The Improvement Compass Finding direc on!

The Effect

Last month, I was one of the fortunate Leadership Com‐mi ee members to have the opportunity to a end the World Conference on Quality & Improvement (WCQI). One of the presenta ons I a ended was related to what LinkedIn does and how it can benefit our careers...or NOT!

Before I con nue, let me ask you...Why are you an ASQ member? Is it to “network” with other professionals in this great career of Quality? If the answer is “YES,” then tell me what is your best tool for reaching out or connec ng with your desired network of professionals?

My friends, LinkedIn has been around since 2003 and I have been on it since. I currently have a total of 89 connec‐

ons and a er a ending the presenta on at the WCQI 2014, I know that I am not doing well. My personal face‐to‐face net‐working is about the same. But...come on! Who has lots of

me to network a web of hundreds of connec ons? Well! I can jus fy my poor networking in many ways, but the truth is that I don’t dedicate the me and effort to it. This is why my personal network is not as robust as I would like it to be.

This is where LinkedIn becomes a great tool. LinkedIn helps make connec‐

ons with people that we have worked with, people we know, people we do business with, etc. LinkedIn’s engine even finds people that you know and suggests you to network with them because of your connec‐

ons with their profile based on your school, jobs, associa‐ons, field of work, etc. Recruiters are using LinkedIn more

than ever to find poten al employees. Your LinkedIn profile is your “live resume,” that can make you an interes ng pro‐spect employee or an less than average professional. I call it a live resume because you need to keep it current and ac ve with updates about your career achievements and not like our old paper resume that is likely obsolete and on shelf, col‐lec ng dust...LinkedIn is also a tool to share that live resume with your network and poten ally open opportuni es for our careers.

Look for our Sec on’s LinkedIn Group and let’s connect! Let’s improve our professional net‐work by using tools like LinkedIn to connect! Our ASQ sec on has over 1300 mem‐bers, yet our Sec on’s LinkedIn only has 398 members. Are you one of them?

DINNER PRESENTATION

Transforma onal Leadership

and Cri cal Thinking Presented by: Nick Dayton, PhD

As a long term quality and business leader (and occasional con‐sultant), endeavoring to con nuously improve his understanding of leadership, psychosocial dynamics, and posi ve outcome achievement, Nick has always embraced the scholar prac oner concept.

Nick’s evolu on as a worker and leader started, as does that of many people, with his trying to understand organiza onal needs, communicate effec vely, demonstrate organiza onal contribu‐

on, and solve problems. Analy cs, focus, and drive became the gateway and path for his advancement – all in all, not bad, but also not complete.

He ini ally mushed the concepts of management and leadership into a single vague understanding, one that was very hierarchically posi oned. You some mes hear about people for whom the very skills and characteris cs that provided them rapid advancement, at a point, are no longer sufficient to maintain personal and pro‐fessional growth – that’s where Nick was heading.

But Nick had the tremendous good fortune of being mentored by

excep onal leaders, both in business and academia, and, by re‐

maining open to learning and changing, has had opportunity to

grow and develop in areas that possibly would have surprised

both himself and those that knew him early on.

By a ending this presenta on, you will learn:

Insights into becoming a transforma onal leader

Thinking about thinking, with the aim of improving our think‐

ing

Our speaker: Nick Dayton, PhD

As Director Quality ‐ Internal Audit, Global Quality & Compliance, Baxter Healthcare, Nick’s efforts have been focused on global compliance audi ng for plants and opera ons worldwide, acquisi on integra on, change management, inspec on prepara on, teambuilding, and driving quality system, compliance and opera onal improve‐ments. Nick has spent the last 25+ years building and strengthening businesses and leading organi‐

za onal development and change ini a ves. While working at Abbo Laboratories, Hospira, and Baxter Heathcare, he has had the tremendous opportunity to build and run plants, perform acquisi on due diligence and onboarding, lead mul ‐plant quality opera ons, and lead divisional and corporate func ons.

Nick brings experience, passion and insights to his presenta ons. He is past‐Vice President, PDA (Parenteral Drug Associa on) Mid‐West Chapter, was a core team member of the PDA team working with FDA on Good Electron‐ic Records Management, has presented at over 50 interna onal and do‐mes c conferences including the PDA Interna onal Congress in Kyoto, the annual Regulatory Affairs Professionals conference in Washington DC, and a number of others in which he co‐presented with FDA leaders. Nick has doctorates in psychology and opera ons research, and masters and under‐graduate degrees in business and opera ons research. He’s had tremen‐dous challenge and fun over the years, and looks forward to sharing some of the things he’s learned and believes.

EN ISO 14971:2012 ‐ Applica on of Risk Management to Medical Devices

Presented by: Jack Dhuwalia, MS ChE, MBA, CQM/OE

Risk Management is an integral part of product design, development, manufacturing and use. Medical Devices are no excep on. Regulatory bodies such as FDA expect a formal process to manage and document these ac vi es. This clinic will present “Fundamentals” of Risk Manage‐ment.

EN ISO 14971 is a recognized voluntary standard that most regulatory bodies recognize and most companies follow them. The last latest revi‐sion to this standard was released in 2012 as compared to the previous 2007 version. Applica on of 2012 revision will be discussed in June.

Following will be takeaways:

By a ending this clinic presenta on, you are invited to par cipate in a discussion as well as learn the significant changes. Following are takea‐ways:

CLINIC # 2

Autoverifica on—A Quality Improvement Tool For Clinical Laboratories

Presented by: Julie Philip

Autoverifica on is a clinical laboratory process improvement tool which employs computer algorithms to allow qualifying laboratory test results to pass directly from automated tes ng instruments to the pa ent’s medical record without interven on by a laboratory technologist. This can result in significant me‐savings, improved turnaround me, and improved consistency in result handling. This presenta on will discuss the current use of autoverifica on in Southern California area hospitals, and the perceived barriers to its use. An example implementa on at a local community hospital will be included.

The presenta on will outline:

What is autoverifica on?

A brief history of autoverifica on use

Current use in Southern California hospitals

Perceived barriers to more widespread use

Use of quality tools to overcome barriers

Regulatory requirements

Example implementa on from a local community hospital

Our speaker: Julie Philip

Julie Philip is the Director of Laboratory Services at Valley Presbyterian Hospital in Van Nuys, California ([email protected]). She has over 30 years experience in the hospital laboratory field, including management of Hematology and Blood Bank sec ons, as well as extensive experience in Labor‐atory Informa on System implementa‐

on. She is a licensed California Clinical Laboratory Scien st, Medical Technologist (American Society for Clinical Pathology), and a member of the ASQ San Fernan‐do Valley Sec on. Julie is a recent graduate of California State University, Dominguez Hills MSQA program.

Shield workers from radia on.

Maintain structural support for the building and cell when the soil is removed from underneath it

Find a final disposal loca on for the contaminated soil that meets all regulatory requirements

Li poten ally very heavy monoliths of grouted contaminat‐ed soil

Cut large monoliths, if necessary, while s ll preven ng radia‐on exposure

Penetrate very hard layers of cobble soil to take contamina‐on samples and radia on readings

Evaluate how to remove shipping containers through the narrow airlock corridor if the soil is removed internally through the cells.

The project team noted a plethora of contradic ons and de‐cided to use the TRIZ approach. They agreed on the following criteria for their Ideal Final Result:

1. Remove the contaminated soil and handle it in one go in the final disposal package.

2. Make the disposal package as big as possible

3. Have zero industrial (safety) injuries and environmental re‐leases

4. Keep radia on dose to workers as close to zero as possible

5. Perform the remedia on work with the exis ng facility

6. Maintain the structural integrity of the building.

Following the TRIZ methodology, these were restated as ge‐neric desired func ons:

1. Move solids (soil)

2. Shield personnel

3. Li heavy loads

4. Stabilize solids (soil)

5. Move monoliths

6. Provide structural support

Lots of contradic ons presented themselves as they examined the desired func ons/results, e.g.,

Removing soil with high levels of contamina on cleans the area BUT creates the need for significant shielding and short du‐ra on work mes to prevent exposure to elevated dose levels

Removing soil with high levels of contamina on cleans the area BUT is likely to weaken structural support

Solidifying soil in monoliths reduces the number of units to be handled BUT requires special li ing capabili es and size reduc‐

on later presents its own difficul es.

Packaging & moving in smaller containers would be safer and easier BUT it would increase the number of containers to be han‐dled and the likelihood of exposure to contaminated soil.

Radical Thinking? By Akhilesh Gula

Solving the problem of Soil Con‐tamina on at Washington Han‐ford Closure

. . . Con nuing the series on Structured Innova on . . . applying TRIZ methodology

A special mee ng of the MEC had been called because Dwain McMullin was in town and Henrie a had arranged for him to share a case study with them. Dwain was one of the TRIZ Coaches at the Hanford Site, a center of radiological re‐search and nuclear fuel fabrica on for the past 60 years. The work performed at this loca on had resulted in highly contam‐inated facili es and waste sites as well as a large inventory of radioac ve material. Mr. McMullin agreed to share his team’s process in coming up with an inven ve solu on to resolve the problems encountered in the clean‐up project. At the outset Henrie a told the MEC that Dwain was not prepared to share the outcome of the project at this short no ce, but the im‐portant thing for them was to understand how they got to the recommenda on stage.

Dwain explained that a significant spill had occurred in the building in the mid‐1980’s. The spill was cleaned up and there was no knowledge that any of the contamina on had breached the floor un l grout was removed from the area in November 2009 and they uncovered a breach in the liner. Readings taken through April 2011 indicated that the contami‐na on had not spread beyond the building’s footprint, and it was above groundwater and not mobile. However, challenges included building deac va on, decommissioning, decontami‐na on and demoli on of the building. A team was formed to help iden fy and solve problems associated with these chal‐lenges.

The problem solving team iden fied a long list of specific challenges with the clean up process:

Eliminate/minimize radia on exposure and contamina on

Eliminate/minimize airborne releases

Stabilize the contaminated soil

Determine the boundaries of soil contamina on

Minimize the number of shipping containers for the con‐taminated soil

Minimize handling and personnel exposure to the ship‐ping containers

Contain the soil excava on process to the facility

Excavate the contaminated soil with remote equipment

Page 8: eScope - ASQ Orange Empireasqorangeempire.org/wp-content/uploads/2014/06/eScope-2014-6.pdf · New ASQ Members Welcome and congratula ons on making the important decision to become

Page 8 eScope June 2014

Moving monoliths removes large volumes BUT would not meet transporta on limita ons and disposal path container requirements.

Stabilizing contaminated soil would allow packaging in shielded containers BUT also has the poten al for mul ple handling, addi onal waste and increased dosage exposure.

Demolishing the facility/building gives clear access to con‐taminated soil BUT removes all shielding and confinement that the building (with its ven la on) could provide.

Washing the soil would concentrate the contaminants BUT make the shielding, handling and disposal issues worse.

A cause and effect diagram was created to help narrow the focus on areas to work on and to ensure that the team’s effort is directed to removing the unwanted ‘effect.’ Fig 1 created by the team clearly indicates that the primary un‐wanted effect is the ‘very high radia on sources present dan‐gerous work condi ons.” All the boxes around the highlight‐ed box are the ‘causes’ that were then analyzed to see how they could be eliminated.

As a reminder, TRIZ emphasizes the use of exis ng re‐sources, to solve the problem, especially resources that are FREE (e.g. air, water, sunshine), previously unused and easily available. Natural bioremedia on sources such as photosyn‐thesis transform hazardous waste or contamina on into a harmless substance and should not be ignored. Dwain’s pro‐ject team made a long list of available resources, including but not limited to:

Clean soil

Heat from contaminated soil

Airborne ac vity

Adjacent cells

Cell floors

Cell ven la on

Air‐lock corridor

Workers

Shielding

Sunlight

Humidity

Barometric pressure

These were all analyzed to see how they could be used to solve the problem.

A func onal interac on diagram was also created to display all relevant components in the system and show the func onal interac ons (Fig 2). Once again, the majori‐ty of the harmful effects were associated with the radioac‐

ve contamina on and the challenge was to stabilize and remove the contaminated soil while elimina ng personnel exposure.

Focusing on generic solu ons to the generic problems iden fied (noted earlier) opened up the solu on set be‐yond the nuclear realm to find cases where ‘someone somewhere has solved this problem’. Once those solu‐

ons were found, the team figured out how to adapt them to our specific cleanup project. The team searched for so‐lu ons on topics such as:

Remote excava on (Con nued on page 8)

Page 9: eScope - ASQ Orange Empireasqorangeempire.org/wp-content/uploads/2014/06/eScope-2014-6.pdf · New ASQ Members Welcome and congratula ons on making the important decision to become

Page 9 eScope June 2014

...Con nued from page 7

Soil stabiliza on

Soil solidifica on

Remote mining

Under building soil removal

Subsurface / soil vitrifica on

Once current methods were iden fied, informa on collected and analyzed for viability; patents searches were also conduct‐ed and iden fied for their relevancy. Various concepts were generated and informal screenings performed subsequently evaluated by the project team. Various concepts were gener‐ated and informal screening performed (using the IFR ele‐ments as success criteria) and a top recommenda ons list was created, listed here in random order:

Create monoliths our of grout and contaminated soil in other cells

Underpin the cell for monolith removal prior to soil exca‐va on

Magne cally remove stron um‐90

Vapor extract cesium‐137

Expand expansion joint to introduce stabiliza on media

Mul ‐use grout as a drilling fluid and for pretreatment

Use limited canister shielding

Leave canister in place

Challenge regula ons

Fig 2 also shows the underpinning of the cell prior to soil excava on as part of the solu on for ensuring structural integ‐rity.

These were all evaluated and solu on concepts developed. Dwain promised Henrie a and the MEC team that he’ll tell them about the implementa on when the informa on release is approved, and he said that he was sure that TRIZ would be used mul ple mes during the implementa on to solve the problems as they arose.
