Logbook
The assessor is required refer to the relevant Qualifications listed in the Annexure and indicate the the exit level outcomes and associated assessment criteria
for the qualification that the learner is completing within the space provided below.
The Qualification Exit Level Outcomes are what the learner must achieve in order to be found competent at the qualification level, rather than only at the unit
standard level. They integrate all the learning that has taken place and use multiple assessment methods in order to demonstrate the learners’ competence.
Each Exit Level Outcome (ELO) has Associated Assessment Criteria (AAC) which helps to explain how the learners can achieve the ELO.
As in the unit standards were there are specific outcomes which are the overall learning outcomes for the unit standard, the ELO has the same function for the
qualification. The assessment criteria in the unit standard help to meet the specific outcome, as do the AAC’s help to meet the ELO.
Assessor to complete:
For this qualification ___________________________ there are ____________ exit level outcomes which have to be met, note the associated assessment
criteria underneath them which help explain:
Notes:
Assessor to indicate relevant ELO and AAC number, next to the functions performed in the logbook template below.
Assessor to indicate whether natural occurring evidence is relevant, sufficient and current.
50% of the AACs per ELO Section to be covered in the workplace in order to sufficiently achieve the ELO
Qualification is only achieved after the ELOs have been assessed
AAC Workplace Task List
The following AAC Workplace Task List is to be completed prior to the workplace learning and must be signed off by the learner, mentor and assessor.
The document needs to be reviewed after three and six months, to add any additional functions that the learner may have been assigned after being placed
in the workplace.
It documents what tasks specific to the workplace the learner has been placed in/is employed in, are appropriate to meeting the exit level outcomes and
associated assessment criteria. These will then be used in the logbook with a time allocation, performance rating and sign off from the mentor and assessor.
Time should be spent, clearly noting what is required and finding a solution if the workplace or the job function the learner is employed does not offer this
opportunity.
