A little bit about me, I served as principal of the Gilbert Campus last school year. Prior to that, I worked in the St. Louis County Schools for 18 years; 8 in administration and 10 as a special education teacher. I have lived in this community for almost 15 years and have three kids attending school here. We have many exciting things going on this summer. Last year we received a $2.1 million grant from the IRR board for use in building upgrades. Some of the major projects we have going on right now are replacing all of the windows at the Franklin Elementary, replacing the entrance and roofing at the Gilbert campus, new security cameras in all of our facilities and a new roof on the Eveleth Campus new gym. Welcome back to the 2014-2015 school year! I am Jeff Carey and

have been hired to replace Mrs. Hilde as superintendent this year.

Although Mrs. Hilde has left big shoes to fill, I am excited for the

opportunity. A little bit about me, I served as principal of the Gilbert

Campus last school year. Prior to that, I worked in the St. Louis

County Schools for 18 years; 8 in administration and 10 as a special

education teacher. I have lived in this community for almost 15

years and have three kids attending school here.

We have many exciting things going on this summer. Last year we

received a $2.1 million grant from the IRR board for use in building

upgrades. Some of the major projects we have going on right now

are replacing all of the windows at the Franklin Elementary,

replacing the entrance and roofing at the Gilbert campus, new

security cameras in all of our facilities and a new roof on the

Eveleth Campus new gym.

Unfortunately most of these projects will not be complete when

the students come back to school. We are working out plans with

the contractors to ensure minimal impact on students and staff.

Recently, the School Board approved moving to four full days a week

Adam Wilson. I am

from Fargo, North

Dakota. My interest in

history and society

have created a passion

for travel. My travels

have taken me to 16

countries all around the world. I am an avid sports fan both

as participant and an observer. I was a member of the

College of Saint Scholastica men’s soccer team and have

played many sports competitively throughout the years. I

like to spend my summers relaxing at the lake and working

on my inconsistent golf game. I have enjoyed my educational

pursuits and I look forward to being a part of the Eveleth-

Gilbert community.

Reida Forsman. I earned a

Bachelor’s of Vocal Music

Education from Concordia

College in Moorhead,

Minnesota and have 14 years

of public school and private

studio teaching experience. I

have directed madrigal

dinners, musicals, and church

ensembles. I enjoy singing for

special occasions.

My husband, Justin, is from Eveleth. I have come to

appreciate the family values and town pride that are present

here….a genuine sense of “home” and belonging. Our oldest

will become a Golden Bear, we hope to share those values

and traditions with our family.

I am excited to teach in a school district where community

spirit is so strong! I look forward to sharing my passion for

music with my students and hope to see you at our concerts

and other performances this year!

Alex Porkkonen. I am from Eveleth and graduated from Eveleth-Gilbert High School in 2009. During my time at EGHS I played

football, basketball and baseball and continue to enjoy many of those same activities. Following high school I attended Mesabi

Range Community College and the University of Minnesota Duluth to pursue my teaching degree in Social Studies. I taught last

year at North Woods High School and it was a wonderful first year experience. I am currently enrolled in the Special Education

masters program at St. Mary’s University. I look forward to a fun and challenging year at my former high school!

teaching career in 1993,

where I taught middle school science in southwestern

Though I am new to this position, some may recognize me from previous roles as the Nelle Shean ADAPT counselor or as one of the Junior High Volleyball coaches.

Parent Portal in PowerSchool. Parents are able to log into the Parent Portal to update contact

information and set preferences for school notifications. We hope that this tool will provide

another opportunity for efficient communication.

SCHOOL SUPPLY INFORMATION FOR JR. HIGH STUDENTS & PARENTS

Students at Eveleth-Gilbert Junior High need to work with their parents/guardians to determine the best way for them to keep their materials organized. This process will be different for each student, but organization is CRITICAL—so make a plan and purchase what you need. Be prepared to make changes if what you’re doing isn’t working and you’re losing assignments or coming to class unprepared. Don’t overdo it. Sometimes less is

more—fewer folders and notebooks mean you have less to keep track of. Purchase extra supplies in the fall when they are less expensive so you can refresh yours if they run out or become damaged.

Locker organizers can be helpful, but they can also create more of a mess to contend with and may prevent the locker from opening or closing. Students are not allowed to put stickers on lockers. Magnets are allowed inside, but tape and stickers are prohibited.

School supplies can become expensive, especially for those with multiple children. Any families who have difficulty for any reason purchasing supplies should contact the principal, counselor, secretary, or any other staff member. We have some supplies on hand that we can provide, no questions asked!

Teachers at Eveleth-Gilbert Junior High will hand out a syllabus at the beginning of the year that may list items not included here. This list, and the elective course supply lists do not include information from all courses or teachers. This list is created to help you get started gathering materials for the 2014-2015 school year.

All students in grades 7 and 8 will need the following supplies for their required courses:

1. No planners are required this year as students will be using their iPads for organization.

2. Pencils (lots of them, with good erasers—mechanical pencils are fine)

3. Pens

4. Scientific calculator

5. Notebooks (1 designated especially for English—80 page)

6. Binders—some classes have required sizes

7. Loose leaf paper

8. Folders (the sturdier the better)

9. Paper grocery bags to cover books

10. Colored pencils

11. Glue sticks

12. Hand held pencil sharpener

Students in Physical Education will need the following: combination lock, athletic tennis shoes (not skateboard shoes), t-shirt, shorts and/or sweats.

Students enrolled in the following elective courses will need:

Family and Consumer Science/FACS: one inch 3-ring binder, loose leaf paper. 8th grade FACS students will also need: $5.75 for sewing materials, wire hanger, pin cushion and dressmaker pins (recommended but not required), magazines with food pictures (not required)

Art: pencils, erasers, hand held pencil sharpener, folder.

Orchestra: one inch 3-ring binder, hole reinforcement stickers, a tuner (iTuner is a free app for IPhone/IPod/iPad), extra set of strings (for students who own a full size instrument), a mute, all lesson books (7th grade Essential Elements 2000 for Strings Book 2 and Suzuki Volume 1. 8th grade Essential Technique 2000 Book 3, Suzuki Volume 2 or higher), Metronome, shoulder rest for vio-linists and violists, rock stop for cellists and bassists, folding music stand for home practice and performances outside of school, cleaning rag, pencil highlighter.

FIFTH GRADE: #2 pencils w/erasers, pens (blue, black, red), colored pencils, crayons (24 count), markers, glue sticks, bottle school glue, one 3-subject notebook, three 1-subject notebook, 1 folder, 1 composi-tion book, headphones or earbuds, 2-inch binder, 1 box Kleenex (200 count), ruler with cm and inches, school scissors, small pencil box, calculator (regular-not scientific or graphing) gym shoes (to be kept in school), $8 for E-G planner/folder system.

SIXTH GRADE: #2 pencils w/erasers, pens (blue, black, red), colored pencils/crayons, markers, glue sticks, ruler (metric and inch), five wide ruled spiral notebooks, 1 package wide ruled filler paper, scissors (adult size), two boxes Kleenex, six quarter plastic storage box, highlighter, gym shoes (to be kept in school, headphones or earbuds, $5 for folder system.

Checks should be made payable to Nelle Shean Activity Fund.

5th and 6th grade students will have lockers near the office for storing winter clothing, gym shoes, etc. Books will be kept in the classrooms. For this reason, we do not recommend purchasing locker storage accessories.

I’d like to introduce myself. My name is Mr. Hake and I am the new Principal of the Gilbert Campus. I have taught science, special education and coached for 15 years is Chisholm and I am very excited to be a Golden Bear! Some of you may recognize me from the swimming pools or tracks around the area. I also have a background in children’s mental health and juvenile justice.

The Gilbert Campus has undergone some major changes over the summer. We have a new entry way going up and the roof over part of the building is being repaired. There is new carpet in some of the class-rooms and the custodians have been very busy shining the place up for you.

I am looking forward to working with all you to have a great 2014-2015 school year! See you in September.

I am excited to welcome back the students, staff and families of the Franklin Elementary. I know we will have a fantastic year of learning and fun activities.

We are busy preparing the Franklin classrooms and offices for the 2014-2015 school year, but we are under construction. The Franklin is getting new windows so please bear with us as we start the year.

Our teachers are preparing classrooms and fun September activities as summer comes to an end. They are ready to be back and teach to all of those smiling faces.

The Franklin will again have Volunteers in Education, a tutoring pro-gram, and BEAR CARE the before and after school age care program in the building for the 2014-2015 school year.

There will be a lot of information going home with students during the first few weeks so please take some time to review it and return the required paperwork with your child. Please check the district website throughout the year for up-to-date information and events. I am also available to any parent, so just call me at 218-744-7710, or stop by the office.

Our OPEN HOUSE will be on Tuesday, August 26, 2014 from 4: 00-5:00 p.m. and all staff will be here to welcome you to your new classroom.

Welcome back to another exciting school year! We are all looking forward to the year and are excited to welcome our students back. Open house for grades 9-12 is Tuesday, August 26th from 3:30-5:30 pm. Come and find your classrooms and meet your teachers! This is especially important for students who are

new to our building but is also a great opportunity for returning students to meet teachers and make sure they remember their way around. You’ll notice some improvements to the building, including new carpet in some areas. We are also wrapping up the installation of a new security system and have added numerous cameras to both campuses. NEW THIS YEAR! Students will be issued a combination lock from the office. These locks will be the student’s responsibility until they graduate. They will only be allowed to use school-issued locks on their hallway lockers (gym lockers still need a store bought lock). Students must also keep to the locker they are assigned each year unless a switch is approved by the office. Lockers must be kept locked at all times. This will be enforced this year and will help us to eliminate a number of issues with security and theft that could occur. The faculty and staff, along with our student organizations, will be working hard this year to build on our school spirit and continue to strengthen our sense of school community. Watch for announcements about Pack the Gym or Field nights and help us support our extracur-ricular groups. Attend a play. Come to a concert. Wear your Eveleth-Gilbert colors! We’ll continue working to spruce up our student areas with our Golden Bear logo in many ways with new custom floor mats, new banners, and new paint. EVERY Friday will be Golden Bear Friday, we encourage all students and staff to show their school colors! If you have suggestions for activities or ways to show our school pride or build our school community, please let us know! The Eveleth-Gilbert High School is pleased to offer a number of new courses this year. Food Science will cover the chemistry standards for high school and provides another option for students to meet this graduation requirement. Hand and Power Tools is being offered in conjunction with Mesabi Range College and was made possible thanks to funds from the Applied Learning Initiative. We will now be offering a CEP Economics class for seniors. Iron Range History is a new social studies course. Students will be able to acquire American Red Cross certifiication by taking our new CPR/AED First Aid course. In addition to these electives, our 9th grade students will now be required to take a Post Secondary Planning course in order to meet the state’s new College and Career Readiness requirements.

Auditions for The Odd Couple by Neil Simon will be held on Monday and Tuesday, September 15 and 16 in the high school auditorium begin-ning at 3:30 pm. 1-1 Device Update The Eveleth-Gilbert Schools will be entering the final year of our 1-1 Device Program. This year, students who were in grades 6-9 last year will be re-issued the same iPad they had last year. Students entering grades 6, 10, and 11 will be issued a new iPad mini. We ordered mini iPads for two reasons to complete this roll out: the iPad 2 is no longer avail-able and we were able to pur-chase more storage space in a mini than we would have in an iPad 4. With all students in grades 6-12 using these devices, teachers will continue to implement digital content, including some new digital textbooks, as we move through the year. Students will use a variety of apps and tools including Schoology, iMovie, Keynote, Notability, and Garage Band. Teacher teams have been working together over the past two years to determine which apps provide the best learning opportunities for students in their curricular areas and grade levels. As always, we encourage parents to use the iPads at home. They will be checked out to you and your student. Have them show you what they’re doing. Use the iPad yourself (we ask that you not let other children in your house use them but parents and guardians are welcome)! The new digital tools we provide to students will make it easier for parents to see what they’re kids are doing in school. We realize that not all homes have wireless internet access and work hard to make sure all students are provided ample opportunity here in school to download anything they need for school work. We will not be allowing any games on iPads this year. Space needs to be reserved for content required by teachers. iPads will be checked periodically either by checking in with students or by using our Management System to monitor them remotely. If you feel your child is not using the iPad appropriately, either by accessing content that is inappropriate or simply from overuse at home, we encourage you to set stricter standards at home. Contact the office if problems continue. It is possible for us to quite severely restrict what students are able to do on the iPads if it becomes necessary. We will be working hard this year to teach students appropriate use of digital tools such as phones, iPads, and computers. They will be learning about cyberbullying, cyber threats, content laws, etiquette, and plagiarism. The internet and electronic devices provide infinite opportunities for learning and connecting with the world. We want to work with families to make sure our students are prepared to make the most of those opportunities. We also want to do our part to en-courage students to connect with others face-to-face and be good citizens without a device constantly in their hands! We thank you for your continued patience as we move through this 1-1 device program implementation. We are aware that it can be a burden as well as an opportunity and appreciate all you do at home to support this initiative! If you ever have questions or suggestions, please contact the teacher or your building principal. We want to know when issues arise so that we may address them in a timely fashion!

Changing Assessment Requirements

The State of Minnesota is working to evaluate the progress of its students toward mastery of its own set of standards in addition to national career and college benchmark standards. In order to do this, they are adding a series of tests for students that will begin when they are in 8th grade. The MCA’s are not going away, these College and Career Readiness tests are designed to measure students progress in getting ready for life after high school. This may mean a career right away or some form of training or continuing education. An overview of the tests all students in Minnesota Public Schools are required to take this year: Grades 3 and 4: Spring MCA Math and Reading Grade 5: Spring MCA Math, Reading, and Science Grades 7 and 7: Spring MCA Math and Reading Grade 8: Fall ACT Explore, Spring MCA Math, Reading, and Science Grade 10: Fall ACT Plan, Spring MCA Reading and Science Grade 11: Spring MCA Math, Spring ACT More information about these assessments can be found on the Minnesota Department of Education webpage: www.education.state.mn.us

Safe Behavior on the Bus

1. Follow the directions of the bus driver at all times

2. Take a seat promptly and remain seated facing forward. Avoid moving around while the bus is in motion

3. Remain on the bus until student has reached their school or designated stop

4. Talk quietly and use appropriate language

5. Keep hands, feet and objects to yourself; no pushing, shoving or horse-play

6. Students must not put head, arms, feet, or any object out the window

7. Fighting, harassment, intimidation or horseplay is not permitted on the bus at any time

8. Students should not throw objects of any kind

9. Use of alcohol, tobacco, or drugs is prohibited

SCHOOL BUS SAFETY WEEK:

The school district designates the third week in October as school bus safety

week to coincide with National School Bus Safety Week.

STUDENT TRAINING:

The school district shall provide students enrolled in grades Kindergarten

through ten with age-appropriate school bus safety training. All students in

grades Kindergarten through six who are transported by school bus and are

enrolled during the first or second week of school receive the school bus

safety training by the end of the third week of school. Students in grades

kindergarten through ten who enroll in school after the second week of

school, are transported by school bus, and have not received training in their

previous school district shall undergo school bus safety training or receive bus

safety instruction materials within four weeks of their first day of attendance.

REQUESTING TRANSPORTATION CHANGE:

When it is necessary to change a student’s regular bus stop, parents and

guardians must complete a Transportation Change Request Form. Requests

for a change in transportation must be received by the principal’s office five

(5) days prior to the requested change date. Forms are available in the

principal’s office or at www.isd2154.k12.mn.us

The Eveleth-Gilbert Public Schools Student Transportation Safety Policy is

Minnesota Statute 121A.59, Transportation by school bus is a privilege, not a right,

for an eligible student. A student’s eligibility to ride a

school bus may be revoked for a violation of school bus

safety or conduct policies, or for violation of any other

law governing student conduct on a school bus, pursuant

to a written school district discipline policy. Revocation

of a student’s bus riding privilege is not an exclusion,

expulsion, or suspension under the Pupil Fair Dismissal

Act. Revocation procedures for a student who is an

individual with a disability under the Individuals with

Disabilities Education Act, United States Code, title 29,

section 794, and the Americans with Disabilities Act,

Public Law 101-336, are governed by these provisions.

13 AUGUST 2014 Immunizations Required for School St. Louis County Public Health and Human Services (SLCPHHS) would like

to remind families that it is important to get immunized on time, every

time. Vaccines have prevented millions of people from suffering debilitat-

ing and sometimes fatal illnesses. Parents should learn all they can about

immunizations. One good resource can be found at the Centers for Dis-

ease Control website www.cdc.gov

Children in Kindergarten and Seventh grade will need to show proof they

are vaccinated against chickenpox (varicella) or have had the disease.

Also, children entering Kindergarten will need to show proof of a second

dose for the measles , mumps and rubella (MMR) vaccine.

In addition to the vaccines required under the Minnesota School Immuni-

zation Law, the Centers for Disease Control (CDC) and the American Acad-

emy of Pediatrics (AAP) recommend that children who are 11-12 years of

age or older receive a Meningococcal conjugate vaccine (MCVW) and a

Human Pappillomavirus vaccine (HPV, for girls only). Although not re-

quired for school attendance, adolescents are more at risk of contracting

certain diseases. Don’t forget, adults need vaccines too. Because cases of

pertussis are at a 45 year high, the CDC is urging parents, siblings and any-

one caring for infants to get their Tetanus, Diphtheria, and Acellular Per-

tussis (Tdap) up to date to prevent transmission of this sometimes deadly

disease to infants.

Immunizations should be part of a preventative health visit with your

regular health care provider. Child & Teen Checkups (C &TC) is a FREE

preventative health care benefit that includes immunizations for children

birth through age 20 who have Minnesota Care or Medical Assistance.

Help in finding a doctor, transportation, or an interpreter is available by

calling SLCPHHS. For those whose insurance does not cover shots,

SLCPHHS hold immunization clinics regularly. For more information please

insurance for children and families who qualify (Medical

Dental Insurance Available To Students Tooth decay is the most common chronic childhood illness, and over 50% of low-income children in Minnesota do not receive dental care. That’s why Smiles Across Minnesota, Children’s Dental Services (CDS), and the Eveleth-Gilbert School District are working together to provide Dental care on-site in the Eveleth-Gilbert School District.

CDS will be providing a full range of restorative and preventative care as well as oral health education to all school children within the district. A consent form will be sent home with students during the first week of school. Upon completion of this form, children are provided a dental exam, a full range of preventative care includ-ing fluoride and fillings.

CDS accepts all forms of public and private insurance. No child is denied treatment due to inability to pay. In the event that a child does not have insurance, CDS as-sists families in applying for medical assistance and offers a sliding scale discount program. Lunch Program Credit Policy National School Lunch and School Breakfast Programs play a critical role in ensuring that Minnesota’s children have access to the nutritious food they need to learn and succeed in the classroom.

The following credit policy has been established to satisfy requirements set by the Food and Nutrition Section of the Minnesota Department of Education. The purchase of school meals is set up on a prepaid basis. Families are required to have funds deposited into school meal accounts in order for students to purchase meals.

Even with good faith efforts, from time to time families run into situations where their children do not have the required funds in their school meals account. Students cannot engage in the learning process when they are hungry, and so the following procedures have been established:

1. A friendly reminder is sent to parents requesting a payment when student accounts do not have adequate funds.

2. A second request for payment is sent if parents have not responded to the first request.

3. If parents do not make payment arrangements after receiving a second request for payments, students cannot charge meals to their past due school meals account.

Students are welcome to select a complementary bag lunch with all food components including: Whole Grain Bread, Protein, Fresh Vegetables, Fresh Fruit and Low Fat Milk when they do not have lunch money.

After five days of providing a complementary bag lunch, the Food and Nutrition staff will contact the Superintendent’s Office so that an appropriate solution may be determined. The Eveleth-Gilbert School District will strive to ensure that nutritious meals are available to all students.

ISD #2154 Non-Sufficient Funds Check Policy

A procedure has been established by ISD 2154 regarding

non-sufficient fund checks and is applicable per family for

each school year with no carryover to succeeding school

years.

1st returned check during school year: The party is subject to a

$15.00 service charge and will be sent a copy of ISD #2154 non-

sufficient funds check policy.

2nd offense during school year: The party is subject to a $15.00

service charge and will be sent a copy of the ISD #2154 non-

sufficient funds check policy

3rd offense during school year: The party will be subject to a

$15.00 service charge. The check writer will be required to remit

in cash, cashier’s check or money order, any purchases or fees

from the District until the end of the School year.

Free & Reduced Meals New for the 2014-2015 School Year: Students who qualify for reduced-price meals will receive FREE lunches and breakfasts. All Kindergarten students qualify for FREE breakfasts!

Who can get free school meals? Children in households participating in the Supplemental Nutrition Assistance Program (SNAP), Minnesota Family Investment Program (MFIP) or Food Distribution Program on Indian Reservations (FDPIR), and foster children can get free school meals without reporting household income. Also, children can get free school meals if their household income is within the maximum income shown for their household size. (See page 17).

Do foster children qualify for free meals? Yes, foster children who are the legal responsibility of a foster care agency or court are eligible for free meals regardless of household income. Complete an application identifying the children who are in foster care.

I get WIC. Can my children get free meals? Children in house-holds participating in WIC may be eligible for free meals. Please fill out an application.

May I apply if someone in my household is not a U.S. citizen? Yes. You or your children do not have to be U.S. citizens for your children to qualify for free meals.

Who should I include as household members? Include yourself and all other people living in the household, related or not (such as grandparents, other relatives, or friends).

What if my income is not always the same? List the amount that you normally get. If you normally get overtime, include it, but not if you get overtime only sometimes.

How will the information I provide be kept? Information you provide on the form, and your child’s approval for school meal benefits, will be protected as private data.

Will the information I give be checked? Yes, and we may also ask you to send written proof.

Families Encouraged to Apply for Free/Reduced Meals

Each year the Eveleth-Gilbert School District asks parents to complete a form asking about household income levels to determine if students qualify for the Free and Reduced Lunch Program. Did you know students eligible for the Free and Reduced Lunch Program also earn the school additional revenue that has nothing to do with the food service program?

Higher percentages of free and reduced lunch counts bring schools additional Federal Title Revenue, Federal Special Education Revenue, Federal E-Rate Revenue (discounts on telecommunication services) and State Compensatory Revenue. This article will focus on State Compensatory Revenue.

Compensatory revenue is used for students who enroll in school who are under-prepared to learn at their age level. State statutes allow schools a broad range of ways to spend compensatory revenue.

Schools can spend compensatory funds on remedial instruction in reading, language arts, mathematics, and other content areas including improvement of study skills. It can fund additional teachers and teacher aides. It can fund lower class sizes and/or a team teaching approach. It can fund longer school days or weeks or extended school year programs. It can fund all day every day kindergarten.

Compensatory revenue can fund programs to reduce truancy and encourage graduation. It can fund health services, nutrition services, and provide funding to ensure a safe and secure

learning environment. It can fund staff development to assist teachers and other personnel in identifying the needs of students.

Compensatory revenue can be used to fund materials and technology to provide alternative instruction. It can be used to present instruction at different rates and to present instruction in a different method than initially used.

For the seven county area of Northeastern Minnesota, this additional Compensatory Revenue is between $659.26 and $5,556.62 per additional student eligible for free lunch and between $329.63 and $2,731.22 per additional student eligible for reduced lunch. When you consider the other revenue sources mentioned above, this can add up to a considerable amount of revenue. For our schools the numbers are:

The district understands that some individuals are uncomfortable providing income information to the schools. I want to assure you that this information is not public information. It is used only for determining eligibility. The number of students eligible is reported only as a total number, so no individual information is released. Students who qualify for free or reduced lunches are not singled out in the lunchroom. Students who qualify for free or reduced lunches are not required to participate in the school’s food program. However, by qualifying they still provide the school with additional revenue.

The district is required to do random income verification, but you are not required to submit paystubs or income tax forms with your application. You only need to provide this information if you are randomly selected for verification.

If your household income is at or below the income levels on the chart below, we are asking you to consider helping the district collect this additional revenue by completing and returning the Free and Reduced Lunch Application form that will be given to each student on the first day of school. If you would like an application form at an earlier date, they are available at the principal’s office at each school and on the district website www.is2154.k12.mn.us.

Please consider helping the school obtain funding by filling out the application if you qualify for the free and reduced lunch program. Thank you.

REQUEST FOR HANDICAPPED ACCOMMODATIONS

Please submit requests at least 48 hours prior to the scheduled event.

Parents Right to Know Parents of the right to request and receive timely information on the professional qualifications of the student’s classroom teachers. This requirement applies to all parents of the children in school, whether or not they receive Title I services, and all Title I schools, both target assistants and school wide.

When districts receive a parent request they must respond in a timely manner, to the requested information, and minimum report the following: whether or not the teacher has met Minnesota’s licensing requirements for the grade level(s) and core academic subject(s) they teach, whether or not the teacher is teaching under a variance status, the education level and subject area of the teacher’s college degree major and any graduate degree or certificate held, whether the child is provided by paraprofessionals, and if so, their qualifications.

Schools that receive Title I funds shall provide to each parent: Information on the level of achievement of the parent’s child on the state academic assessments, timely notice that the parent’s child has been assigned or has been taught for four or more consecutive weeks by a teacher who is not highly qualified.

This information provided to parents shall be in an understandable and uniform format and, to the extent practicable, provided in a language that the parents can understand.

Parental Review of the Contents of Instructional Materials

Minnesota Statutes 126.666 and Independent School District #2154 recognizes the right of a parent, guardian, or an adult student 18 years or older, to review the content of instructional material and, if he or she objects to the content, to make reasonable arrangements with school personnel for alternative instruction. Alternative instruction may be provided by the parent, guardian, or adult student if the alternative instruction, if any, offered by the School District does not meet his or her concerns. The School District is not required to pay for the costs of alternative instruction provided by a parent, guardian, or adult student. School personnel may not impose an academic or other penalty upon a student merely for arranging alter-native instruction. For the purpose of this policy, instructional material is defined as all print and non-print resources used in the education of a student.

1. Instructional material will be available through the office of the building principal. 2. The School District will notify the public of this policy prior to the beginning of each school year by mail via the district newsletter and by including the policy in each year’s student handbook. 3. If any course changes occur after the beginning of the school year, the District will notify any involved parent, guardian or adult student by mail. 4. If a parent, guardian or adult student has any questions about instructional material, he or she will contact the course instructor to discuss the matter. 5. If the parent, guardian or adult student objects to the material, he or she will make arrangements with the building principal and the instructor to discuss alternative instruction, if any, offered by the school. 6. If the school offered alternative instruction does not meet the concerns of the parent, guardian or adult student, reasonable arrangements for parent, guardian or adult student alternative instruction, if any will be made. 7. School personnel or a third party may evaluate and assess the quality of the student’s work.

DISTRICT CRISIS MANAGEMENT POLICY

The Eveleth-Gilbert School District has developed a Crisis Management

Policy for each attendance site. This policy includes general crisis

procedures for securing the building. Classroom evacuation, building

evacuation, campus evacuation, and sheltering during the time of

crisis. Copies of the policy are available to the pubic by contacting

the building principal.

Eveleth-Gilbert School District Indoor Air Quality Notice

The Eveleth-Gilbert School District is proud to be taking a leadership role in providing a safe, comfortable and productive environment for our students and staff so that we achieve our core mission—educating students. Our school will follow the EPA guidelines to improve our indoor air quality by preventing as many IAQ problems as possible, and by quickly responding to any IAQ problems that may arise. Good air quality requires an ongoing commitment by everyone in our school, because each of our daily decisions and activities are affected by the quality of the air we breathe.

School staff, students and parents can obtain checklists or self-help information so they can properly evaluate their child’s home or other out-of-school situations by contacting the school staff. Parents can also obtain information about school facility construction, maintenance and housekeeping practices, chemicals used, mold and HVAC-related information, chemical producing academic subjects, and pesticide and herbicide applications to determine the extent to which school activities contribute to a child’s symptoms by contacting the school.

19 AUGUST 2014 ANNUAL NOTIFICATION OF RIGHTS, PROTECTION, AND PRIVACY OF STUDENT RECORDS

Pursuant to the requirements of the School District regarding the requirements of Federal Law, the following constitutes the Eveleth-Gilbert Public School District’s annual notification to

parents and students regarding the data privacy practices of the School District.

The School District has adopted a Student Record Data Privacy Policy incorporating State and Federal requirements as to data privacy rights in student educational records. In summary,

the policy provides:

Privacy Rights: PPRA affords parents certain rights regarding our conduct of surveys, collection and use of information for marketing purposes, and certain physical exams.

These include the right to: Consent before students are required to submit to a survey that concerns one or more of the following protected areas (“protected information survey”) if the

survey is funded in whole or in part by a program of the U.S. Dept. of Education. Political affiliations or beliefs of the student or student’s parent/guardian; Mental or psychological prob-

lems of the student or student’s family; Sexual behavior or attitudes Illegal, anti-social, self-incriminating, or demeaning behavior; Critical appraisals of others with whom respondent has

close family relationships; Religious practices, affiliations, or beliefs of the student or the student’s parent/guardian Legally recognized privileged relationships, such as with lawyers, doc-

tors, or ministers; Income, other than as required by law to determine program eligibility. Receive notice and an opportunity to opt a student out of Any other protected information

survey, regardless of funding; Any non-emergency, invasive physical exam or screening required as a condition of attendance, administered by the school or its agent, and not necessary to

protect the immediate health and safety of a student, except for hearing, vision, or scoliosis screening, or physical exam or screening permitted or required under State law; and Activities

involving collection, disclosure, or use of personal information obtained from students for marketing or selling, or otherwise distributing the information to others. Inspect, upon request

and before administration or use: Protected information surveys of students; Instruments used to collect personal information from students for any of the above marketing, sales, or

other distribution purposes; and Instrumental material used as part of the educational curriculum. These rights transfer from the parent to a student who is 18 years old, or an emanci-

pated minor under State law.

ISD #2154 has developed and adopted policies, in consultation with parents, regarding these rights, as well as arrangements to protect student privacy in the administration of protected

information surveys and the collection, disclosure, or use of personal information for marketing, sales, or other distribution purposes. The School District will directly notify parents of

these policies at least annually at the start of each school year, and after any substantive changes. The School District will also directly notify, such as through U.S. Mail or email, parents/

guardians of students who are scheduled to participate in the specific activities or surveys noted below and will provide an opportunity for the parent to opt his or her child out of partici-

pation of the specific activity or survey. The School District will make this notification to parents at the beginning of the school year if the District has identified the specific or approximate

dates of the activities or surveys at that time. For surveys and activities scheduled after the school year starts, parents will be provided reasonable notification of the planned activities and

surveys listed below and will be provided an opportunity to review any pertinent surveys. Following is a list of the specific activities and surveys covered under this requirement. Collec-

tion, disclosure, or use of personal information for marketing, sales or other distribution; Administration of any protected information survey not funded in whole or in part by the U.S.

Department of Education; Any non-emergency, invasive physical examination or screening as described above.

Directory information includes the following: The School District proposes to designate the following personally identifiable information contained in a student’s education record as

“directory information” and it may disclose that information without prior written consent from the parent of the student or the eligible student: the student’s name, the student’s grade

level, photo- graph, participation in officially recognized activities and sports, honors and awards received, and the student’s weight and height if a member of any athletic team; The

parent of a student or an eligible student, upon receipt of this notice, will have two weeks (14 calendar days) to advise the School District in writing ( a letter to the Superintendent’s Office)

of any or all of the items they refuse to permit the District to designate as directory information about the student. This designation will remain in effect until it is modified by the written

direction of the student’s parent or the eligible student. In addition, two federal laws require local educational agencies LEA’s) receiving assistance under the Elementary and Secondary

Education Act of 1995 (ESEA) to provide military recruiters, upon request, with three directory information categories – names, address and telephone listing – unless parents have advised

the LEA that they do not want the student’s information disclosed without their prior written consent.

Directory information, as defined by statutes may be disclosed regarding students who are no longer in attendance within the District.

Inspection of Records: The Family Educational Rights and Privacy Act (FERPA) affords parents and students over 18 years of age (“eligible students”) certain rights with respect to the

student’s education records. These rights are: The right to inspect and review the student’s education records within 45 days of the day the School receives a request for access. Parents

or eligible students should submit to the School Principal a written request that identifies the record(s) they wish to inspect, the School official will make arrangements for access and

notify the parent or eligible student of the time and place where the records may be inspected. The right to request the amendment of the student’s education records that the parent or

eligible student believes are inaccurate or misleading. Parents or eligible students may as the School to amend a record that they believe is inaccurate or misleading. If the School decides

to amend the record as requested by the parent or eligible student, the School will notify the parent or eligible student of the decision and advise them of their right to a hearing regarding

the request for amendment. Additional information regarding the hearing procedures will be provided to the parent or eligible student when notified of the right to a hearing; The right to

consent to disclosures of personally identifiable information contained in the Student’s education records, except to the extent that FERPA authorizes disclosure without consent. One

exception, which permits disclosure without consent, is disclosure to school officials with legitimate educational interests. A school official is a person employed by the school as an admin-

istrator, supervisor, instructor, or support staff member (including health or medical staff and law enforcement unit personnel); a person serving on the School Board; a person or company

with whom the School has contracted to perform a special task (such as an attorney, auditor, medical consultant, or therapist); or a parent or student serving on an official committee,

such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to

review an education record in order to fulfill his or her professional responsibility. Upon request, the School discloses education records without consent to officials of another school

district in which a student seeks or intends to enroll.

Transfer of Records to other Schools: The School District forwards educational records of students to other schools and school districts in which a student seeks or intends to enroll upon

request of that school or school district. A parent of a student or a student who is eighteen (18) years of age may request and receive a copy of the records which are transferred and may,

pursuant to this policy, challenge the accuracy of the records. The District does not, however, notify parents of students or students of age eighteen (18) or older, prior to such transfer.

Complaints for Non-Compliance: Parents of students or student of eighteen (18) years of age or older who feel there has been a violation of the rights accorded them may submit written

complaints to the Family Educational Rights and Privacy Act Office, U.S. Department of Education, 400 Maryland Street, Washington, D.C. 20202.

The review of the data privacy rights of students and parents in the educational records maintained by the School District is intended only to be a summary of the provisions of the

“Protection and Privacy of Pupil Records” and of applicable State and Federal Law. Requests for copies of the policy and questions related to that policy should be addressed to:
