Applying Value-Based Healthcare in Hospital Management with Process Mining and Real Time Location Systems MySphera is deployed in more than 40 Hospitals in Europe. Value-based Healthcare is a new health paradigm which aim is being focused in optimizing health processes maximizing the value offered to the patient. This philosophy is a natural consequence of paradigms like Evidence based-Medicine and, most recently, Lean Healthcare. One of the main barriers of the application of these methodologies is the necessity of measuring real health processes. The high load of clinical staff, the lack of some crucial missing functionalities in Hospital Information Systems (HIS) or the lack of standardization culture in health domain are some of the barriers that hospital managers should overcome in order to optimize their process. Real Time Location Systems provide a new minimal obtrusive way to achieve precise and valuable information about the patient in their health process in a hospital. Collecting the location and timestamp of individual patients in the hospital allows to know their precise stay information without increasing the load of health staff in a transparent way and without interfering in the workflow of health professionals. Big size hospital using MySphera in Operation Rooms unit increased 49 cases per month, decreased Operation Rooms turnaround time pre/post implementation by 23%, reduced inter-department phone calls by 82%; saved > 1125 hours per year, and got immediate notification of family post surgery

Medium size hospital increased their Emergency Department daily capacity from 250 to 300 patients using MySphera, avoiding to open a new ED area and saving $800.000 yearly.

PALIA Suite 4 MySphera

PALIA Suite is a Web based Process Mining tool designed for the creation ofcustom dashboards that supports experts in the application of Discovery,Conformance and Enhancement techniques in a simple layout. PALIA Suite 4 MySphera [2] is a customized dashboard specifically though for supporting HealthProfessionals in the understanding of processes via inferring Logs based in RealTime Location Systems.The flexibility of PALIA allows the application of Interactive Models [4] troughQuestion Driven methodologies [1]. In this way, PALIA provide Tools for Filtering,Discovery, and analysis using enhancement and conformance checking algorithmsfor highlighting specific parts of the processes for a better understanding of theexperts

PALIA Suite uses PALIA as their discoveryalgorithm, and provide several customizablefilters that allows select the parts of the log.In the standard version PALIA Suite hasfilters for selecting dates, Process Segments,daily hours, stratification by patient datacollected from the HIS, among others.

PALIA Suite is able to provide specificinformation about individual patientsproviding their flow or access to statistics ofthe flows presented collected from HIS(Source Medical Units, Surgeon ID, Level ofUrgency) The statistics information availablecan be customized by integrating theirinformation in the CSV file of the Log

Heat Maps allows highlight using colour gradients, the time spent in each oneof the activities of the process and the most common paths based on numberof patients.

PALIA Suite provide a simple dashboard that integrate Filtering, Discovery,Enhancement and conformance in a page.

By simply selecting an arrow on thelog, it is possible to now the set ofassociated patients and even knowthe flow follow by an individualpatient

Using Clustering filters, logs can be partitioned depending on their behaviour.

PALIA Suite has some enhancement tools that support theanalysis phase. Heat maps highlight the time spent in theactivities and the most common flows in the arrows.Occupation maps allows showing the saturation of theplaces in each moment. Also, each inference can be savedand compared with others. Numeric statistics are availablevia tool tips allowing a numerical comparison among theflows

Advanced filters like Clustering allows split the logs accordingtheir behaviour and, using the statistics associated to theflows and heatmaps, support experts in the understanding ofdifferent kind of behaviours of the real deployed process andcomparing them.

All the stats of all the nodes and edges ofthe process are numerically available viatooltips in the workflow model

Some Success IndicatorsAn European hospital overestimated its surgical performance at 79%. After the initial MySphera deployment, it was detected that the real surgical performance was 48%. MySphera was able to optimize performance, reaching 69%.
