EdChoice Expansion Renewal Instructions This scholarship can be awarded to families who are low income qualified and can cover the complete Requirements Form. If your income has increased since receiving your first years scholarship, you may still qualify for a partial scholarship based upon your income. Scholarship and Grant Opportunities We understand that funding your child’s education often requires sacrifices, and sometimes, families do not

think that a Catholic, private education is possible because of the cost. We never want the cost of a Catholic education to prohibit a student from attending. Therefore, we have many financial assistance and scholarship opportunities available for our students! Some of these opportunities are listed below. For more information, please contact our school office at 740-345-4049 ext. 300.

• St. Francis de Sales Parishioner Grant: Registered and active parishioners may receive a parish grantby meeting the requirements established.

• Ohio EdChoice: This scholarship from the Department of Education provides students who reside indesignated public school districts the opportunity to attend participating private schools. Scholarshiprecipients will receive $4,650 towards the cost of their private school tuition. The applications andthe list of designated public schools will be available March 2nd, 2021.

• Ohio EdChoice Expansion Scholarship: This scholarship is available for families who are low-incomequalified and covers the complete cost of tuition.

• Educating for Eternity Tuition Assistance: Each year, thousands of dollars of tuition assistance aregiven to our families who qualify based upon special circumstances and family need throughestablished endowments with the Catholic Foundation.

• Diocesan Tuition Assistance: Each family is also welcome to apply for tuition assistance that is giventhrough the Diocese of Columbus. To apply, please complete the 'Grant and Aid' application usingyour FACTS account.

• F+C+J+M Family Scholarship – This scholarship can support the education a child in need. Open toSFDS parishioners only.

• Brady Hempleman Memorial Tuition Assistance Endowment – This scholarship is available to a SFDSJunior High (grades 6-8) student athlete. It is awarded in the spring for the following school year.

• Knights of Columbus Tuition Assistance Fund – This scholarship of $500 is given to a student who isalso a St. Francis de Sales parishioner. Recipients are nominated by teachers and staff.

• Livestream Tuition Assistance Fund – This scholarship is awarded to a student who is also a St.Francis de Sales parishioner.

Tuition Rates for 2020-2021 Academic Year at Saint Francis de Sales School. Tuition Rates for the New 2021-2022 school year will be published in March 2021.

For the 2020-2021 academic year we will use the FACTS for all our tuition billing. FACTS offer a convenient billing solution for the school as well as a convenient payment option for families. Two ways to pay:

o Automatic Debit from your checking or savings account. o By Credit Card (a 2.75 % service fee applies)

Three payment options: o Paid in full by July 1, 2020 through Automated Clearinghouse (ACH), Credit Card or Invoice ($150 discount per

family awarded if this option is selected). o Two payments by ACH or Credit Card due July 2020 and January 2021 (one-time processing fee of $15). o Three to 11 payments starting July 2020 through May 2021 (one-time processing fee of $55). You may also

choose one of two payment dates: the 14th of the month or the 28th of the month If you are enrolled in FACTS already, please log in and change your payment preferences and dates as

EDCHOICE SCHOLARSHIP PROGRAM 2021-2022 RENEWAL FORM

Proof of residency is required of all first-year and renewal applicants and must be submitted to the school with the application. Parents/Guardians must document residency by providing the school with a current (less than 3 months old) utility bill. The utility bill MUST SHOW MATCHING SERVICE AND MAILING ADDRESS in the name of the parent/guardian. Post office boxes and Cell Phone Bills have no Service Address and therefore are not accepted.

Acceptable Utilities (must show matching service and mailing address: Electric, Gas, Water, Sewer, Cable/Internet. Other Acceptable Documents: Monthly mortgage statement and signed Lease/rental agreement and one (1) other official document with parent’s name and address. Additional information can be found on the scholarship webpage.

* I have submitted only one EdChoice Scholarship application for the student. The scholarship amount shall only be applied to the tuition of the enrolling school and I may be required to pay other fees and costs as prescribed by the policies of the school.

* I will sign all scholarship checks received by the private school for my student in a timely manner. I understand that if I fail to endorse the scholarship checks to the school, I will be responsible for paying the student's tuition.

* If I transfer my scholarship to another participating chartered nonpublic school, I will notify the school of my intent to withdraw and I will return to the original school to sign any remaining checks.

* I will apply for any and all financial aid or tuition discounts and adjustments made regularly available to the students attending the school in which the student is accepted for enrollment.

* I will abide by the Ohio Department of Education (ODE) dispute resolution process outlined in Ohio Administrative Code Section 3301-11-14.

* If I am not a low income parent or did not complete the income verification process, I will be responsible for paying any difference between the scholarship amount and the tuition of the chartered nonpublic school.

* I must inform ODE and the chartered nonpublic school of any change in the student’s residential address or custody status.

* I will not be able to renew my child’s scholarship if; our family has moved to another city school district and our new neighborhood public school is not a designated EdChoice school, my child fails to take each state achievement test required for his/her grade level, my child has more than twenty unexcused absences during the school year, or I fail to complete therenewal process. If my child has received an EdChoice Expansion Scholarship I must maintain Ohio residency and verify my income annually.

Income Verification is one step in the scholarship application process. Your child must also be enrolled at a participating school. The Income Verification Process is important for some families to determine if they meet low income requirements of the scholarship program. If you are a new applicant of the Scholarship and you qualify for low income status, you will not have to pay tuition above the amount of the scholarship. EdChoice & Cleveland Scholarship Program 2021-2022 Income Verification Form 2021 FEDERAL POVERTY GUIDELINES

Traditional EdChoice and Cleveland Scholarship applicants qualify for low income status if income is at or below 200% of the Federal Poverty Guidelines. Note that EdChoice-Expansion applicants qualify if income is at or below 250% of poverty. This chart will help you determine if you may qualify.

Income status determines priority for awarding scholarships. It also determines if your family will be responsible for paying any tuition that is not covered by the scholarship.

Based on the number of people in your household, you may qualify for low income status if your gross annual income is the amount listed on the chart or less.

Household size is determined by the following: the scholar-ship student, the birth mother or the legal guardian of the scholarship student, the spouse (also includes birth father of any child in the household), all children under the age of 18 which the legal guardian or spouse also has legal custody.

How to Complete the Income Verification Process

1. Obtain the Income Verification Form on our website at: http://education.ohio.gov/edchoice orhttp://education.ohio.gov/clevelandscholarship or the nonpublic school where you have applied for or renewed ascholarship. (Page 1 and 2 of this document.)

2. Complete the parent/guardian information on page 1, filling in all lines. This should be the same information you haveprovided on the scholarship application/renewal form.

3. List household members (i.e. spouse, children) on page 1 and provide all the information requested.

4. Write your sources of income on page 2 and provide copies of acceptable, supporting documentation.

5. Sign at the bottom of page 2. Do not return page 3.

6. Based on your household, determine from the list below which one fits your status. For example: If your statusis (a) of the choices below, you only have to submit the documents for that option, not all of them.

a) If you are currently employed, and have the same job you had all of last year, send either 4 current pay stubs foreach job, your W-2 forms, your 2020 Federal Income Tax Return forms or your 2020 Federal Income Taxtranscripts which may be obtained at either: WWW.IRS. GOV or by mailing the 4506-T form to the IRS.

b) If you are currently employed but did not work your current job for all of last year, send 4 current pay stubs foreach job.

c) If you are self-employed, send a copy of your 2020 Federal Income Tax Return forms, including all schedules oryour 2020 Federal Income Tax transcript.

d) If you receive other income sources such as food stamps/OWF, child support, unemployment, Social Security, etc.,then you must send copies of official documentation which show how much you receive from each source.Example: If you currently work and receive food stamps and child support, you must submit four current pay stubs,official documentation that shows how much you receive in food stamps, and official documentation that showshow much you receive in child support.

e) If you have no income or you do not have pay stubs or W-2's, provide your 2020 Federal tax transcript from theIRS. Go to WWW.IRS.GOV. Please mail the request form to the IRS and once you receive your transcript ,pleasemail a coy of that form to our office with the Income Verification form.

DO NOT send original documents. Make copies (ex. W-2, check stubs, etc.) to send to the school and block the first 5 digits of all social security numbers on all documents only leaving the last 4 digits to be seen. Submit only one (1) form per family. (Ex. A family with 3 students in the program only needs to send the form one time per school year.) Keep a copy for your records.

Other Acceptable Documents: Monthly mortgage statement and Lease/rental agreement (signed) and one (1) other official document (like a pay stub, bank statement, insurance statement, car payment statement, etc) with parent’s name and address. If the student’s parent/guardian has no utilities in his/her name, then the parent/guardian must provide the following:

1. A signed and notarized statement from the person (i.e., third party) with whom the parent/guardian and the student live or fromwhom they rent that confirms that they reside at the address. This letter must be from the third party, not the parent/guardian.

2. A copy of a current utility bill in the name of that third party, AND

3. A copy of a piece of current business type mail in the name of the parent/guardian.a. Business mail would be things such as pay stubs, car notes, car insurance, monthly bank statements, and official

document from a government agency. It must be a business with which the parent/guardian is currently doing regularbusiness.

b. NO credit card solicitations or service set-up work orders. NOTHING HAND WRITTEN.

4. Following is an example of how this alternative works: Ms. Smith’s daughter has an EdChoice scholarship. She and her

daughter reside with her uncle, Mr. Brown. Mr. Brown will need to write or type a statement and have it notarized, which should

include him signing the statement in front of a notary. He will also need to provide a copy of a current utility bill in his name,

since he owns or rents the property. Ms. Smith must provide a copy of last month’s bank statement. Compiled together, this

alternative will suffice as proof of residence for the student regarding the current school year. She must do this annually. If she

should move and obtain utilities in her name, then this alternative method is no longer her option and she must comply with the

required utility bill requirement instead.

5. Another example of how this alternative works is the following: Mr. Johnson’s son has an EdChoice scholarship. Mr. Johnsonand his son live in an apartment. All of the utilities are included in the price of the rent, so Mr. Johnson does not receive anyutility bills in his name. Mr. Johnson will need to obtain a notarized letter from the rental office confirming that he and his sonlive in the apartment. Mr. Johnson will also need to provide another form of address proof, such as a current pay stub or bankstatement.

Unacceptable proof of address includes cell phone bills, tax forms, junk mail, and driver’s licenses. Old and outdated (more than 3 months) address proof is also unacceptable.

Parents/guardians must remember to keep the school informed of any address changes that occur and to submit the

required documentation to ensure continued program eligibility.

All families applying for an EdChoice-Expansion scholarship must have their income verified by the

EdChoice Office. When applying for an EdChoice-Expansion scholarship for the first time, the total

household income must be at or below 250% of the federal poverty guidelines (see chart above). All new EdChoice-Expansion applicants must have a household income of 250% or lower to be awarded a scholarship.

Families renewing their child’s EdChoice-Expansion Scholarship for the next school year will be awarded an amount based on where their family’s household income falls on the federal poverty guidelines:

• 0% - 250% = full scholarship = maximum award of $4,650 K-8, $6,000 9-12

• 251% - 300% = 75% of the scholarship = maximum award of $3487.50 K-8, $4,500 9-12

• 301% - 400% = 50% of the scholarship = maximum award of $2,325 K-8, $3,000 9-12

• Over 400% = cannot renew the scholarship

Expansion families with a household income of 250% or lower qualify for low income status. This means that the private school cannot charge these families for any tuition that is not covered by the scholarship.

Expansion families with a household income of 251% or higher are responsible for paying any tuition difference not covered by the scholarship.
