Dasar Molekuler Kanker
Kerusakan genetika nonletal adalah inti dari
karsinogenesis
Agen- agen lingkungan (kimiawi, radiasi atau
virus)
Defek didapat dari agen eksogen atau produk-
produk endogen dari metabolisme sel.
Tidak semua mutasi diinduksi oleh lingkungan,
beberapa terjadi spontan.
Kelainan sel embrional / sel ovum dan sperma/
germ line yang diwariskan

Suatu tumor dibentuk oleh ekspansi klonal dari satu
sel prekursor dengan kerusakan genetika yang
tidak dapat diperbaiki (Tumor adalah monoklonal)
Empat kelompok gen regulator normal
1. Protoonkogen pendorong pertumbuhan,
2. Gen pensupresi tumor penghambat pertumbuhan,
3. Gen yang meregulasi kematian sel terprogram
(apoptosis),
4. Gen yang terlibat dalam perbaikan DNA – adalah
target prinsip dari kerusakan genetika
Karsinogenesis adalah proses bertahap- tahap baik di
level fenotipe dan genetika.

Perubahan Esensial Transformasi
Maligna Mampu memproduksi signal pertumbuhan: tumor
mampu berproliferasi tanpa stimuli eksternal,
biasanya sebagai konsekuensi aktivasi onkogen
Insensitifitas terhadap sinyal inhibisi pertumbuhan:
tumor tidak berespon terhadap molekul yang
menginhibisi proliferasi sel normal dan inhibisi
langsung dari cyclin-dependent kinases (CDKIs)
Mengelak apoptosis: tumor resisten terhadap
kematian sel terprogram, sebagai konsekuensi
dari inaktifasi p53 atau aktifasi gen anti apoptosis.

Potensial replikasi tidak terbatas: menghindari
cellular senescence dan kekacauan mitosis
Angiogenesis yang terus menerus
Kemampuan menginvasi dan bermetastasis
Gangguan dalam perbaikan DNA. Tumor gagal

instabilitas genom dan mutasi protonkogen dan

Siklus Sel Normal
Untuk memahami sifat alamiah dan fungsi onkoproteindan perannya dalam kanker, perlu memahami tahapanproloferasi sel normal.
Ikatan growth factor pada reseptornya
Aktivasi sementara dan terbatas reseptor faktorpertumbuhan, di mana , pada gilirannya, mengaktifkanbeberapa protein transduksi sinyal pada bagian dalammembran plasma.
Transmisi dari sinyal transduksi dari sitosol ke dalamnukelus melalui second messengers atau melaluikaskade melekul- molekul transduksi sinyal
Induksi dan aktivasi nuclear regulatori factors yang menginisiasi transkripsi DNA.

Siklus sel berperan dalam rangkaian kejadian
menghasilkan duplikasi DNA dan pembelahan sel.
Fase siklus sel telah diketahui selama 40 tahun,
tetapi hanya tahun- tahun terakhir gen yang terlibat
dalam siklus sel diidentifikasi.
Siklus sel dibagi menjadi 4 tahap: fase gap
pertama (G1), replikasi DNA (S), fase gap kedua
(G2), fase mitosis (M).
Panjang siklus sel bervariasi, sekitar 16- 24 jam.

Pada fase G1, sel- sel dipersiapkan mensitesis DNA dan biositesis RNA dan protein
Fase S, DNA bereplikasi dan sintesis histon. Akhirfase S DNA sel menjadi dua, dan kromosombereplikasi.
Fase G2, sel- sel bersiap- siap untuk pembelahansel, replikasi komplek DNA.
Fase mitosis inti dan sitoplasma membelah, menghasilkan dua sel turunan. Yang kemudianmasuk dalam siklus sel baru jika kondisinyamenunjang pertumbuhan selanjutnya.

Tanpa adanya mitogen atau deplesi nutrisi, sel
masuk dalam fase istirahat disebut fase G0.
Transisi dari satu fase ke fase berikutnya
diregulasi, beberapa checkpoint yang mencegah
dimulainya fase berikutnya sel secara prematur
dari siklus sel.

Saat sel berkomitmen untuk membelah, mencapai
akhir fase G1, menjadi refrakter terhadap faktor
pertumbuhan penginduksi sinyal dan inhibitor faktor
pertumbuhan seperti TGF dan bergantung pada kerja
protein- protein siklus sel untuk meregulasi progresi.
Progresi dari 1 tahap ke tahap berikutnya dikontrol
secara ketat oleh pembentukan, aktivasi dan degradasi
atau modifikasi dari rangkaian siklin dan pasangannya,
the cyclin-dependent kinase inhibitor (CKIs) penting
untuk transduksi sinyal dan koordinasi dari setiap
tahap siklus sel.

Beberapa checkpoint yang telah diketahui yang
mengevaluasi integritas DNA dan pembentukan
spindle dan spindle pole.
Defek DNA itu sendiri menyebabkan penyusunan
ulang kromosomal dan pengiriman DNA yang
rusak,
Kerusakan spindel dapat menyebabkan mitotic
nondisjunction, suatu proses menghasilkan
perolehan atau kehilangan seluruh kromosom

Check poin I. START atau restrictionpoint (R) danterjadi pada akhir dari fase G1.
Pada tahap ini sel harus mempersiapkan untukreplikasi DNA dan pada poin ini setiap kerusakan DNA dapat dimonitor dan jika perlu siklus selnya dihentikansampai telah diperbaiki.
Hilangnya checkpoint ini dapat menyebabkaninstabilitas genomik dan ketahanan hidup sel yang rusak.
Chekpoin lanjut II terjadi selama fase S dan pada G2 sampai transisi M pada monitor sintesis DNA danpembentukan dari spindel fungsional

Checkpoint terakhir melindungi segregasi
kromosom jika kromosom tidak intak.
Degradasi dari berbagai siklin terjadi pada setiap
checkpoint dan mekanisme ini bersamaan dengan
interaksi CKI yang memungkinkan sel memasuki
fase berikutnya.

Pertumbuhan
Onkogen: gen yang mengalami mutasi, pendorong
pertumbuhan sel secara autonom dalam sel kanker
Protoonkogen: gen yang tidak mengalami mutasi
Onkoprotein: protein produksi onkogen tanpa
elemen regulasi interna yang penting, dan proses
produksinya tanpa tidak bergantug pada faktor-
faktor pertumbuhan atau sinyal eksternal lainnya
Dengan jalan inilah pertumbuhan sel menjadi
autonom lepas dari pengontrolan dan
ketergantungan terhadap sinyal eksternal.

Protoonkogen memiliki berbagai peran, dalam fungsiseluler berhubungan dengan pertumbuhan danproliferasi.
Protein mengkode protoonkogen mungkin berfungsisebagai faktor pertumbuhan yang berikatan denganreseptornya, transduksi sinyal, faktor- faktortranskripsi, atau komponen siklus sel.
Onkoprotein dikode oleh onkogen umumnya berfungsimirip dengan protein sejenis.
Mutasi mengubah protoonkogen menjadi onkogenseluler yang aktif yang terlibat dalam pertumbuhantumor, karena onkoprotein mengkode sel denganpertumbuhan yang tercukupi sendiri.

Oncogenes are mutated forms of normal cellular genes involved in growth signalling
pathways (proto-oncogenes). When these genes become mutated the cell does not
require the presence of pro-growth signals (e.g. growth factors) in order to undergo cell

Tumour suppressor genes are genes often involved in the apoptotic pathway. Normally
tumour suppressors detect breaks or defects in the DNA – if present in low
concentrations these proteins will pause the cell cycle and active DNA repair
mechanisms. If present in high concentrations, tumour suppressors shut down the cell
cycle or cause apoptosis. When these genes are mutated to be dysfunctional then the

tumbuh dan berpotensi letal memerlukan lebih dari
satu perubahan genetika.

3. Persisten
• Tahap persisten dicapai saat proliferasi klonal dari
sel- sel tumor tidak lagi membutuhkan adanya
inisiator atau promoter, dan sel- sel tumor
menunjukkan pertumbuhan terautonomisasi.
• Pada awalnya terjadi perubahan ekspresi genetika
(seluler atau protoonkogen) menyebabkan sel tidak
berfungsi dengan baik dan menyebabkan sel
mampu tumbuh dengan stimulasi autokin.

Tahap lanjut, proses yang melibatkan
induksi pertumbuhan vaskuler ke dalam
tumor melalui faktor angiogenik tumor,
sehingga pertumbuhan tumor dapat
ditopang ole perfusi dengan nutrisi, dan jika
tumor maligna, pembentukan tumor
sekunder oleh proses metastasis.
