1. Purpose. To establish a Helicopter Training Squadron TWENTY EIGHT (HT-28) FlightInstructor Standardization and Training (FIST) program which addresses Naval Air Training and Operating Procedures Standardization (NATOPS), Chief of Naval Air Training (CNATRA), and Commander, Training Air Wing FIVE (TW-5) operating directives within the Naval Air Training Command.
2. Cancellation. HT-28INST 3740.5H
3. Scope. This instruction is applicable to all instructors engaged in the flight training ofStudent Naval Aviators (SNAs) and Instructors-Under-Training (IUTs) at HT-28.
4. Discussion. The requirement for a vigorous and continuing standardization program isoutlined in reference (a). References (b) and (c) delineate flight instructor standardization requirements applicable to this squadron. This instruction further amplifies additional requirements for standardization within HT-28.
5. Action. Implement subject program per references (a) through (h). Submit recommendedchanges to HT-28 Standardization (STAN) Department.
6. Records Management. Records created as a result of this instruction, regardless of media andformat, must be managed per Secretary of the Navy Manual 5210.1 of January 2012.
7. Review and Effective Date. Per OPNAVINST 5215.17A, HT-28 will review this instructionannually around the anniversary of its issuance date to ensure applicability, currency, and consistency with Federal, DoD, SECNAV, and Navy policy and statutory authority using OPNAV 5215/40 Review of Instruction.

CHAPTER ONE RESPONSIBILITIES AND PROGRAM IMPLEMENTATION
101. General. This chapter establishes the policies and requirements of the HT-28 FIST program and is applicable to all HT-28 personnel engaged in flight training. Procedures for standardization shall be established and monitored by the HT-28 STAN Department to ensure the safe and effective training of SNAs and IUTs.
102. Responsibilities. HT-28 conducts a vigorous and continuing FIST program, designating the most qualified flight instructors to become Squadron STAN Instructors for each stage. The Squadron STAN Department Head, Stage STAN Leaders, Selected Reserve augments to the STAN Department, and Additional Duty instructors to augment the Helicopter Instructor Training Unit (HITU) are carefully selected to fulfill their responsibilities per references (b) and (c).
a. STAN Officer. Following the responsibilities listed in references (b) and (c), the STANOfficer shall:
(1) Manage the FIST Program and Stage STAN Programs for all instructors in the squadron.
(2) Submit required reports to TW-5 as directed by reference (c).
(3) Forward a copy of the Squadron Standard Operating Procedures and squadron FIST instruction to TW-5 STAN when those documents are published or modified.
(4) Maintain a FIST Jacket (separate from NATOPS Qualification Jacket) on each instructor per reference (b).
(5) Coordinate the scheduling of instructor initial/upgrade stage training and STAN Check Flights, as required.
(6) Ensure student (SNA or IUT) UNSAT events result in a complete training jacket review with a focus on continuity of training and to determine possible Instructor Pilot (IP) deviancies.
b. STAN Board shall:
(1) Be comprised of the Commanding Officer (CO), Executive Officer, OperationsOfficer, Safety Officer, STAN Officer, Stage STAN Leaders, and NATOPS Officer. The Aviation Safety Officer may represent the Safety Officer on a case-by-case basis.
(2) Meet monthly as required, but in no case less than quarterly, with all members present (or appropriate representative).

(3) Provide recommendations with supporting detail on all instructor requests for transition to another stage of instruction, taking into account instructor experience, performance, and the needs of the squadron.
(4) Provide recommendations with supporting detail on all instructor requests for upgrade to Standardization Instructor, taking into account instructor experience, performance, and the needs of the squadron.
(5) Approve or disapprove all changes to instructor qualifications.
c. Squadron Standardization Instructors shall:
(1) Receive their annual/quarterly standardization check flight(s) with another Standardization designated instructor per references (b) and (c).
(2) Conduct instructor standardization check flights per references (b) and (c) with an emphasis on catching common student errors and defensive posturing.
d. IPs shall: Be responsible for achieving and maintaining qualifications, currency, andproficiency per references (b) through (d) and this directive.

201. Helicopter Instructor Qualification. Reference (c) governs the procedures for qualifying and upgrading TH-57 IPs in Advanced Training.
202. Stages. Per reference (c), a stage is a group of related flight blocks. Table II-1 defines each stage:

203. Initial Designation. Per reference (c), new instructors will be designated by TW-5 as IPs and qualified to instruct in Instrument, Navigation, and Contact “C” stages upon satisfactory completion of the IUT syllabus per Appendix A-1. New IPs shall request initial designation as an HT-28 instructor in these categories as well as a CRM Facilitator designation letter.
204. Stage Transitions
a. Qualified instructors seeking transitions to another stage of instruction shall make awritten request using the appropriate form found in Appendices A-2 through A-6. Stage transitions will be per reference (d).
b. The request shall be briefed by the Squadron Stage Leader and a recommendationprovided by the STAN Board to the CO. All prerequisites (including exams) shall be completed in order prior to transition flight events. The IUT shall inform Flight OPS when all prerequisites are complete and prior to being placed on the flight schedule.

c. To be recommended for transition, IPs should meet the following minimum requirements:

d. The CO may waive minimum requirements based on recommendation from the STANBoard and IP experience.
e. The STAN Officer, CNATRA Stage Manager, or Squadron Stage Leader shall conductthe transition check flight or final flight for stage transition as required, unless waived in writing by the CO. The STAN Officer and/or the Squadron Stage Leader should conduct at least one flight, and optimally one flight each.
f. Following the Transition flight(s), the Transition Request/Authorization Form shall beproperly annotated and forwarded to the CO via the STAN Department for final signature.
g. After CO’s designation, the completed Transition Request/Authorization Form shall beinserted in the Transition IPs training jacket and the Qualification Matrix updated accordingly.
h. IPs shall not conduct, and the Operations Department shall not schedule, the Transition IPfor events within the new stage unless they have been designated on the Qualification Matrix or the Upgrade Authorization Form has been signed by the CO and entered in the IP’s FIST jacket.
i. The Instrument X Qualification shall only be used to conduct I4290 events.
205. Contact “B” Stage Restrictions. The following restrictions apply to newly-designated Contact “B” IPs:

a. Prior to being assigned on-wings, new Contact “B” IPs shall fly flights in the C4500 blockand flights between C4102-C4203 for a minimum of 50 “B” IP hours. New Contact “B” IPs shall not triple on days they are scheduled to fly with their first set of on-wings. It is the responsibility of newly-designated Contact “B” IPs to advise their Flight Leader when they are eligible to be assigned their first set of on-wings.
b. First On-Wing Sets: As a quality assurance mechanism, a new Contact “B” Stage IP shallensure that his first on-wing set is sent off-wing at least once to a Contact “B” Stage standardization instructor prior to C4390. The standardization instructor shall debrief the IP and advise the STAN Officer if the second on-wing set should be sent off-wing in the same manner.
c. Upon completion of the first set of on-wings, the STAN Officer may lift all schedulingrestrictions, or direct the newly-designated IP to complete an Error Detection flight with a Contact “B” Stage standardization instructor to address any issues with instructional technique.
d. Following the completion of the Error Detection flight, the STAN Officer may lift allscheduling restrictions. However, if necessary, the STAN Officer may recommend that the IP receive additional monitoring or remedial training through a CO’s Letter of Instruction.
206. Formation Stage Restrictions. New Formation instructors shall be paired with a Formation Standardization Instructor for their first student instructional formation event followed by two additional student instructional events by any previous designated Formation instructor. At least one of the three flights should be a F4001 (not Tilt-Rotor syllabus event). Upon successful completion of these events, the Formation Stage IP may receive a Section Leader designation documented in his/her NATOPS jacket. It is the responsibility of the newly-designated Formation IP to advise the STAN department when they are eligible for Section Leader designation.
207. STAN Upgrades
a. An upgrade to Standardization instructor is an internal squadron qualification denotingthe special trust and confidence of the CO in the IP’s ability to train other IPs and conduct student check flights.
b. Qualified instructors seeking an upgrade to Standardization instructor shall make awritten request using the appropriate form found in Appendices B.
c. IPs should meet the requirements outlined in table III-3 to be considered forStandardization instructor.

g. Initial standardization upgrade flights should be flown with squadron STAN Officer, ifqualified, or the squadron Stage leaders. If neither are available, then the flight should be flown with the CNATRA Stage Manager, the HITU STAN Officer, or the TW-5 STAN Officer in that order utilizing the appropriate Upgrade Flight Grade Sheet (Appendices C-2 through C-6).
h. The focus of the Upgrade Flight is to ensure the Upgrade IP possesses the requisitestandardized knowledge and instructional technique to conduct student syllabus check flights and provide stage-specific training and standardization to other instructors.
i. Upgrade Flights are not the same as Stage Check Flights. However, an Upgrade Flightmay be flown in conjunction with an annual stage check flight or IP/STAN IP flight provided all requirements and objectives for each flight are met.
j. Recommended hours per event (H/X) for each upgrade flight are contained on eachstandardization instructor Upgrade Flight Grade Sheet. STAN instructor upgrade flights may deviate from this H/X if, in the opinion of the STAN instructor conducting the Upgrade Flight, the Upgrade IP possesses the requisite standardization knowledge and instructional technique.
f. Following the Upgrade Flight, the completed grade sheet shall be routed to the STANDepartment. The Upgrade Request Form shall be properly annotated and forwarded to the CO via the STAN Department for final signature.
g. After CO’s designation, the completed Upgrade Form shall be inserted in the Upgrade IPstraining jacket and the Qualification Matrix updated accordingly.
h. IPs shall not conduct events within the new stage unless they have been designated on theQualification Matrix or the Upgrade Authorization Form has been signed by the CO and entered in the IP’s FIST jacket.
i. Instrument Check Pilots (ICP), ANIs, and CRM Facilitators shall be designated in writingby the CO. ICP designation shall be generated with Instrument Stage STAN designation. A copy of the designation letter shall be placed in ICP’s NATOPS jacket.
208. Functional Check Pilot (FCP). Reference (f) delineates FCP qualification requirements and procedures.
209. Aircrewman Training and Designation. The training and designation of squadron instructor aircrewmen is governed by reference (g).
210. Previous Guidelines. IP’s recommended for an upgrade under previous guidelines remain recommended.

CHAPTER THREE INSTRUCTOR CURRENCY, PROFICIENCY, AND WARM-UP FLIGHTS
301. General. This chapter establishes the policies and requirements for maintaining currency and proficiency for all HT-28 personnel per reference (c).
302. Currency. Reference (c) specifies general and stage-specific currency requirements for advanced helicopter instructor pilots. Specific to HT-28:
a. Kneeboard cards for each flight listed in reference (c) are provided by TW-5 and will bemade available by the STAN Department.
b. It is every IP’s responsibility to track and maintain their individual currencies in stage andstandardization qualifications. IPs should not lose currency unless there are extenuating circumstances.
303. Proficiency. Reference (c) specifies general and stage-specific proficiency requirements for advanced helicopter IPs. Specific to HT-28:
a. IPs may request a stage or event proficiency warm-up flight at any time. The OperationsDepartment will consider assigning a warm-up flight following extended leave or TAD periods. IPs can request warm-up events to meet annual minimums but should be used as a last resort and minimums should be achieved through the scheduling of standard syllabus instructional events to the maximum extent practical. IPs should snivel to facilitate these goals as appropriate.
b. Stage Leaders shall monitor their stages by completing an IP brief evaluation monthly andmonitor their stage STAN IPs by completing an IP Brief Evaluation (Appendix D) quarterly. STAN Department IPs can perform an IP brief evaluation as required to further monitor their stage.
304. Warm-Up Flights. Reference (c) specifies general and stage-specific warm-up flight requirements for advanced helicopter IPs. General and stage-specific warm-up flights may be conducted in conjunction with IP/IP proficiency flights, provided all requirements are met.
