• For å oppnå målet om primærforebygging må en best
mulig kunne identifisere dem som har høy risiko for å
utvikle psykose
• To store studier på konversjon fra high-risk til psykose
– NAPLS (N=291), 35% konversjon til psykose ila 2.5
år

– EPOS (N=245), 19% konversjon ila 18 mndr.

POP studien: Inklusjonskriterier
• 1) The patient is listed in the national register and residing in the catchment
areas of: Stavanger and Fonna.
• 2) Between 13 and 65 years.
• 3) Meet diagnostic criteria for prodromal syndrome SIPS criteria (Miller et
al, 2003)
• 4) Does not meet current or life-time criteria for any psychotic disorder
• 5) The symptoms are not better accounted for by an axis I, axis II or substance
use disorder with the exception of schizotypal personality disorder (the presence
of any of these disorders in itself is not an automatic reason for exclusion).
• 6) Does not use any antipsychotic medication currently and have not used
antipsychotic medication (regardless of dosage) for more than four weeks
lifetime.
• 7) No known neurological or endocrine disorders that may have caused the
presenting psychotic symptoms.
• 8) The patient is not mentally retarded with an IQ below 70.
• 9) The patient must be able to understand and speak Norwegian.
• 10) The patient must be able to understand and sign an informed consent or
assent for minors’ document.
POP- status
• 83 inkluderte pasienter totalt, hvorav 10 har
konvertert til psykose ila 2 år (12%).

• Som del av POP studien inviteres pasientene til å delta i
en fMRI/MRI studie

• ”Hårete” mål : Forstå etiologiske faktorer ved psykoseutvikling
• Mer realistisk mål: Forbedre prediksjon av konversjon til psykose- i
fremtiden redusere antall falske positive?
• Mest realistisk mål: Forstå og beskrive UHR som state/trait
• fMRI aktiveringsmønstre som er distinkte for UHR tilstanden
relativt til matchede kontroller.

Schizophrenia MRI
thespacebetween neuronsthat may beaconsequenceof exuberant syn-
aptic pruningduringadolescence. Several interneuron populationsin the
prefrontal cortex are reduced, such as those containing parvalbumin,
which show consistent signsof reduced GABAergicneural transmission8,
but also those expressing neuropeptides such as somatostatin or the
cannabinoid receptor CB1. In thehippocampus, cell bodiesof pyramidalneuronsaresmaller,dendriticspinesarereduced,and thereisinconsistentevidenceof aberrantly located or clustered neuronsin adjacent structures,
especially the endorhinal cortex. In the thalamus, some studies indicatereductions in neuron number, especially in themediodorsal nucleusand
pulvinar9.Taken together, thesefindingsarein reasonableagreement with
structural imagingresults. They suggest abnormalitiesin local processing,especially in glutamatergic drive to GABAergic parvalbumin-containing
interneurons and intracortical connectivity, but are also indicative of
changes in long-range connectivity, including thalamic afferents. Animportant sourceof modulation in prefrontal cortex function isdopami-
nergic.Thedelicatebalancebetween informationmaintenanceandflexible
adjustment of information that characterizesexecutive function dependscritically on an optimal level of dopaminesignalling10 which reachespre-
frontal cortex from midbrain and ventral striatum; theseprefrontal inputsappear to bereduced in schizophrenia.
Function
Cortical and subcortical information processing is functionally abnor-
mal in both first-episodeand chronic schizophrenia (Fig. 1). Most func-
tional studies use an ‘activation paradigm’ in which a cognitive task isused to engage brain systems of interest, and the results are therefore
usefully summarized under thesecognitivedomains, without suggesting
acausal relationship between cognitive(sub)function, brain system andschizophrenia symptoms.
Executive function. Much attention has been focused on executive
function (including working memory and selective attention), subser-ving flexible adaptation of behavioural patterns to external demands.
Here, patientsshow quantitativeabnormalities in dorsolateral prefron-
tal cortex (which have been linked to negative symptoms11), rostralanterior cingulateand inferior parietal lobule. In dorsolateral prefrontal
cortex, patients show relatively inefficient prefrontal activation under
low cognitive load, indicative of decreased signal-to-noise ratio, and adecreasein activation when executivedemandsexceed capacity12.There
is evidence for compensatory activation in the ventrolateral prefrontal
