You will be held to all clinic policies, procedures, rules and regulations noted in the clinic handbook.

SCHEDULES MUST attend clinic 3 days (minimum)
EVERY WEEK, until the end of the semester. NO EXCUSES.
Time must be made up if missed Friday On-Call list – email Adams and
Clinic GA Clinic comes 1st, GA jobs come 2nd. Be prepared to put in extra time. Think of
it as opportunity, not burden 100 hours are REQUIRED, but reaching
100 hours is NO REASON to slack off, you will be penalized if you do.

FIELD BASED SUPERVISOR Your assigned supervisor is your primary contact Who has contacted their supervisor already?
If you have not contacted your supervisor, YOU ARE LATE!!
Student bio must be sent F-b supervisor approves your schedule, hours worked,
workload, presentations done on site, etc. Directs you to any site-based orientations that need to be
completed Signs off on all clinic contact hours Signs “Clinical Skills Checklist” Completes the following grading forms
Session observation (optional) Dx session observation (optional) Midterm grade – modified CBS form / grading rubric Final grade – modified CBS form / grading rubric
Clinic Expectations Document: Best ways to contact supervisor, etc, etc, etc. This will be
sent to your supervisor to ease orientation

Refer to your f-b supervisor as the EXPERT. Even if you are curious about WHY they do something or you have been told in previous clinics to do something differently, you follow the current supervisors lead, unless there is something unethical being done. There is a way to inquire without insulting your supervisor about their procedures.
We need a copy of all supervisor’s credentials – check to see your supervisor has submitted paperwork

of placement, & schedule in one email Weekly Tracking Form MUST be sent weekly with
updated hours. Notify of any tardies, absences and changes in
schedule If you need help finding materials, tools, goals,
etc in your field placement If you are connecting with your supervisor If there are any ethical concerns at your site,
with your supervisor or if you are having any other problem completing your required assignments at the placement site
Planning for your first day in f-b clinic… Statistics! What are the top three things that
clinicians worry about / want to know before the first day of clinic???
PLANNING FOR YOUR FIRST DAYS IN F-B CLINIC Contact supervisor to get information Go early – everyday! Professional dress (Wear what they wear) or CSD polos and pants
Adams discuss wardrobe Be prepared with materials/resources appropriate for population served
Evidenced-Based Practice (EBP) Watch what you discuss – you are not his/her friend, you are a student Follow the attendance policy that the supervisor follows (in theory) Let supervisor know about staffing dates, ASHA travel and Comps
meetings ASAP. Review the clinic grading rubric so that you know what skills / behaviors
are being graded and observed by your supervisor HIPAA – Confidentiality agreement
