1.3 Personnel

The Principal, Mr Philip Hibbs, has over twenty five years experience in the Occupational Health & Safety and Environmental Management and reviews the services provided by the Company.

The senior personnel are highly qualified, have extensive experience and have been involved in the establishment of industry standards. Copies of the CVs for the senior personnel are contained in Appendix 2.

Our multi-disciplinary team approach to problem solving ensures clients of a practical, cost effective solution, tailored to their specific requirements.

4.0 OHS MANAGEMENT SYSTEMS

An Occupational Health & Safety Management System refers to a structured and documented set of plans, actions and procedures designed to manage occupational health and safety in the workplace. As with all good business systems, an effective OHS Management System will help to ensure legislative compliance, and standardisation of procedures within an organisation. Hibbs & Associates Pty Ltd can assist organisations to develop and implement an effective OHS Management System.

This consultancy can range from simply documenting existing policies and systems, through to the development and implementation of an entire OHS Management System. OHS Management Systems are developed in conjunction with the Client to ensure a relevant, all encompassing system that is specific to the organisation.

The Australian Standard AS/NZS 4801:2001 provides guidance on the framework for OHS Management Systems. However, it does not specify requirements for legislative compliance. An effective OHS Management System will assist an organisation to comply with its statutory obligations and needs to reflect the OHS issues specific to the organisation. Hibbs & Associates develop OHS Management Systems in accordance with the framework of AS/NZS 4801: 2001 and relevant statutory, and other, obligations.

In conjunction with providing effective OHS Management Systems Hibbs & Associates can also assist organisations to work towards accreditation.

An OHS Management System from Hibbs & Associates Pty Ltd will ensure:

• a documented OHS policy demonstrating the organisations commitment to OHS.

• establishment and review of ‘SMART’ objectives.

5.0 ENVIRONMENTAL MANAGEMENT SYSTEMS

As with Occupational Health and Safety, an Environmental Management System refers to a structured and documented set of plans, actions and procedures designed to manage environmental risks.

It is not a legal requirement to have en Environmental Management System. However with increasing focus on environmental issues, an Environmental Management System will assist an organisation to manage their environmental risks, comply with relevant legislation, demonstrate due diligence and display their environmental concern to their clients and customers.

Hibbs & Associates Pty Ltd can assist organisations to develop and implement an effective Environmental Management System.

This consultancy can range from simply documenting existing policies and systems, through to the development and implementation of an entire Environmental Management System. Environmental Management Systems are developed in conjunction with the Client to ensure a relevant, all encompassing system that is specific to the organisation.

An internationally recognised standard exists for Environmental Management Systems:- ‘AS 14001:2004 Environmental Management Systems – Requirement with Guidance for Use’. This standard, in conjunction with AS 14004 (General guidelines), provides guidance on the framework for Environmental Management Systems. Hibbs & Associates Pty Ltd can assist organisations to develop and implement Environmental Management Systems in preparation for accreditation to AS14001 by a certified body.

6.0 ASBESTOS MANAGEMENT SERVICES

The general recognition of the health risks associated with the exposure to asbestos fibres requires employers, building owners and managers to identify the locations and monitor the condition of asbestos materials in their buildings. Hibbs & Associates Pty Ltd provides a comprehensive asbestos Risk Management service to assist clients to control asbestos hazards in the workplace and comply with the relevant legislative requirements. The senior company advisers are recognised in the industry as an experts in the area of asbestos and hazardous building materials management.

The asbestos management service provided by Hibbs & Associates Pty Ltd is designed to identify, evaluate and control the wide variety of asbestos hazards encountered in the workplace. The range of services includes:

6.1 Asbestos Survey The purpose of the asbestos survey is to identify the hazard by locating the visible and accessible asbestos containing materials. The survey process involves a review of available drawings and a thorough physical inspection of the site to identify, locate and record the visible, accessible sources of asbestos present. The scope of the inspection includes an examination of all the accessible areas of the premises, including the construction materials, finishing materials and building services, and the collection and analysis of materials suspected of containing asbestos.

At the completion of the survey a report is provided which details the locations, type and general condition of the asbestos containing materials identified during the survey. The report is supplemented with relevant photographs and drawings as may be required to assist with identifying the asbestos materials’ locations.

6.2 Risk Assessment The building survey is the first step in the process. The next step is to evaluate the risk that the asbestos materials pose to all the building’s occupants. The risk assessment does not occur as a separate entity but as an integral part of the visual inspection process. The risk assessment is based on the condition and location of the materials and the potential for exposure.

6.3 Hazard Control Strategies Recommendations and priorities for control strategies are provided based on general Risk Management principles and the ASCC “Code of Practice for the Management and Control of Asbestos in Workplaces [NOHSC: 2018 (2005)] and include:- leave and maintain, enclosure, encapsulation and removal.

6.4 Asbestos Management Programmes The success of any option that involves leaving asbestos materials in situ is contingent on the necessity to ensure that they are remain undisturbed and are maintained in excellent condition. This is achieved through the implementation of an Asbestos Management Programme. Hibbs & Associates Pty Ltd is able to develop a comprehensive Asbestos Management Programme and assist with its implementation.

6.5 Asbestos Abatement Project Supervision

Adequate supervision of asbestos abatement projects is the only way to ensure that the works are performed to the necessary high standards and in accordance with the relevant legislation, codes of practice and standard occupational hygiene practice.

Hibbs & Associates provides a comprehensive project supervision service to ensure work quality with minimal disruption to the client. Project supervision services include:

• Specification preparation. • Daily on site supervision of the works to ensure conformity with the specification,

regulations, codes of practice and standard occupational hygiene practice. • Clerk of works functions in regard to approval of progress claims. • Asbestos fibre air monitoring.

6.6 Laboratory Services

The Hibbs & Associates Pty Ltd Laboratory Quality System is Accredited by NATA to “ISO/IEC 17025-2005 General Requirements for the competence of testing and calibration of laboratories”, Accreditation No. 14911. Refer Appendix 4.

Accreditation is currently held under ‘7.82 - Workplace environment and hazards’ for:

1. Asbestos fibre air monitoring 7.82.01 - Asbestos fibre counting; and 7.82.86 - Volume measurement (air); and

2. Asbestos bulk sample analysis 7.82.31 - Asbestos identification, qualitative identification in bulk samples.

6.6.1 Asbestos Fibre Air Monitoring

Asbestos fibre air monitoring, both static measurements and occupational exposure measurements are performed in accordance with Hibbs & Associates Pty Ltd Test Method No. 1. This method is based on ASCC “Guidance Note of the Membrane Filter Method for Estimating Airborne Asbestos Fibres [NOHSC: 3003(2005)]”.

6.6.2 Asbestos Bulk Sample Analysis

Representative samples of building materials and soils are analysed for asbestos content using Hibbs & Associates Pty Ltd Test Method No. 2. This method uses Polarised Light Microscopy supplemented with Dispersion Staining and is based on Australian Standard “AS4964-2004 Method for the qualitative identification of asbestos in bulk samples” and “MDHS 77 (Methods for the determination of Hazardous Substances) Asbestos in bulk materials”.

6.7 Hazardous Material Surveys

In addition to the range of services provided for management of asbestos hazards, each of these services can be extended to include other hazardous materials such as polychlorinated biphenyl’s, synthetic mineral fibre products and lead based paint systems.

7.0 INDOOR AIR QUALITY

The increasing requirement to cut costs and conserve energy is putting greater demands on building Heating, Ventilation and Air Conditioning (HVAC) systems and on building owners and managers. Tighter buildings are now being constructed to reduce the infiltration of outside air through building leakage. The amount of fresh air introduced through the air conditioning system is also reduced. In addition, there is now recognition of the health effects associated with passive smoking and the problems associated with off-gassing from carpets and modern synthetic construction materials.

Associated with this has been an increase in the incidence of health complaints and problems reported by building occupants. In response to these problems, various standards, codes of practice and regulations have been developed in Australia to establish procedures for monitoring and maintaining commercial building environments.

Hibbs & Associates Pty Ltd offers a comprehensive building health management service to assist clients to effectively manage the indoor environment and ensure compliance with their legislative obligations. Our aim is to ensure that potential health risks associated with the building environment are suitably managed and comfort criteria optimised. This is achieved by consideration of the three principal aspects affecting air quality and comfort: Air Quality Testing, Building Ventilation Assessment and Cooling Tower Inspection.

The ability to develop a comprehensive investigative approach requires the disciplines of occupational hygiene, chemistry and engineering. 9.0 PROJECT MANAGEMENT SERVICES Hibbs & Associates Pty Ltd is in a unique position to project manage of demolition projects that require the removal of hazardous building materials underground storage tanks and site remediation.

Our project management and engineering team has recently and successfully completed large projects which included the removal of asbestos materials, demolition of steel-framed structures, reinforced concrete structures, timber-framed structures, infrastructure such as roads, footpaths and other different structures and buildings. Many workplaces contain spaces that are defined as “confined spaces” because of intrinsic risks or their configurations hinder the activities of any employees who must enter into, and exit from them. The enclosed nature of these spaces can also trap dangerous, poisonous or explosive gas mixtures and other contaminants, and as a result confined spaces can pose a serious, sometimes lethal, threat to worker health and safety. Project: Assessment of the exposure of Ambulance Officers to anaesthetic gases during the administration of inhalation analgesia to patients in the back of ambulances.

Our Involvement:

The principle objective of the studies was to assess the exposure of ambulance officers to the inhalation analgesics: methoxyflurane and nitrous oxide. Two separate studies were conducted. The assessment of exposure to methoxyflurane was conducted by field trial only. The nitrous oxide study was conducted in two stages:

Stage 1 - Simulation Trial: conducted to assess the potential exposure of the Treating Officer and Driver to nitrous oxide in a series of predetermined exposure scenarios and to determine the need to proceed to field trial.

Stage 2 - Field Trial: conducted with ambulance officers treating patients in the field. The field trial was conducted to assess the exposures of Ambulance Officers administering a proprietary brand nitrous oxide gas mixture to patients in the field.

The studies were conducted over a period of approximately six months and covered the following general elements:

Design of the study - simulation tests (nitrous oxide only) and field study including: selection of suitable sampling systems, design of sampling protocols and designs of data recording systems.

• Training of Ambulance Officers to conduct the field study.

• Co-ordination of the field trials.

• Preparation of database for processing the data. Statistical interpretation of the data and report preparation.

• Briefing the Ambulance Service management, WorkCover Authority NSW and the ambulance officers’ union representatives on the findings of the studies. Presentation at, and participation in the Ambulance Service Focus Group discussions.

• Assessment of air exchange rates in the patient cabins of NSW ambulances.

• Review of the relevant Australian Standards and overseas standards for ventilation supply rates of the patient cabin in ambulances.

2. Indoor Air Quality Assessment – Odour Investigation

Project: Investigation of Odour from Client’s Regional Administration Office.

Our Involvement:

This comprehensive indoor air quality assessment was requested following reports of malodours and ocular complaints from the occupants of the facility. The source of the malodours was initially determined to be the hospital grade polyvinyl chloride (PVC) backed carpet tiles installed at the facility. These tiles were subsequently removed and replaced with a standard-line carpet system (also with PVC backing) affixed to the concrete floor which was treated with a moisture barrier system. Within two months, irritant odours reappeared. Our investigations included an assessment of possible volatile organic compound (VOC) emissions from all sources in and around the building and also a direct assessment of VOC emissions from both the hospital-grade and the standard-line carpet tiles. The contamination of the soil below the slab was also considered as a potential source of the odours.

The assessment was multi-staged and included:

• Specialised sampling and analysis methodologies were used to assess VOC emissions from both the hospital-grade carpet tiles and the replacement standard-line carpets;

• Air sampling techniques capable of detecting ultra low levels (i.e. ppb v/v) were used to assess VOC emissions from various sources, including the office air, the headspace above the concrete slab, a sub-slab core sample, and other potential contaminants emitted from the soil underneath the slab;

• From the results obtained, we were able to identify and confirm that the malodour resulted from a specific chemical which is formed as a result of the alkaline degradation of a plasticiser (di-2-ethylhexyl-phthalate) used in PVC carpet backing, in the presence of moisture and lime in the concrete floor;

• As a result of our findings, a paper was written, approved and published in the Journal of Occupational Health and Safety – Australia and NZ, Vol. 23(3), June 2007, p.241-250 “Degradation of Additives and Water-Based Adhesives in Vinyl-Backed Carpets: Impact on Indoor Air Quality” by D. Le Van.

Our Involvement:

The project involved a review of the City of Sydney management procedures for the handling of Workplace Hazardous Substances (HS) and the storage and handling of Dangerous Goods (DG) with respect to the requirements of the Occupational Health & Safety Regulations 2001 and appropriate Australian Standards.

Over a 3 year period, all City of Sydney facilities and the recently amalgamated South Sydney Council facilities were assessed. Hazardous Substances Registers and Dangerous Goods Registers were compiled. Material Safety Data Sheets (MSDS) were reviewed and updated as required. Hazardous Substances risk assessments were conducted. A review of the Dangerous Goods storage, segregation, compatibility, and placarding requirements was also conducted and recommendations provided.

Hazardous Substances Awareness Training sessions were conducted for selected City of Sydney personnel who will be involved in the maintenance and upkeep of the revised system.

4. Welding Fume Exposure Assessment

Project: Welding Fumes Exposure Assessment at Auburn MainTrain Facility.

The project involved a detailed occupational hygiene assessment of welding operations conducted in various sections of the Facility.

Breathing zone (personal) samples were collected from 11 operators. These were supplemented with 7 fixed point samples. The samples were collected over various types of welding to evaluate operators’ exposures to welding fumes, metals, carbon monoxide and ozone, in relation to the appropriate occupational exposure standards.

The personal protective equipment (PPE) used by the operators was also reviewed and recommendations given as appropriate.

A presentation session was subsequently delivered on site to present the results, findings and recommendations to the operators at risk in the presence of the appropriate management personnel.

5. Airborne Contaminant Study inside Major Arterial Road Tunnel

Project: Airborne Contaminant Study inside M5 East Tunnels

Our Involvement:

The project involved a detailed air contaminant study inside the 2 M5 East tunnels (eastbound and westbound) each over a full 7 days period. The objective of the study was to estimate the levels of selected atmospheric contaminants generated by vehicle traffic inside the eastbound and westbound tunnels. The levels of oxygen (O2), carbon monoxide (CO), carbon dioxide (CO2), nitrogen oxide (NO), nitrogen dioxide (NO2), sulphur dioxide (SO2), benzene, 1,3-butadiene and diesel particulates were monitored at selected static locations within the tunnels according to an agreed schedule.

Although the results obtained are from static (paraoccupational) sampling and do not reflect actual worker exposure, they have been used as relative indicators of exposure against established Occupational Exposure Limits (OELs) to allow the review of safe work procedures for the tunnel maintenance personnel.

Project: Investigation and assessment of bitumen fume during the laying of bitumen road paving.

Our Involvement:

The project involved the assessment of exposure for various operators, to bitumen fumes, during the laying of Ground Rubber Crumb Modified Bitumen and Styrene Butadiene Styrene (SBS) Modified Bitumen. The exposure assessment involved the collection of breathing zone air samples from the paving machine drivers, screed board operators and rake hands during the laying of bitumen based road paving. The samples were collected using the BHP-EHL sampling heads fitted to Dupont portable sampling pumps and analysed for a variety of materials including total fume (total chromatographable matter), polycyclic aromatic hydrocarbons (PAH- Total Measurable PAH and USEPA Priority PAH’s) and the cyclohexane soluble fraction. The results obtained were compared with Australian and International standards.

Our Involvement:

The Orica Australia Pty Ltd warehousing and distribution facility at Chester Hill handles a wide variety of commercial, agricultural and industrial chemicals, for storage and transhipment. The assessment was multi-staged and included:

• Review of the site procedures for the storage, labelling and handling of Hazardous Materials to ensure compliance with the specified Orica procedures, the Occupational Health & Safety (Hazardous Substances) Regulation 1996 and the various WorkCover Authority NSW and Worksafe Australia, Codes of Practice.

• Measurement of personal exposures of the storemen and forklift truck drivers to a variety of vapours, chemicals and dusts handled and stored at the facility.

• Measurement of diesel forklift exhaust gas emissions (CO, CO2, NOx), measured at the exhaust outlet in accordance with the requirements specified in the Department of Mineral Resources Mines Inspection Act 1901 - Guidelines for Diesel and Operator Environment Testing in Underground Coal Mines, MDG No. 29, July 1995.

• Conduct noise dosimetry measurements on the fork lift truck drivers to ensure compliance with the Occupational Health and Safety (Noise) Regulation 1996.

Project: Indoor air quality assessments at the Sydney Air Traffic Services Centre and the Sydney, Canberra, Wagga Wagga and Albury Airport Control Towers.

Our Involvement:

The centres are essential for Air Traffic Control throughout Sydney and Country NSW and their uninterrupted operation must be maintained. Monitoring and control of the indoor environment is an on going issue in all major buildings but is of particular importance in facilities such as Control Tower Cabins.

The assessments included a comprehensive investigation of the HVAC plant and systems and environmental monitoring of the key indicators of indoor air quality as outlined below.

• Inspection of the facilities HVAC systems (air-conditioning plant, air handling systems and water systems), review of the maintenance practices etc. to ensure general conformance with Australian Standards AS3666-1995 “Air-handling and water systems of buildings - Microbial Control” and HB32:1995 “Control of microbial growth in air-handling and water systems of buildings”, and recognised guidelines.

• Measurement of key indicators of indoor air quality including:

- temperature and humidity (thermal comfort factors), - carbon dioxide (fresh air supply), - carbon monoxide (entrainment of exhaust gasses), - airborne microbial activity (fungi, moulds and bacteria), - formaldehyde (off-gassing from new furnishings etc.), - volatile organic compounds (VOC’s) (entrainment of Avgas fumes etc.), - total suspended particulates, and - asbestos fibre air monitoring (ATSC only).

Project: Removal of limpet asbestos materials from the Round Mountain Radar Facility and Dubbo Airport Control Tower. Both of these facilities are essential to the effective operation of the air traffic control in NSW. Our Involvement:

• The facilities were surveyed to identify the precise locations of the asbestos materials to ensure accurate and comprehensive documentation of the removal works.

Prior to this he was a partner at a large national SHE consulting firm, where he held the position of NSW Director for a period of five years. He set up the NSW Division of that company and ran its operation for eight years. Prior to this he was employed by the Health Commission of Victoria in their Occupational Health Division to provide advice on hazards related to asbestos, dusts and chemicals.

Mr Hibbs’ principal areas of expertise include the evaluation and control of hazards associated with asbestos, hazardous building materials, hazardous substances, chemicals and dusts, height safety, confined spaces and indoor air quality. He has also developed and conducted occupational health & safety reviews and environmental assessments for a wide variety of workplaces and industries.

He has been providing Risk Management services to the construction and manufacturing industries since the early 1980’s. His consulting assignments have included the investigation and assessment of hazardous substances, hazardous building materials, asbestos contamination in soil, height safety, confined spaces, and Preliminary and Detailed Environmental Investigations.

NO COMPROMISE Hibbs & Associates Pty Ltd was founded with the driving vision that every worker is provided with a workplace free from health and safety risks and the natural environment be preserved for future generations. NO COMPROMISE Hibbs & Associates Pty Ltd is committed to providing products and services of the highest quality. Our Policy is to be the premier provider of professional Risk Management consulting services in the areas of occupational health & safety and environmental management.

To achieve this objective we will:

assist our clients to meet their responsibilities for Occupational Health & Safety and Environmental Management by providing them with reliable, accurate and representative data and professional consulting services;

educate our staff to recognise that service and quality is the responsibility of every person involved from point of contact through to project completion;

aim to achieve outstanding levels of client satisfaction by ensuring that the management and staff are committed to meeting, and exceeding, our clients’ service and delivery expectations at all times;

implement and maintain a Laboratory Quality System, including a process of “continuous improvement” in strategic and business planning that provides ongoing verification of our capabilities and ensures continuing satisfaction with all services provided by Hibbs & Associates Pty Ltd;

operate in accordance with the requirements of Australian Standard AS 17025 “General requirements for the competence of testing and calibration laboratories”;

ensure our staff are aware of the need, and benefits of the quality management process through ongoing training and development; and

promote active staff participation in both the implementation and ongoing improvement of the quality management system. All personal involved with the testing and calibration activities within the laboratory are required to be familiar with the contents of the Laboratory Quality Manual, and implement the policies and procedures in their work.

OCCUPATIONAL HEALTH & SAFETY POLICY SAFETY IS EVERYONE’S BUSINESS

Hibbs & Associates Pty Ltd is committed to providing employees, contractors and visitors with a safe working environment where occupational health and safety risks are identified and controlled. The Company will implement all reasonable measures to achieve these aims and undertakes, through its Senior Management and Project Managers, to ensure:

compliance with the statutory requirements, Australian Standards and Codes of Practice relevant to occupational health and safety, as a minimum requirement;

that occupational health and safety is integrated into all aspects of our business operations and activities; and

resources and support are provided to successfully develop, implement and operate occupational health and safety programmes.

Hibbs & Associates Pty Ltd promotes a proactive occupational health and safety management philosophy based on effective communication and consultation; the systematic identification, assessment and control of hazards; and the encouragement of innovative OHS solutions.

As an organisation, we are committed to:

FUTURE GENERATIONS

Hibbs & Associates Pty Ltd is committed to the principles of ecologically sustainable development, conservation of our natural resources and minimising our impact on the environment. As a member of the Australian business community, Hibbs & Associates Pty Ltd will act in an environmentally responsible and ethical manner. To support these commitments, we will:

• comply with the requirements of the relevant Commonwealth and State environmental legislation, Codes of Practice and other initiatives to which we subscribe;
