Two-Earners/Multiple Jobs WorksheetComplete this worksheet if you have more than one job at a time or are married filing jointly and have a working spouse. If you

don’t complete this worksheet, you might have too little tax withheld. If so, you will owe tax when you file your tax return and might be subject to a penalty.

Figure the total number of allowances you’re entitled to claim and any additional amount of tax to withhold on all jobs using worksheets from only one Form W-4. Claim all allowances on the W-4 that you or your spouse file for the highest paying job in your family and claim zero allowances on Forms W-4 filed for all other jobs. For example, if you earn $60,000 per year and your spouse earns $20,000, you should complete the worksheets to determine what to enter on lines 5 and 6 of your Form W-4, and your spouse should enter zero (“-0-”) on lines 5 and 6 of his or her Form W-4. See Pub. 505 for details.

and 10 to comply with the new hire reporting requirement for a newly hired employee. A newly hired employee is an employee who hasn’t previously been employed by the employer, or who was previously employed by the employer but has been separated from such prior employment for at least 60 consecutive days. The Employee Enrollment/Change form (includes the premium surcharge attestations) must be received no later than 31 days after you become eligible for PEBB benefits.

The MetLife Enrollment/Change form or enrollment through the MetLife MyBenefits portal (link below) for basic and optional life must be received, by MetLife, no later than 31 days after the you become eligible for PEBB benefits.

The Long-Term Disability Enrollment/Change form* for basic and optional LTD must be received no later than 31 days after you become eligible for PEBB benefits. *Port Commissioners and seasonal employees who work a season of less than 9 months are eligible for basic LTD only.

Stacking: Faculty may establish eligibility and maintain the employer contribution toward PEBB insurance coverage by working as faculty for more than one institution of higher education. Faculty workloads may only be stacked with other faculty workloads to establish eligibility under WAC 182-12-114(3) or maintain eligibility as described in WAC 182-12-131(3). "Academic year" means summer, fall, winter, and spring quarters or summer, fall, and spring semesters and begins with summer quarter/semester. In order to be eligible for the employer contribution through two-year averaging, the faculty must provide written notification of his or her potential eligibility to his or her employing agency or agencies within the deadlines established by the employing agency or agencies (WAC 182-12-131(3)(d)).

Faculty who lose eligibility for the employer contribution: All benefits-eligible faculty (eligible as described in WAC 182-12-114(3)(a) and (b)) who lose eligibility for the employer contribution will regain it if they return to a faculty position where it is anticipated that they will work half-time or more for the quarter/semester no later than the twelfth month after the month in which they lost eligibility for the employer contribution. The employer contribution begins on the first day of the month in which the quarter/semester begins (WAC 182-12-131(3)(e)).

INSTRUCTIONS TO ACCESS THE EMPLOYEE EARNINGS & LEAVE WEB APPLICATION,

INSTRUCTOR BRIEFCASE, & REQUIRED CANVAS COURSES

Contact Payroll at 442-2220 to obtain your employee SID and/or 6 digit PIN. To access Employee Earnings & Leave, go to the LCC website and select “Faculty & Staff Website” from down arrow on the right, then you will see “Earnings & Leave” listed on the left.

Make it easy and save the link to your favorites: https://lcc.ctc.edu/kiosk/empearnlv/

After selecting “Proceed to Login,” you will be prompted to enter the following information:

Employee ID—this is your employee SID or better known as your 900#

Employee PIN—6 digit number setup with payroll Employees may view the following information:

Earnings, deductions, and contributions associated with a given check Current and previous year-to-date gross earnings and deduction balances Leave balances Your SID and other information

To access Instructor Briefcase, go to the LCC website and select “Faculty & Staff Website” from the down arrow on the right, then you will see “Instructor Briefcase” listed on the left.

Save time and add the shortcut to your favorites: https://lcc.ctc.edu/kiosk/ibc/ Once in Instructor Briefcase you will be asked for:

Instructor ID—this is your employee SID or better known as your 900#

Employee PIN—6 digit number setup with payroll

Quarter—select the quarter you wish to access If you have questions or encounter any problems using Instructor Briefcase, contact Kara McElroy in Registration.

Canvas Courses Within a month of your hire date, you will be assigned two Canvas courses.

1. The Right to Understand 2. FERPA

Each course should be completed within 30 days of being assigned so keep an eye out for them to appear in your course list in Canvas. You are required to take a short quiz at the end of each course and can re-take the quiz until you pass. If you have any questions regarding your Canvas courses please call HR at 442-2120.
