History Taking

Chief Complaint :

peeling off the skin

Systematic Anamnesis :Experienced since 1 week ago in almost all of

the body. The lesion started as red patches at

the face and then spreads to the chest,trunchus, upper and lower extremities. The

patient also feels itchy.

A few days later, the lesion thickened, scaly and wet

surface. Patient feels like a burning because of the

lesion peeling off. Treatment history : Patient had a treatment before

(diarhea and vomiting), he get tetracyclin capsul.

He never had experiencing the lesion and no family

history with the same complaint.

A man, 23 years old, came to the Labuang Baji Hospital.Experienced since 1 week ago in almost all of the body. The

lesion started as red patches at the face and then spreads to the

chest, trunchus, upper and lower extremities. The patient also

feels itchy. A few days later, the lesion thickened, scaly andwet surface. Patient feels like a burning because of the lesion

peeling off.
