What is an EHR?

The Electronic Health Record (EHR) is a longitudinal electronic record of patient health information generated by one or more encounters in any care delivery setting (from multiple healthcare providers). Included in this information are patient demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data, and radiology reports. The EHR automates and streamlines the clinician's workflow. The EHR has the ability to generate a complete record of a clinical patient encounter, as well as supporting other care-related activities directly or indirectly via interface—including evidence-based decision support, quality management, and outcomes reporting.

What is an EMR? An electronic medical record (EMR) contains the standard medical and clinical data gathered in one provider’s office. An EMR is a digital version of a paper chart that contains all of a patient’s medical history from one practice. An EMR is most commonly used by providers for diagnosis and treatment.

Key Components 1. The patient medical record with all the information found in a paper chart; 2. a clinical decision support system (CDS) which provides suggestions for appropriate treatment

options based on the current “best practices”, as increasingly required by health insurance plans for full payment;

3. a drug prescription component that includes generating electronic prescriptions (ePrescribing or eRx), calculating drug dosages, alerting for drug-drug interactions and required diagnostic studies associated with a specific drug, and indicating the health insurance payer formulary status for a particular drug (and recommending formulary substitutions if the desired original drug is non-formulary);

4. an interface with an Internet-accessed patient portal; 5. an interface for submitting orders for diagnostic lab studies and receiving the results 6. and the ability to be used to communicate medical information to government agencies and

PHR – Personal Health Record

What is a personal health record?

The PHR is a tool that you can use to collect, track and share past and current information about your health or the health of someone in your care. A personal health record (PHR) is simply a collection of information about your health. If you have a shot record or a box of medical papers, you already have the beginnings of a personal health record. And you've probably encountered the big drawback of paper records: You rarely have them with you when you need them. Electronic personal health records remedy that problem by making your personal health record accessible to you anytime via a Web-enabled device, such as your computer, phone or tablet.

Are there different types of PHRs? Yes, there are two main kinds of personal health records (PHRs).

Electronic Personal Health Records: With an electronic PHR, patients fill in information from their own records and the information is stored on a patient’s computers or the Internet. In some cases, an electronic PHR can also accept data from external sources, including providers and laboratories. With some electronic PHRs, patients can add diet or exercise information to track progress over time. Patients can decide whether to share the information with providers, family members, or anyone else involved in their care.

Paper Personal Health Records: Patients collect information about their health history on their own and store it in the location of their choosing, usually at home. The documentation can be handwritten, or typed out, by the patient. It can also contain copies of documents they have received from various health care providers over the years. Many patients keep this documentation in a binder or box specifically for their health information and take it with them when they visit a new doctor.

Is my PHR the same thing as my medical record? No, medical records and your personal health record (PHR) are not the same thing. Medical records contain information about your health compiled and maintained by each of your healthcare providers. A PHR is information about your health compiled and maintained by you.

Personal health records are also not the same as electronic health records or electronic medical records, which are owned and maintained by doctors' offices, hospitals or health insurance plans. Some offices or healthcare organizations may limit your access to this information, but it is becoming more common to allow patients to access and print their records at any time through a patient portal.

It is important to note that a patient portal is also not a PHR, although many healthcare providers are branding their patient portals as PHRs. Patient portals are not personal health records because the patient does not control the content of the portal. Remember that the PHR is maintained and controlled by the patient. The patient may add information to the PHR that is provided to them by a healthcare provider, but the PHR information is still controlled by the patient. A person may give their healthcare provider a copy of their PHR, but the healthcare provider can only use that information as a means of knowing who to contact to get official records. This is because the PHR is not a legal record of health care.

It is important to understand that PHRs – which generally are not part of a provider’s electronic health record – are not considered legal health records and therefore, are not covered by HIPAA.

It seems that there is always confusion surrounding the difference between a Personal Health Record (PHR) and an Electronic Health Record (EHR). The picture below depicts a simplified concept of the differences.

What information is part of a personal health record? You decide what you put in your personal health record. In general, though, it needs to include anything that helps you, and your health care providers, manage your health. It is always best to start with the basics:

• Your primary care doctor's name and phone number • Allergies, including drug allergies • Your medications, including dosages • Chronic health problems, such as high blood pressure • Major surgeries, with dates • Living will or advance directives • Family history • Immunization history

You can also add information about what you're doing to prevent disease, such as:

• Results of screening tests

• Cholesterol level and blood pressure

• Exercise and dietary habits

• Health goals, such as stopping smoking or losing weight

Depending on the type of PHR, individuals also may be able to input family histories and emergency contact information. They may also have the ability to track and chart the health information of their children or others whose care they manage.

So I really don’t need a PHR, right?

Don’t misunderstand and think that the content of a PHR is not valuable, because it can be life-saving. Let’s say you are on vacation, far from home, and involved in an accident. You are taken to the ER and are able to provide access to your electronic PHR that contains your medication history. The healthcare providers can use that information to make sure you are not given any drugs that you are known to be allergic to or that may interact with a drug you are currently taking.

EHR Key Facts to Remember

EHRs are being implemented in health systems and physician practices at a rapid pace. With each implementation, and in the time leading up to the implementation, there is often concern about whether or not the investment of time, money and angst will pay off. With leadership commitment and careful planning and implementation, healthcare organizations are increasingly reporting on the benefits that they and their patients are reaping as a direct benefit of the EHR.
