: For both sexes we found a significant inverse association between walking intensity and risk of death, but only a weak inverse
association to walking duration. For women walking with average intensity, the adjusted hazard ratio (HR) of death was 0.75 [95% confidence interval (CI) 0.61-0.92; P<0.01] and walking with fast intensity 0.48 (95% CI 0.35-0.66; P<0.001) compared to women walking with slow intensity. For men the relative risks were 0.54 (95% CI 0.45-0.67; P<0.001) and 0.43 (95% CI 0.32-0.59; P<0.001), respectively.
