Pathology is a unique specialty because it touches all of medicine as diagnosis is the basis of all patient care. More than 70% of all decisions about diagnosis and treatment, hospital admission and discharge rest on medical test results.

Anatomic pathologists are concerned with the diagnosis of disease based on gross, microscopic, and molecular examination of organs and tissues. Clinical pathologists diagnose disease through the laboratory analysis of bodily fluids and tissues. The line between the two have become increasingly blurred by new technologies that require new expertise and the need to provide patients and referring physicians with integrated diagnostic reports.

2. Management Overview

GlobalMedia has the knowledge and experience needed to make the development and construction of the Veterans Administration’s telemedicine program a success. GlobalMedia has successfully provided telepathology to Barrow Neurological in Phoenix, Arizona. Dr. Stephen Coons has used the ProLab system which includes the iRez 2100c, CapSure Medical Image Automation software and video conferencing in the frozen section lab since 2005.

Case Study: Neuropathology Chief Saves Time, Gains a Valuable Teaching Tool with CapSure® Image Automation System from GlobalMedia

SCOTTSDALE, Ariz. (September 9, 2010) —Barrow Neurological Institute of St. Joseph’s Hospital and Medical Center in Phoenix, Arizona, was established in 1962 under the auspices of John R. Green, M.D., the founding director. Dr. Green began performing neurosurgery in Phoenix in the 1940s and 1950s. Now led by Robert F. Spetzler, M.D., the institute’s director for the past 24 years, Barrow treats patients with a wide range of neurological conditions, including brain and spinal tumors, cerebrovascular conditions, and neuromuscular disorders.

Barrow was recognized in 2006 by U.S. News & World Report as one of the nation’s top ten hospitals for neurology and neurosurgery. The institute is one of the few centers in the United States that offers the CyberKnife, a non-invasive radiosurgery device for removing previously unreachable tumors and lesions. And Barrow was one of the first neuroscience centers to use Gamma Knife radiosurgery, a non-invasive alternative to traditional neurosurgery.

Problem

Much of the focus of the neurosurgeon is on tumors and knowing whether they are cancerous or benign. That determination

falls to a physician that patients rarely see-the neuropathologist. Stephen W. Coons, M.D. joined the staff at Barrow after completing a Neuropathology fellowship there in 1989. Dr. Coons has been Chief of the Section of Neuropathology since 1996.

Barrow Institute shares the neuropathology laboratory with St. Joseph’s Hospital, on its sprawling campus in Central Phoenix. The hospital is also home to a residency program to train neuropathologists. The busy lab is located on the hospital’s second floor, while Dr. Coons’ office is in the basement where he conducts research. His principal interest is in translating the results from basic research in the molecular genetics of brain tumors to clinical applications in the pathology laboratory.

In the past, when a surgeon would send the lab a specimen for inspection, the trip from his basement office to the second floor lab would take Dr. Coons between five and ten minutes, depending on the availability of a service elevator. The delay meant the surgeon, and the patient on the operating table, had to wait that much longer for the crucial information. After interpreting the fresh frozen specimen, Dr. Coons would then make the return trip—another five to ten minutes—back to his office. Depending upon the number of surgeries scheduled each day, the walks from the office to the lab and back could amount to an hour or more of lost time in Dr. Coons’ day. The GlobalMedia Solution

Working with Dr. Coons, GlobalMedia Group, LLC, a telemedicine design, manufacturing and marketing company based in Scottsdale, Arizona, developed a system that allowed him to remain in his basement office and perform his interpretations there. The result was the CapSure® Image Automation System, specifically designed for microscopy and macroscopy applications.

“In my situation, and the situation of a lot of pathologists, we work in multiple laboratories, different offices, different suites,” explained Dr. Coons. “You can’t be in more than one place at the same time physically, but you can with the camera system. The transit time involved in walking from one lab to another amounts to a tremendous amount of time that can be saved by reviewing the images on screen, live or real-time, rather than spending that time walking back and forth.”

GlobalMedia paired a robust iREZTMi2100c digital video camera with a powerful microscope which connects to a PC through a USB 2.0 connection. In the path lab, a neuropathology resident places the specimen under the microscope and alerts Dr. Coons that they have images to examine. The resident shares the live image he is seeing through the microscope viewfinder to Dr. Coons.

Then, GlobalMedia’s CapSure®, a real-time image automation, collaboration and information management software, takes the split-beam from the microscope and sends it to Dr. Coons’ computer with EasyShare® VC, a complete visual communications solution for point- to-point and multipoint calls. In the time he normally spends walking to the path lab, Dr. Coons is able to develop his opinion and deliver it to the neurosurgeon, saving precious time for himself and the surgical staff.

Benefits

Dr. Coons believes that GlobalMedia’s telemedicine solutions are essential for training future neuropathologists. “One of the real problems academic pathology has with the training of residents is they don’t get opportunities to function independently,” Dr. Coons explains. “There are no...simple levels of things that they can do straight out of medical school that gradually get more complicated.”

Pathologists must have the right answer the first time. The CapSure® ProLab system allows the student to sit at the microscope and operate it without an attending physician looking over his or her shoulder. Residents develop a sense that they are fully involved and have told Dr. Coons that “they like the opportunity to be the one person at the microscope.”

“The camera system plays a major role in resident training when it comes to this process,” said Dr. Coons. “It has to be performed by the attending pathologist, and yet, this is the absolute most critical skill that a practicing pathologist has to have and thus, that a resident must learn. I found that having a remote camerasystemisthenextbestthingtobeingthere;Icanbeataremotelocationreviewingtheimagesthat the resident is responsible for producing and generating. The resident has the impression that he or she is operating independently. It’s really quite amazing how not having someone looking over your shoulder adds that burden of responsibility the residents need to feel. They’ve told me that it absolutely makes a difference in terms of what they learn.”

Dr. Coons sees other potential uses for the GlobalMedia solution. He thinks that they can use the CapSure software to “rapidly document cases, particularly interesting cases, as they come in.” He adds, “We can then link them directly to the case, allowing us to easily retrieve that information in the future.” He believes that linking a full color digital image to the written report will make the report more accessible, both to the clinician seeing the report and to future reviewers.

He founded GlobalMedia in 2002 and continues to design, develop and deliver proven solutions to highly respected institutions including; Stanford, Harvard, and ColumbiaSchools of Medicine, Barrow Neurological Institute, HCA Healthcare and many others. Mr. Barthelemy attended both St. Cloud State and the University of Minnesota and received an honorable discharge as a non-commissioned officer from the United States Marine Corp in 1987. Michael Harris, Development Director

Michael D. Harris is the Director of Development for GlobalMedia Group, LLC, and has been a designer, developer and marketer of technology products for over 25 years.

Mr. Harris was also a founder of several companies including, iREZ Research, Corp., which designed and marketed patented mobile video products and Harris Laboratories, Inc., which designed and marketed memory and processor accelerators. He also held key leadership positions with VideoLabs, Inc. (VLAB), including Director of Digital Technologies and Marketing during its initial public offering.

Michael completed the Electronics program at North Central Technical Institute in 1977. From 1977 thru 1981 he studied Music and Mathematics at the University of Wisconsin and Computer Sciences at the University of Minnesota-Minneapolis in 1986. Michael did post-graduate work in 1988, while attending and working at the Katholieke Hogeschool Brugge-Oostende in Brugge Belgium, in Industrial Engineering Sciences. During his time off, Michael is a Scout leader and has been an avid martial-artist for the past 33 years.

Brad Schmidt, Finance Director

In January 2011, Mr. Schmidt joined GlobalMedia as its Finance Director. Mr. Schmidt holds a bachelor's degree in Accounting and Finance, earned in 1992 at the University of Arizona in its nationally ranked finance program. Following graduation, Mr. Schmidt obtained his CPA license and accepted a position with Arthur Andersen. He gained extensive public accounting and financing experience and earned several promotions, becoming Manager of health-care, real estate, and distribution-based enterprises. On the partner track by 1999, Mr. Schmidt was nonetheless intrigued by the investment banking transactions which he worked on with clients.

Thus, when offered a senior finance role with Styling Technology, a roll-up of beauty product distribution companies, in 1999 he jumped at the chance to apply his capital raise, finance, and accounting skills to the consolidation of businesses acquired by the venture, remaining there for approximately one year.

Building on his interest in growing start-ups to mid-size businesses, in 2000 Mr. Schmidt became the Chief Financial Officer of PAR Technologies, Inc. At PAR, he managed the financial strategy and staffs of multiple units of an enterprise which designed, manufactured and distributed micro and memory chips. Soon after the strategic consolidation of these businesses in 2002, he left to become the Chief Financial Officer of the GloalMedia, which focused on a full line of video and web collaboration technologies for businesses, including the healthcare industry. At GlobalMedia he gained substantial experience in ISO compliance, contract negotiation, government certification and effective/efficient operations management.

In 2004, Mr. Schmidt was recruited by recent start-up, ZipRealty, to serve as Director of Sales for Zip's cornerstone Arizona territory. Mr. Schmidt's leadership soon resulted in record profits, which were a major factor in the success of the company's $60M IPO in 2004. ZipRealty, which is the #1 most searched on-line real estate brokerage company, and the 5th largest national brokerage for most homes closed, has grown to a $100M+ revenue company and now operates across the U.S. Over several years, Mr. Schmidt was promoted to positions of increasing responsibility in the company. Most recently serving as the

Bruce J. Adams, Operations Director Bruce J. Adams, Operations Director for GlobalMedia Group, LLC., has over 18 years of experience in operations. Prior to GlobalMedia, Bruce had his own service company for seven years, where the culture was customers always came first, communicate and keep it simple. From 1989 to 2002, Bruce held positions of Operations Manager, General Manager, and Vice President in areas of aerosol paint, plastics recycling, and specialty food manufacturing, all of which were global and multi-million dollar companies. Bruce has eight years experience managing the intellectual property of an international company which had over 100 patents in the U.S., and all over the world.

Today, Bruce is directing the operations of GlobalMedia with three principals in mind - simplicity, collaboration and accountability. Jay’s introduction to sales came in 1999 in Valencia, California, where he served as Account Manager with Boone Air Parts, Inc. Jay has extensive sales and marketing experience in the health care products industry ranging from pharmaceuticals to hospital integration systems.

As Global Sales Director for GlobalMedia, Jay's objectives are to grow the sales force while also concentrating on new markets and opportunities. A current focus involves integrating stimulus funding with the health care industry. Jay's responsibilities also include brand identity, quality assurance and maintaining GlobalMedia's excellent reputation worldwide.

Jay also was an award-winning Respiratory Sales Specialist for Merck & Co., one of the world's largest pharmaceutical firms.

A native of Denver and married father of two, Jay is an honors graduate of Biola University in La Mirada, California. He and his family enjoy a variety of hobbies ranging from snowboarding to BMX bike racing to scuba diving.

Product Description:CONi Services is a VistA validated modality that allows work stations with CapSure® Client Services integrated to communicates and translate DICOM (and other) queries back and forth with VistA Imaging Servers. In doing so, CONi assists in storing images from Telemedicine/Telepathology consultations into VistA Imaging by purging images to applicable patient file at the close of each session and wiping PC clean of residual images. Thus allowing users to stay within DICOM 3.2 compliance while storing patient images.

SECTION III: Interoperability

CapSure software along with CONi (CapSure Over Network Interface) is DICOM compatible and is DOD/VistA validated. You can capture images and send those images complete with calibrated measurements, staining and annotations into VistA with only one mouse click.

SECTION IV: Service Levels

Basic Warranty:

All GlobalMedia products come with a 1 year limited warranty. Products that become defective during the first year (365 days), after the order is shipped will be repaired or replaced by GlobalMedia free of charge. This limited warranty is contingent upon proper use of the product and does not cover products that have been damaged (scratches, bent metal, broken components) misused, modified, or subject to unusual physical or electrical stress. All returns for any other reason must be made within the first 30 days from time of shipment, and will be subject to a 25% restocking charge. Buyer must include all original components, literature, and packaging in the same salable condition received to avoid any additional charges. All terms and conditions listed at www.globalmedia.com shall apply to this limited warranty mutatis mutandis and, in particular, to the services.

Extended Warranty Plan:

An extended warranty plan extends the original warranty period, www.globalmedia.com from the 1st day the original warranty expires and continues until 365 days later. All terms and conditions remain in force with each extended year purchased. Fast-Track Service is not included but can be purchased in addition to the extended warranty.

Fast-Track Service:

In addition to the GlobalMedia standard equipment warranty available at www.globalmedia.com or an extended warranty plan, the Fast-Track Service gives next business day (NBD) replacement parts with shipping costs covered within the contiguous 48 United States. This is a parts only agreement, labor is not included and GlobalMedia will not reimburse for any labor or re-installation of product covered under this agreement. An Authorized Reseller of GlobalMedia may provide a labor warranty outside this agreement.

Response Time - GlobalMedia standards of response Monday-Friday 8-5pm MST is within 2 hours. If 24/7/365 extended service contract is purchased the response time is within 30 minutes. Delivery Time - GlobalMedia’s performance standard varies depending upon level of integration, it can be anywhere from next day to 6 weeks. (i.e. replacement parts can be shipped for next day delivery).

Fill rate - GlobalMedia’s standard is to fill purchase orders in agreed times and agreed products. Standard quoted fill rate is 4-6 weeks of receipt of all purchasing documents and video equipment.

On-time delivery - GlobalMedia’s standard is to deliver according to agreed delivery dates.

Back orders and how customers are notified - GlobalMedia’s standard is to notify via telephone and confirming email.

Inventory accuracy - GlobalMedia standard is accomplished by daily cycle counts, monthly usage counts, quarterly full counts and year end independent audited counts.
