Online Registration Coming for 2018-19 School Year: Infinite Campus Parent Portals Required Beginning with the 2018-19 school year, the Fort Dodge Community School District will be using an online registration process. Parents/ guardians will be required to have an Infinite Campus parent account to complete registration process. Parents/guardians who do not al- ready have an Infinite Campus account should sign up for an account now. This will make your registration process in July quicker and easi- er. To request an Infinite Campus account please complete the Infinite Campus Agreement Form (last pages of this newsletter and also posted under the Parents tab of fdschools.org) and return it along with proof of guardianship (birth certificate, court documents, tax form, immun- ization record, etc) to Diane Jass at the Central Administration Build- ing. Starting now, all change in address requests made to the FDCSD will require proof of residency. Accepted address veri-fication documents include:

Utility Bill or Confirmation of Services (within the last 60 days)

Webster County Assessors – print copy of web page verifying property ownership

Property Tax Statement

Vehicle Registration

DHS Paperwork

Shelter Stay Verfication

If you are residing in another person’s home, please provide a notarized statement from that person stating you and your child(ren) are living there, including their name and address, as well as one of the above approved address verification documents in their name.

All families will be required to provide address verification during the online registration process in July.
