CWS/CMS Training Module Conventions

Module Purpose and Organization

This student guide is business process oriented. Certain sections, notebooks and pages may be skipped in a specific module and presented later when they are relevant to a process.

This module introduces you to CWS/CMS tools used in creating and updating a Health and Education Passport.

Skill Building Exercise: Creating the Health & Education Passport

Cases: Bobby B

You are currently assigned Bobby B’s case. You have just received the following information, which you must enter into CWS/CMS to complete the Health and Education Notebooks. After entering the information you will have the data to generate a Health and Education Passport Document for Bobby B.

HEALTH NOTEBOOK

Mrs. J, Bobby’s teacher, sends you a copy of Bobby’s medical records.

After consuming a bag of peanut M&M’s Bobby was hospitalized at Sutter Memorial Hospital for one day on June 21, 2004. Dr. H, ERMD admitted Bobby. After receiving two nebulizer treatments of Albuterol, he was placed in the observation unit for 24 hours. Bobby was discharged the following day by Dr. Bob P MD, the attending pediatrician with a prescription for a Proventil inhaler to be used as needed. He diagnosed Bobby as ‘severe allergic reaction-secondary to peanut consumption’ and gave him a referral to see Dr. Raj W MD, an allergy specialist for follow up care. The referral has been processed and he was seen on June 26, 2004.

While Bobby was being treated at Sutter Memorial Hospital, a review of his medical records indicated that he had not been immunized against Measles, Mumps and Rubella (MMR). Mrs. B was advised of this but stated that Bobby would not be getting this immunization based upon the family’s religious beliefs.

After observing Bobby demonstrating strange behavior, Mrs. J called and asked Mrs. B to take Bobby to Dr. James P for diagnosis. This behavior was first noticed on April 21, 2004. Bobby stated that he found funny looking cigarettes in his father’s home office desk drawer and wanted to imitate “those guys on MTV”. On April 24, 2004, a toxicology test proved positive for THC. Bobby was then referred to Dr. John P. J Ph.D. for substance abuse counseling. Mr. B was advised to attend with him. The referral has been processed and he is seen weekly. His first visit was May 1, 2004.

During a substance abuse counseling session in late July, 2004, Bobby was diagnosed as ADHD and prescribed Ritalin for this condition. Both Mr. B and Mrs. B objected to this medication and refused to allow Bobby to have the medication administered to him.

EDUCATION NOTEBOOK

Your first visit is with Bobby’s teacher, Ruth J. A decision was made to keep Bobby in his current school of origin on August 2, 2004.

She teaches at Ryder High School. The phone number there is (916) 000-7720. The school’s address is 318 Pretend Street, Sacramento CA, 95819. She provides you with education information about Bobby. He is in school between 8:30am and 3:20pm. He is in the 9th grade but he is performing at the 8th grade level. He was transferred to the school on February 12, 2004. Mrs. J offers a year-end report card dated June 12, 2004. The report card shows Bobby to be performing far below average for his grade level. Mrs. J also offers an Attendance Record that begins on February 12, 2004. The Attendance Record shows that Bobby missed over one day a week on average during the last four months of the 2003-04 school year. Bobby requires math tutoring.

You also visit with the school counselor Margot S.

Ms. S is a counselor with the Sacramento City Unified School District. She has offered an Individual Education Plan (IEP) on April 6, 2004, recommending summer school and math tutoring for Bobby. A tentative slot is being held for him in Mrs. M’s’ 9th grade class. If Bobby’s mother complies and Bobby’s performance improves, he will be starting 9th grade on September 13, 2004.

You also receive a report from the Public Health Nurse that developmental and mental health screenings were completed on 6/21/2004. No mental health issues were reported, however a referral for developmental concerns was made on 6/20/2004 to the local educational agency. The referral was accepted on 6/22/2004, and consent was given by Bobby himself due to his age. Bobby will receive help from the Child and Family team through his Intervention Plan with a recommended intervention of natural supports.

ASSOCIATE SERVICE PROVIDERS TO THE CLIENT AND UPDATE HEP

Joy A, the substitute care provider, has informed you that Bobby received his CHDP medical and dental exams on July 3, 2004. Dr. Al P M.D. and Dr. James H DDS examined Bobby. Dr. H makes a referral to Dr. Kat S for periodontal care. On August 20, 2004, Bobby returned to Dr. P’s office for a follow-up exam and to receive any required immunizations. Mrs. B formerly refused to allow Bobby to receive the MMR vaccination, due to religious beliefs. Mrs. B did agree to the influenza immunization. Bobby should return in a year for another influenza vaccination. On August 20, 2004, Bobby also began receiving services from the Alta Regional Center for assistance with his developmental delays.

Creating a Health & Education Passport

Introduction

This module will walk you through entering the health and education information for a client. It will also show you how to create a Health and Education Passport (HEP). Once completed, the HEP is pre-populated with information recorded from the Health, Contact, Education, and Client Notebooks.

Objectives

After completing this module, you will be able to do the following with little or no assistance:

Use the Health notebook located in the Client Management Section of either the Referral or Case Folder in focus, to record and view health-related information for a client

Use the Education notebook, located in the Client Management Section of either the Referral or Case Folder in focus, to record and view education information for a child client

Generate the Health and Education Passport

Record CHDP medical and dental exams

Record Dual-Agency Services Received

Associate Service Providers to a client

Update Health & Education Passport

Starting the Client Services Application

The first step in creating the Health and Education Passport is to start the CWS/CMS Control Panel and the Client Services application. To start the CWS/CMS Control Panel and the Client Services application, follow these steps:

1. From the Windows desktop, double click on the CWS/CMS icon.

2. In the CWS/CMS Control Panel logon window, (A) verify the User ID, (B) type the password, and (C) click the OK button.

3. From the CWS/CMS Control Panel, (A) click the Client Services application icon.

Opening an Existing Case Folder

Use the Case Folder to open an existing case. The Case Information notebook pages will display and all notebooks for that case will be accessible.

4. (A) Click on the Open Existing Case Folder.

5. From the Open Folder dialog box; (A) select the appropriate case, (B) click the OK button and, (C) click the Yes button in the resulting dialog box.

Open this Case Grid:If there are multiple cases that require opening, it is not necessary to wait for each case folder to open. Open the Case Folder for the remaining cases as soon as the first case has been selected.Additionally, double-clicking the name of a case will also open the folder.

Health Notebook

The Health notebook is used to input and/or view health information about a client. When a Client notebook is created for a client, a Health notebook is automatically created for that client.

6. To view or input information into a Health notebook, (A) click the Client Management Section (blue button) and (B) click the Open Existing Health notebook.

7. To select the applicable record, (A) select the client from the For this Client grid, and (B) click the OK button.

Diagnosed Condition Page

Use this page to list alert status, health problems, practitioner who diagnosed the condition, onset date/first visit, and end date for every diagnosed condition for this client. Only record health related conditions diagnosed by a certified medical professional on this page. Information documented here will automatically populate the Medications, Hospitalizations, Medical Tests and Referrals pages.

9. (A) Click on the Diagnosed Condition page tab.

10. To add a diagnosed condition, (B) click the “+” in the Diagnosed Condition grid to create a new row, and (C) check the Alert check box, and complete all known and mandatory fields.

Diagnosed Condition:Option buttons are available to indicate whether Medications, Hospitalizations, Medical Tests, or Referrals exist for a particular diagnosed condition. When entered on the respective page tab for a particular diagnosis, the corresponding option button will automatically be selected.

Alert Check Box:This information will be reflected in the Alerts section of the Health and Education Passport document only if this box is checked.

No Known Health Condition:If “No Known Health Condition” is selected as the Diagnosed Condition Category the health Problem field is set to “No Known Health Condition” and the For This Diagnosed Condition and Communicable Disease frames are disabled. When saving to database or when printing the Health and Education Passport document, the system will ensure that no overlap exists between the dates for a “No Known Health Condition” and the dates for any other diagnosed condition.

Closing a Case:When a Case is closed and if the case focus child is not a referral client involved in an active referral, the system will end date any active “No Known Health Condition” for that client to the case end date.

Observed Condition Page

Use this page to list alert status, health problems, person who observed the condition, onset date, and end date for every observed condition for this client. Record any health conditions observed by someone who is not necessarily a certified medical professional.

11. (A) Click on the Observed Condition page tab.

12. To add an observed condition, (B) click the “+” in the Observed Condition grid to create a new row, and (C) complete all known and mandatory fields.

Medications Page (continued)

If the medication being administered to a client has been marked as psychotropic, or is being administered for psychiatric reasons, you will need to indicate if consent was given by the parents or by the Court to administer the medication. Once the Diagnosed Condition has been added, click on the Medications Page tab.

14. To add a medication, (A) click the “+” in the Medications grid to create a new row (B) check the Alert check box, and complete all mandatory and known fields.

15. (C) Indicate if the medication is a psychotropic, (D) if the medication is being adminstered for psychiatric reasons, and (E) whether the consent given is Parental or by Court Order and the date.

Referrals Page

Use this page to list the date the referral was made by the provider, provider to whom child was referred, client condition, and provider who made the referral. To record entries on this page, you must first record a diagnosed condition.

The Client Condition field displays the child’s condition for which a medical referral was given. This read-only field populates from the diagnosed condition you selected in the Select Client Condition dialog box.

If available, enter the name of the place where the immunization was given or the name of the healthcare worker or physician who gave it. Immunization Page (continued)

Use this page to indicate if a parent has waived a particular immunization, or if the parent has waived all immunizations for their child. Only immunizations mandated by the State of California may be selected. All information can be edited after it populates into the Immunization page fields.

Mrs. B has refused to allow Bobby to receive the Measles, Mumps, and Rubella (MMR) vaccination due to religious beliefs.

20. To indicate that the immunization has been waived; (A) click the Immunization page tab, (B) click the “+” in the Immunization grid, (C) enter all known information in the Select Immunizations dialog box, click in the check box for “Waived” and (D) click OK button. (E) Complete all known and mandatory fields, including the check box for “Waived”. Be sure to indicate in the “Comments/Results” text box the reason for the waiver, if known.

Creating the Health and Education PassportThe Health and Education Passport document was designed to meet the State requirements for the provision of health and education information to the substitute care provider/foster parent. Substitute care providers require accurate and up-to-date health and education information in order to provide appropriate care for children placed with them. The Health and Education Passport provides them with this information. They in turn can share this information with the child’s health care providers and educational professionals, as appropriate. In this way, the substitute care providers also can play an important role in helping to keep the Health and Education Passport up to date. It is expected that the social worker will discuss the purpose and use of this document with the substitute care providers.

Once it is given to the Substitute Care Provider, it should be documented in the Placement notebook-ID page, by placing a date in the ‘Date SCP given HEP and Informed of Purpose’ date field.

The Health and Education Passport document pulls data from various parts of the CWS/CMS database to reflect the child’s important current, and historical, health and education information.

1. To generate the HEP document, click the (A) Open Existing Document-Client notebook. Please note that some sections will not print on the HEP document if data is not populated in the CWS/CMS system.

Entering CHDP Medical and Dental Exams

The CHDP (Child Health and Disability Prevention) program is a preventive health program serving California's children and youth. CHDP makes early health care available to children with or without evident health problems. These exams are required for all children in placement. CWS/CMS will automatically generate a reminder for the next due date of these exams.

Use the Associated Services page of a contact notebook to record when a child has received a CHDP exam. Proper recording of the exams will automatically populate the Well Child page of the Health notebook for the client.

Completing Required Information Using The Federal Reporting Navigation Tool

The NYTD Federal Reporting Navigation Tool checks data that all states are required by the Chafee National Youth in Transition Database reporting standards to collect on all current and former foster care youth who receive Independent Living Program (ILP) services and 17 year olds who are in foster care with follow up data after they age out.

1. (A) Click on the Open Existing Case Folder.

2. From the Open Folder dialog box; (A) select the appropriate case, (B) click the OK button and, (C) click the Yes button in the resulting dialog box.

6. In the Federal Reporting Navigation Tool, missing required information will be green fields (A). To enter the missing data, click on the folder icon next to the section with the green field to take you to that notebook in the application to complete the missing data. Continue this process until there are no longer green fields appearing in the tool.
