• I understand that while Chippewa Valley Music Festivals Inc and Fun In The Sun Inc (Seller) has undertaken reasonable steps to lessen

the risk of the transmission of COVID-19 in connection with the Festival for which I am purchasing tickets (the “Festival”), there are still risks

for attending the Festival. I understand that the World Health Organization has classified the COVID-19 outbreak as a pandemic and that

COVID-19 is a highly contagious and dangerous disease, and that being in contact with a person or object carrying COVID-19 may result in

significant personal injury or death.

• I am fully aware that participation in the Festival carries inherent risks related to COVID-19 transmission that cannot be eliminated,

regardless of the care taken to avoid such risks. Inherent risks may include, but are not limited to: (1) coming in contact with individuals or

objects that are carrying COVID-19, (2) the risk of transmitting or contracting COVID-19, and (3) injuries and complications ranging in

severity, including death, resulting directly or indirectly from COVID-19 itself, the treatment thereof, or long-lasting effects of COVID-19.

I• hereby voluntarily accept and assume all risk of loss, personal injury, sickness, death, damage, and expense arising from such inherent

risks and exposure to COVID-19 and release Seller from any and all liability. I further agree to indemnify and hold harmless seller for all

damages I incur, or damage to third-parties as a result of my attendance at the festival, including minor children under my custody, care,

and control, as a result of such inherent risks and exposure to COVID-19 at the Festival.

• I further attest that if I test positive for COVID-19 or I am exposed to COVID-19 or in close contact with someone who tests positive for

COVID-19 within fourteen (14) days prior to my anticipated attendance of the Festival, I will not attend the Festival.
