Seven Conditions Often Confused with Alzheimer’s DiseaseBy Ava M. Stinnett

When a loved one starts to exhibit signs of dementia, it can be difficult to know what to do first. Are thesigns typical of the normal aging processor, or is it cognitive decline that indicates the onset ofAlzheimer’s disease or another form of dementia? Alzheimer’s is not the only condition that affectsbrain functionality. How does one get an accurate diagnosis when there are numerous other medicalissues that mimic dementia that are treatable and, perhaps, reversible? Let’s take a look at someconditions that can be confused with dementia.

Infection. A number of different types of infection can be confused with Alzheimer’s disease. Thisincludes urinary tract infections, skin infections, and lung infections like pneumonia.

Medications. Many popular medications come with some significant side effects. These medicationscan affect both mental and physical well-being. For example, antidepressants can make patientsappear tired, both mentally and physically.

Dehydration. Dehydration can significantly reduce normal body functionality and can also negativelyaffect cognitive functionality. To prevent confusion, experts typically recommend that we drink morewater during the hot summer months or when engaging in strenuous exercise to ensure we don’t feelrun down and unable to perform at our best.

Stroke. A stroke can result in paralysis, slurred speech, confusion, impaired decision-making, andother problems affecting cognitive functionality. That’s why it’s so easy to confuse a single, powerfulstroke or several “mini strokes” with forms of dementia like Alzheimer’s disease or vascular dementia.

Sleep Apnea. Few people get the recommended seven to eight hours of sleep each night. Somestruggle to get enough sleep because of sleep apnea, a sleep disorder that’s associated withintermittent breathing problems that prevent the body from entering a deep and restful sleep. Theresult can be an exhausted individual who appears sluggish, both physically and mentally, which caneasily be mistakenfor dementia.

High Blood Pressure. Rapid adjustments to blood pressure can have a dramatic effect on ourphysical appearance and mental functionality. The problem is often apparent in older adults,particularly those struggling with high blood pressure or heart disease. Oftentimes, this issue may alsobe related to medications that affect blood pressure.

Brain Tumor. There are few diagnoses more frightening than a brain tumor, which can affect anindividual’s behavior. Although popular opinion insists that this condition is often inoperable, many braintumors can be effectively removed without dramatically affecting the behavior of the patient.

If you recognize these signs in yourself or a loved one, it would be wise to see a physician. Your familydoctor may do part of an evaluation and then recommend a doctor who is experienced in Alzheimer’sdiagnosis such as a neurologist, geriatrician, or other specialist to complete testing. Early testing canhelp a physician determine if Alzheimer’s is a likely factor, allowing the individual and family members toplan for the future.

From The New York Times: “Falls in Older Americans Are Rising. Here’s How to Prevent Them.”This week, The New York Times examined a new study published in the medical journal JAMA, which found that"for people over 75, the rate of mortality from falls more than doubled from 2000 to 2016." While the overallpicture shows that this is likely due to people living longer with conditions that would have been fatal in the past,the trend is concerning. However, the good news is that falls are not inevitable, and there are ways to prevent orreduce the recurrence. Although The New York Times highlighted the following list of steps that the general publiccan easily take to reduce the risk of falling, only a few of these suggestions can be applied for residents living withdementia living in memory care communities:

1. Exercise: At least 20 minutes a day of strength and balance training can greatly reduce your risk of falling.2. Mind your meds: Medications, especially sleep aides, can be particularly dangerous for older people since

they can build up in your system and cause dizziness. Melatonin is recommended as a natural, milderalternative.

3. Re-accessorize: Footwear is critical when assessing fall risk. Slip-on shoes without backs, slippers andheels increase the chance of falling. Even though it may be difficult to accept that a cane or walker hasbeen recommended, using it will increase your ability to be independent for longer.

4. Assess Tripping Hazards: Pets, small rugs, extension cords and uneven floors can easily cause falls.Keeping a light on at night can help prevent tripping on thresholds between rooms or sleeping pets.

5. Early and Often to the Bathroom: Good hydration helps prevent dizziness, so definitely drink plenty ofwater, but don't wait until you're desperate to go to the bathroom. Rushing can cause a fall!

Those living with dementia have a different set of risks that could result in falls, and I believe The New YorkTimes’ list is a compilation of standard practices but are not enough for memory care. The most effective way toprevent or reduce recurrence of falls in memory care is person-centered fall management. Memory care providersneed to go beyond the standard practices and really focus on the individual needs of their residents to be mostsuccessful. Here are a few proactive measures that memory care communities take to reduce falls and preventan unnecessary trip to the emergency room:

1. Environment: Design the environment specific to the individual for comfort, while placing most commonlyused items within reach of the bed or accessible above the knees.

2. Mobile Aids: Place proper aids within reach of the bed allowing the resident to move independently or ifunable to ambulate, place safety precautions to avoid injury if/when the resident self-lowers to the groundto move out of the bed.

3. Bed Positioning: When performing nightly well checks on the residents, make sure care staff check theposition of the resident on the bed to avoid rolling or slipping out of bed.

People living with dementia can present special challenges for caregivers both in the home and in memory careunits, so finding ways to prevent falls is critical to keep loved ones safe and give their families peace of mind.
