* = Belmont; Alfred; Wellsville Clinic visit breakdown.
Explanations: Note the # of clients seen in Feb. 2019 were accommodated in ½ FP clinic day in Belmont packing up and having the other ½ day
in Wellsville and 3 full clinic days in Alfred, and in 2018 we held 9 full FP clinic dates. This is accomplished by an increase in mandatory
documenting, lab surveillance, FPBP PE application completion and patient follow-up on non-clinic days. With the retirement of Nancy
Brinkwart, FNP (providing physicals during her last 6 months of employment), and the Family Planning clinics being booked, until the FT position
is filled this service will be referred out for job track and DOT physicals. If in house for employment with the county and availability in a
Thursday Family Planning Clinic - we will try our best to accommodate with Susan Dougherty, NP (hence the 1 physical this month).

M E M O R A N D U M O F E X P L A N A T I O N Introduction No: __________________ (clerk’s use only) COMMITTEE: Human Services DATE: April 3, 2018 The Health Department requests approval to enter into a contract with Andover Central School to provide Evaluation services to our Pre-School Program recipients. Providers are difficult to find in rural areas, so this is an excellent partnership for the children we serve in Allegany County. We have had an excellent and ongoing relationship with Andover School for these services. We bill Medicaid for these services and also recoup 59.5% of what Medicaid will not cover, from the state. I will be happy to answer any questions you have regarding this contract. Feel free to call me at x9247 or send me an e-mail. Thank you for your consideration of my request. M E M O R A N D U M O F E X P L A N A T I O N Introduction No: __________________ (clerk’s use only) COMMITTEE: Human Services DATE: April 3, 2018 The Health Department requests approval to enter into a contract with Alfred-Almond, Andover, Arkport, Belfast, Bolivar-Richburg, Canaseraga, Canisteo-Greenwood, Cuba, Fillmore, Friendship, Genesee Valley, Keshequa, Portville, Scio, Wellsville, and Whitesville Central School Districts to provide transportation services to our Pre-School Program recipients. Transportation is a required service and these school partnerships keep Allegany County from having to contract with private bus companies, which results in significant savings to the County. Since there is no reimbursement mechanism for these services, it is critical that we maintain these cost saving contracts. I will be happy to answer any questions you have regarding this contract.
