1. PROTOCOL SYNOPSIS
1. Scientific justification
Devenir des Spondyloarthrites Indifférenciées Récentes (DESIR) is the French national
multicentric cohort of inflammatory back pain suggestive of axial spondyloarthritis
(axSpA).(1) The cohort was launched in 2007 (first inclusion in 2007, last inclusion in
2010) and included 708 patients, which makes it to date the largest inception cohort of
early axSpA. Originally only a 10-year follow up of these patients was anticipated and
is ongoing, but recent data on several aspects of the disease suggest that a further
follow-up would be of high scientific interest:
Radiographic outcomes: Structural progression rate is slow in the first years. We
anticipate that the 10-year evaluation (as initially planned in DESIR) will not be
sufficient to determine the natural history of radiographic progression and structural
damage in axSpA, and therefore a longer follow-up is warranted.
Natural history and comorbidities: In the DESIR cohort, patients’ mean age is about
34±9 years and as age is a key factor for comorbidities, a longer follow-up is necessary
for both natural history and comorbidities.
Health and societal costs: AxSpA disease onset usually occurs in the patient’s most
productive years, and therefore axSpA can have an important socioeconomic impact.
High disease activity and loss of physical function are the most important factors
associated with the total costs of SpA. Only long-term (i.e. > 10 years) cost-
effectiveness analyses, taking into account incident comorbidities, will allow to
determine long term costs and consequences and if the high cost of biologics result in
a higher productivity over the working-life of axSpA patients. New biologics and
treatment patterns: Many costly but effective biologic drugs are available in axSpA.
With the imminent regulatory approval of IL-17 and IL-23 inhibitor treatment for
axSpA patients, we can anticipate that a DESIR extension will be a useful setting to
perform multiples analyses pertaining to real-life conditions of use, efficacy and
safetyfor biologics.
Primary objectives
To evaluate the radiographic progression rate over a 20-year follow-up in an early
axSpA cohort

To evaluate the natural history and the incidence of comorbidities over a 20-year
follow-up in an early axSpA cohort
To evaluate treatment patterns and derived health and societal costs in early axSpA
over a 20-year follow-up.
Secondary objectives
To evaluate over the 20-years of follow-up of the DESIR cohort:
The MRI status (both with regards to inflammatory and structural
lesions) over 20 years in early axSpA
The multiple prognostic markers for radiographic progression including
clinical, (disease activity), biological, genetic and imaging markers
The long-term function and quality of life outcomes of early axSpA
The factors associated with comorbidities
The hereditary aspects as pertains to the children of DESIR participants
The uptake and effectiveness in real life of biological and synthetic
disase modifying drugs including TNF inhibitors, IL-17 inhibitors, IL-23
inhibitors, etc.
To determine the treatment trajectories in axSpA, especially since the
arrival of new biologics
The work outcomes in this population
2. Study design
The DESIR cohort is a multicentric national prospective cohort. This study will be the
continuation of the follow-up of this cohort for a period of 10 years i.e. 2017-2020 (the
first patient will reach 10 years in 2017 and the last in 2020) to 2027-2030 (idem for 20
years).
Twenty-four of the 26 centres that are already participating in the DESIR cohort will
continue participating (Annex V). No new patients will be included.
3. Target population
No new patients will be included. The DESIR patients are defined as:

Age : 18 to 50 years old
Inflammatory back pain
o for > 3 months but <3years
o fulfilling the Calin et al. or Berlin criteria for inflammatory back
pain. (2,3)
o Suggestive of axSpA: confidence in SpA according to the
rheumatologist ≥ 5 in a 0 to 10 numerical rating scale.
4. Data collection
We will follow up all the patients in the DESIR cohort, who are still available at the end
of the currently planned follow-up (ie 10 years) and data will be collected in 2 ways:
Center study visits (i.e. patients will visit their centre to undergo their study visit) every
2.5 years (at M150, M180, M210, M240), the following data will be collected in an e-
CRF :
o Treatments (sDMARDs, bDMARDs, NSAIDs, corticosteroids, analgesics, others)
o Comorbidities (gastro-intestinal events, cardiovascular diseases and risk
factors, cancer, infections, endocrinologic diseases, alcohol intake, smoking,
vaccinations, osteoporosis, fibromyalgia, other comorbidities to calculate a
Charlson Comorbidity Index)
o Family comorbidities
o In women: birth control methods, pregnancies, menopausal status and
treatments
o Extra-articular features (uveitis, inflammatory bowel diseases, psorisasis)
o Self-administered patient questionnaires (demographic and medico-economic
data, BASG, BASDAI, BASFI, HAQ-AS, ASAS-HI, SF-36, AS-QOL, EQ-5D)
o A Bone mineral Densitometry will be performed at M180 and M240.
o Physical examination (anthropometric measures, SCJ, TCJ, MASES)
o Biological data (ESR, CRP, Hemoglobin, white cell count, platelets, creatinine,
cholesterol, fasting glucose)
o Imaging data (at M180 and M240): spine and pelvic X-ray, spine and SIJ MRI,
DXA-scan
Virtual visits via an electronic case reported form (eCRF) completed by the patients
every 6 months, comprising disease activity and treatment data and work status.

5. Duration and terms of organization of research
▪ Study Location: DESIR Study is a national study in France.
▪ Recruitment mode: All patients already followed in DESIR cohort will be invited to
continue the follow-up in DESIR cohort ten more years.
▪ Total observation time per patient: 20 years (10 years in the context of this present
protocol).
▪ Follow-up: after the end of the followup in DESIR up to 10 years, patients will be
followed every 6 months via home-visit e-CRF and every 2.5 years via centre visit. X-
Rays, MRI and DXA-scans will be performed at 15 years and 20 years.
6. Ethics
The type of follow-up of this second part of DESIR is usual care, since patients will
complete well-validated, widely used questionnaires that would be anyway completed
at any rheumatologist visit every 6 months, and will not undergo any additional
biological or imaging exams besides the usual-care follow-up.
This present protocol will be submitted to the Ethics Committee and the Commission
nationale informatique et libertés (CNIL). Patients will sign an informed consent form.

2. SCIENTIFIC JUSTIFICATION AND GENERAL DESCRIPTION OF THE RESEARCH
Devenir des Spondyloarthrites Indifférenciées Récentes (DESIR) is the French national
multicentric cohort of inflammatory back pain suggestive of axial spondyloarthritis
(axSpA).(1) The cohort was launched in 2007 (first inclusion in 2007, last inclusion in
2010) and included 708 patients, which makes it to date the largest inception cohort of
early axSpA.
The study of this cohort has led to more than 20 publications in the highest impact
factor journals in the field of rheumatology, shedding light on several areas of the
disease: from knowledge on these patients’ phenotype(4–9), imaging
abnormalities(6,10–12) to treatment efficacy in real life (13,14) and validation of
criteria and instruments (15–18). This is a highly successful cohort with reasonable
attrition rate and good collaborations across French expert centres.
Originally only a 10-year follow up of these patients was anticipated and is ongoing,
but recent data on several aspects of the disease suggest that a further follow-up will
be of high scientific interest.
o Radiographic progression
Ossification of the spinal anterior ligament enthesis (i.e. syndesmophyte formation)
that leads to ankylosis of the spine is the hallmark of the disease, and represents the
worst outcome in terms of function, with a limitation of the spine mobility in these
patients. Ankylosis of the spine is usually assessed by plain X-rays, and measured using
the modified Stoke Ankylosing Spondylitis Spinal Score (mSASSS)(19) that evaluates
the cervical and lumbar spine, and ranges from 0 to 72. Structural progression rate (i.e.
the ankylosis progression) has been reported to be slow in patients with very
established forms of SpA, with a progression rate of 2 mSASSS points every 2 years
(20); and as expected, in early forms of the disease, ankylosis of the spine has been
recently reported to appear at an extremely slow rate. In a recent communication
from Ramiro et al. (21), radiographic progression rate (i.e. mSASSS change) has been
reported to be of 0.27 points over 2 years in the group of patients fulfilling the
Assessment of SpondyloArthrits international Society (ASAS) criteria for axSpA(22,23)

within the DESIR cohort; progression was observed in patients fulfilling the imaging
arm of the ASAS criteria for axSpA, and particularly in the subgroup of patients with
only structural lesions of the sacroiliac joints (i.e. fulfilling the modified New York
criteria(24) without MRI inflammation of the SIJ) at the rate of 0.82 points over 2
years, but also in the group of patients fulfilling the clinical arm (i.e. patients without
imaging abnormalities of the sacroiliac joints) particularly in the subgroup of patients
with abnormal CRP, with a progression rate of 0.14 mSASSS points over 2 years. These
extremely slow progression rates suggest that a 10-year follow-up will not allow
capturing the full natural history of spinal ankylosis and radiographic progression of
early axSpA, and that a longer follow-up is needed.
The clinical arm of the ASAS criteria for axSpA is still not very well recognised by health
authorities and many colleagues, although it has been shown that the burden of
disease(15) and effectiveness of Tumor Necrosis Alpha Blockers (TNFb) (14) does not
differ in patients fulfilling the imaging and the clinical arms.
Some have suggested that the non-radiographic (i.e. without mNY sacroiliitis) forms of
axSpA are simply “pre-radiographic” forms of axSpA that will all eventually evolve to
radiographic forms. Progression from non-radiographic SpA to radiographic SpA after 2
years has been reported to be of only 12% in the german GESPIC cohort and
particularly more frequent in patients with inflammatory disease (i.e. increased CRP)
(25), and more recently, as low as 3.5% patients progressing to a radiographic at 2
years in DESIR (i.e. with early disease), and particularly in patients with MRI
inflammation of the SIJ, with HLAB27 and smokers (26). These results , with 2-years
data, and given slow radiographic progression rates (26,27), suggest that the 10-year
evaluation (as initially planned in DESIR) will not be sufficient to determine the natural
history of radiographic progression and structural damage in axSpA, and therefore a
longer follow-up is warranted.
o biomarkers in the DESIR cohort
All patients who were included in the DESIR cohort underwent a blood extraction and
gave a urine sample at each study visit, resulting in more than 80 000 biologic samples
(including serum, DNA, RNA and urine).

2.1.1. Genetic markers
Although there is consistent evidence that axSpA patients share a common genetic
background, as suggested by the significantly higher prevalence of Human Leukocyte
Antigen (HLA)-B27 in patients suffering from this disease, where the allele is present in
75-90% patients with AS (28,29), the exact role of HLA-B27 in the SpA pathogenesis
has not been clearly established yet. One of the hypotheses is that, in a predisposing
genetic environment (i.e. the presence of HLA-B27), an infectious agent might trigger
the disease, but this has yet to be confirmed. Recently, other genes have been
identified as potential additional risk factors for SpA development (e.g. Interleukin (IL)-
23 receptor gene, and the endoplasmic reticulum aminopeptidase 1)(30), but to date
no specific genetic marker allows to predict the long-term outcome of axSpA patients,
both in terms of disease progression, but also for more hard endpoints such as
prosthetic joint replacement, comorbidities or even mortality. To identify genetic
markers associated with enthesis ossification and syndesmophyte progression, a long-
term radiographic assessement is necessary.
2.1.2. Other biomarkers
Research has focused on mediators of skeletal development and homeostasis as
systemic biomarkers of syndesmophyte growth. Among biomarkers of interest,
inhibitors of Wnt pathway including Dickkopf-1 (DKK-1), sclerostin, and the secreted
Frizzled-related proteins have been studied in several animal models.(31) Based on its
biological effects and animal studies, DKK-1 and sclerostin levels might be predicted to
be lower in patients with AS than healthy controls, or lower in patients with more
extensive syndesmophytes. However results of studies involving humans are
conflicting (32–35) and further resarch is necessary focusing on syndesmophyte
progression in SpA. Other biomarkers such as VEGF, MMP-3, VICM and certain
adipokines might be also interesting markers of ossification as previously shown.(36–
39)
2.1.3. Biologic samples storage
We can anticipate that many projects evaluating the impact of genetics or other
biomarkers in the outcome of patients after 10 years will be performed.

Currently all the samples are stored in the CRB (Centre de Ressources Biologiques) at
Bichat hospital, but in order to allow such analyses (i.e. after 10 years) there is a need
to continue such storage for the serum and urine bank.
o Natural history and comorbidities
Spondyloarthritis is a long-standing chronic disease that causes axial ankylosis, spine
deformities and decreases of quality of life. As bone ossification is a very slow process
(see imaging paragraph), clinical changes are often observed after 20 to 30 years of
disease evolution. Furthermore, extra-articular features are unfrequent in early
spondyloarthritis (5 to 15% of patients included in the DESIR cohort at 5 years of follow
up) and new extra-articular manifestations increase very slowly over time. To have a
chance to observe enough extra-articular features in DESIR cohort to perform
association studies looking for factors associated to the incidence of new extra-
articular manifestations, a longer follow-up is necessary.
A recently published study (the ASAS-COMOSPA study) has confirmed that SpA
patients, like RA patients, present with comorbidities particularly regarding
osteoporosis, gastro-duodenal ulcers and cardiovascular (CV) disease. (40) Some data
on comorbidities (CV disease, infections and neoplastic diseases) has also been
reported in DESIR by Gherghe et al. (41) with no higher prevalence rates of
cardiovascular disease but a greater "heart age" in DESIR patients, compared to the
general population from France. This finding (i.e. a prevalence of CV disease
comparable to the general population) might be due to the younger age of patients in
the DESIR cohort compared to ASAS-COMOSPA, where patients had a mean age of 44±
4years, vs. 34± 9 years, in the DESIR cohort. Indeed, age is a key factor for
comorbidities. Unpublished data indicate that at 5 years comorbidities are rare, and a
longer follow-up would be necessary (data on file).
Low bone mass is frequently observed in patients with SpA as well as vertebral
fractures. (42)Few studies have considered the incidence of vertebral fractures in SpA
and the factors associated with their occurence. The age was about 34± 9 years, in the
DESIR cohort at baseline, thus a longer follow-up will permit to observe new fractures
and thus will allow analysis of factors associated with their occurence. Furthermore,

bone mineral densitometry performed at 15 and 20 years will reflect bone loss in these
patients.
In this sense, a longer follow-up of DESIR patients will allow a unique exploration of
the prospective comorbidities (and also potentially mortality) and be an ideal setting
to estimate the future burden of patients with axSpA in terms of comorbidities.
o Health/societal costs
AxSpA disease onset usually occurs in the patient’s most productive years, and
therefore axSpA can have an important socioeconomic impact. Lower employment
rates in axSpA patients compared to the general population have been reported along
with increased SpA-related work disability.(43) High disease activity and loss of
physical function are the most important factors associated with the total costs of
SpA.(44) Recent data have been presented for the first 3 years of follow-up of the
DESIR cohort(45), indicating a lessening of the productivity loss compared to previous
reported data on SpA, but with costs being driven essentially by the use of biologic
drugs.
Only long-term (i.e. > 10 years) cost-effectiveness analyses, taking into account
incident comorbidities, will allow to determine (a) long term costs and consequences
and (b) if the high cost of biologics result in a higher productivity over the working-life
of axSpA patients.
o New biologics and treatment patterns
Many costly but effective biologic drugs are available in axSpA. Randomised controlled
trials give us invaluable information on their efficacy and safety in the context of trials.
However, both regulatory agencies, payers, pharmaceutical companies and users need
more information regarding these drugs.
A real-life setting such as DESIR (where treatments are freely prescribed by the
treating rheumatologist) or of great interest in this context.
Some questions include
- Who is being prescribed which drug?
- What are the treatment patterns and successions of treatments used
preferentially?

- What is the efficacy of these drugs in real life populations?
- What is the efficacy of these drugs on different manifestations of the disease?
Such questions can be at least partly answered using DESIR, since DESIR is a well-
phenotyped and well-followed up population of patients.
The DESIR cohort has been recently used to confirm the efficacy of TNF alpha blockers
in real life, with daily practice prescription conditions and this publication in a high-
impact journal raised much interest.(14) New statistical techniques such as propensity
scores allow high-quality data interpretation even of observational data such as the
DESIR data. We are aware of course that given the relatively small sample size (around
500 patients) not all questions can be answered.
With the i regulatory approval of IL-17 inhibitor and soon IL-23 inhibitor treatment for
axSpA patients, we can anticipate that a DESIR extension will be a useful setting to
perform multiples analyses pertaining to real-life conditions of use, efficacy and safety
for biologics.

4. STUDY DESIGN

Twenty-four of the 26 centres that are already participating in the DESIR cohort will
continue participating (Annex V). No new patients and no new centres will be included.
5. INCLUSION CRITERIA
No new patients will be included.
Therefore, we only remind the readers of the inclusion criteria of the DESIR cohort:
Age : 18 to 50 years old
Inflammatory back pain
o for > 3 months but <3years
o fulfilling the Calin et al. or Berlin criteria for inflammatory back
pain. (2,3)
o Suggestive of axSpA: confidence in SpA according to the
rheumatologist ≥ 5 in a 0 to 10 numerical rating scale.

Patients will fill in a home-based follow up CRF every 6 months (except for timepoints
of center visits) to collect questionnaire data.
Patients will attend a visit every 2.5 years (i.e. 4 visits in total at timepoints M150,
M180, M210 and M240 compared to DESIR baseline) at their DESIR care center, for
completion of a visit including questionnaires and a physical examination and to
perform the imaging exams and bone densitometry at the 15-year and 20-year time
points. Data collected at each type of study visit is detailed below.
6.1.1. Home-based follow-up e-CRF
1. Logistics and feasibility:
All data will be collected via a secure online e-CRF. The e-CRF is planned to be
completed every 6 months and will be desgined to be short and user-friendly. The
questionnaire should take less than 15 minutes (usually less than 10 minutes) to
complete.
Given more than 85% of French people have access to internet, along with the young
age of the patients in the cohort, we anticipate that a virtual visit will be feasible for
most of the patients. Email addresses will be collected at the 10-year visit. The e-CRF
will be explained at the 10 year visit by the nurse.

In case a patient does not have an internet access, telephone-based CRF completion
will be performed (ie someone will read the questions to the patient and enter the
answers similar to a telephone survey).
2. Schedule
Patients will be asked to fill out an e-CRF every 6 months (except for center visit
timepoints), in order both to efficiently collect treatment and disease activity data, but
also as a strategy to reduce the number of patients lost to-follow-up (i.e. the eCRF
being a reminder that they are still participating in the study) without a great burden in
terms of transport/work absence that a centre visit represents. Data collected in these
visits is detailed below.
Then one week before completion is due, patients will receive (a) a first email, (b) a
second email in case of non response, (c) a third and final email in case of non
response then (d) up to 2 phone calls from a call centre to perform a telephone-based
completion of the questionnaire taking around 15 minutes.
The first email is sent 1 week before the theorical date, the second email is 2 days after
the date, and 3rd email 1week after the first email. The patient is called between day 8
and 16 post theoretical date. The questionnaire stays available until filled or until the
next theoretical questionnaire arrives. Confidentiality is fully addressed in section 6 of
this present protocol.
6.1.2. Centre visit
1. Schedule and practical aspects:
Classical centre visits will occur every 2.5 years (i.e. M150, M180, M210 and M240)
(Figure 1).
The CRO will remind the centres to contact the centres to ask them to schedule the
centre visit 8 weeks before.
As is currently the case in the DESIR centres, the study personnel sends blood test
prescriptions if needed, and organizes the imaging and bone densitometry at year 15
and 20.

The CRF is filled in paper format OR directly into a dedicated e-CRF. If a paper format is
used, the data are then entered into the e-CRF by the local study personnel.
The organisation is currently fluid and successful in DESIR which is another reason to
use the existing infrastructure.

2. Clinical data
Disease data will be collected using widely validated questionnaire.
The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI)(46), the Bath Ankylosing
Spondylitis Functional Index (BASFI) (47) and the Bath Ankylosing Spondylitis Global Score (BASG)
(48) will be collected. Patients will also be asked to enter their last available CRP (last 6 months) in
order to allow the calculation of the Ankylosing Spondylitis Disease Activity Score (ASDAS-CRP).(49)
3. Treatment information
Finally data on NSAID and biologics treatment will also be collected using drop-down menus.
We anticipate patients will take 5 to 15 minutes to fill in this e-CRF.

The French version of the International Physical Activity Questionnaire-Long form
(IPAQ-L).(59) The IPAQ-L consists of 27 questions, evaluating the time spent during

the preceding week doing moderate or vigorous activities, walking and sitting. It
includes professional and home-related activities as well as transport and leisure
activities. The result can be expressed as a categorical variable (low, moderate or
high physical activity) and as a continuous variable, in Metabolic Equivalent of Task
(MET)-minutes per week (1 MET is defined as 1 kcal/kg/hour for the average adult
and is roughly equivalent to the energy cost of sitting quietly). A sufficient level of
physical activity is defined as proposed by the World Health Organisation as either
150 minutes of moderate-intensity aerobic physical activity or 75 minutes of
vigorous-intensity aerobic physical activity per week, or an equivalent of moderate
and vigorous activity, performed in bouts of at least 10 minutes’ duration.

Bone densitometry:
Osteoporosis is the most frequent comorbidity in SpA (40), therefore, in order to evaluate the
presence of a low bone mineral density, a DXA-scan will be performed at year 15 and 20.
This will include evaluation of bone densitometry of the lumbar spine and femoral neck.
Schedule: Only those centres equipped with the appropriate devices will collect such data.
2. Biological data
Biological data will be collected as part of usual care, i.e. no specific blood test will be performed
for the study, but data will be collected upon the available blood test performed in the 6 months
preceding the visit. If no blood tests have been performed over the last 6 months this is not in

agreement with good clinical practice; in this case, patients will be sent a blood test prescription for
a simple very basic blood test comprising:
Acute phase reactants: Erythrocyte sedimentation rate and C Reactive protein
Hemoglobin
White cell count
Platelets
Creatinine
Cholesterol + tryglycerides
Fasting glucose.
3. Imaging data
Since imaging outcomes are the main objective of the continuation of the DESIR cohort, imaging
(i.e. MRI and X-rays of the spine and the SIJ) will be performed every 5 years, as recommended in
usual care.(60)
Plain Xrays:
These Xrays include the cervical, thoracic and lumbar spine (AP and lateral) and a pelvic Xray, and
should allow to calculate the modified Stokes Ankylosing Spondylitis Spinal Score (mSASSS) (19) ,
but also to show a potential cyphosis or vertebral fractures.
Techical aspects is decribed in ANNEX III
Timing: these Xrays will be collected at the center visits at M180 and M240. Each investigator will
be asked to evaluate the sacroiliac joints according to the modified New York grading of the
sacroiliac joints (24), the mSASSS of the spine, and to report the results in the paper CRF.

will proceed with the anonymisation of the imaging. Thereafter, the anonimized images will be
ready to be sent out to the central readers, who will be selected by the DESIR Imaging Task Force.
MRI:
An MRI of the cervical, thoracic and lumbar spine and of the sacroiliac joints will be performed at
M180 and M240. Technical aspects are described in ANNEX IV.
Each centre will be asked to score the MRI according to the scoring detailed in ANNEX IV and to
report the results in the CRF.
In parallel, a central Reading of the Xrays will be organized:
For each patient, the images in DICOM format will be recorded in a CD with the following
information in the CD:
o Name of the investigator
o Center number
o Patient number
o Patient initials
o Exams date
These CD will be sent to the Clinical Investigation Center who will verify the content of the CDs and
will control the anonymisation of the imaging. Thereafter, the anonimized images will be ready to
be sent out to the central readers, who will be selected by the DESIR Imaging Task Force.
7. SAFETY
Non-interventional studies involving humans pose no risk to participants.
Any adverse reactions observed in the participants are reported by the investigators as per the local
vigilance procedures according to standard of care.
8. DATA MANAGEMENT
1. Right of access to data and source documents
Persons with direct access to data, in accordance with applicable legal and regulatory provisions,
notably sections L.1121-3 and R.5121-13 of the French Code of Public Health (investigators, quality
control managers, investigators, clinical research assistants, auditors and any other people working

on trials) shall take all the necessary precautions in order to ensure the confidentiality of
information relating to the investigational medicinal products, to the trials, to persons taking part
and notably concerning their identity and the results. The data collected by these persons during
quality controls or audits shall be made anonymous.
2. Transmission of data from the case report form to service providers
At each centre visit, the investigators enter the data into a e-CRF onsite. The data will then be
consultable on a secure internet site accessible by the coordination centre only. This will enable
centralised monitoring and amendments, tracked in the database directly.
3. Data and document storage
All data and documents for the study will be kept for 15 years after publication.
9. QUALITY CONTROL
1. Qualifications of collaborators
The Coordinating Investigator, the qualified person or the Scientific Director will ensure that all
collaborators are qualified to perform their respective tasks.
2. Data quality
The CRAs representing the coordination team shall visit the investigation centres according to the
separate document (specifications).
3. Transcription of data in the case report form
All information required by the protocol must be provided in the electronic case report form. The
investigator must provide an explanation in the case of missing data.
The data must be transferred to the case report forms as it is collected, whether it is clinical or
paraclinical data.
Anonymity of patients shall be ensured using a code number and the initials of the person taking
part in the research, which will feature on the research documents. Patients’ names shall also be
erased from copies of source documents used for documenting the research, if appropriate.

Computerised data on file shall be declared to the CNIL, the French data protection authority,
according to the relevant procedure.
10. ETHICAL AND LEGAL CONSIDERATIONS
The type of follow-up of this second part of DESIR is usual care, since patients will complete well-
validated, widely used questionnaires that would be anyway completed at any rheumatologist visit
every 6 months, and will not undergo any additional biological or imaging exams besides the usual-
care follow-up.
This present protocol will be submitted to the Ethics Committee and the Commission nationale
informatique et libertés (CNIL).
1. Role of the sponsor
Assistance Public - Hôpitaux de Paris (AP-HP) is the sponsor of this study, and it is represented by
the DRCI.
2. Applicable texts
The study will be conducted in accordance with:
- the updated version of the Declaration of Helsinki;
- French Good Clinical Practice guidelines, version dated 30 November 2006;
- Article L.1121-3 of the Code de la Santé Publique
3. Request for opinion from the French ethics committees
In accordance with section L.1123-6 of the French Code of Public Health, the research protocol
must be submitted to an ethics committee by the sponsor. The opinion of this committee is notified
to the competent authority by the sponsor before the research starts.
4. Responsibilities of the investigator towards the sponsor
The Coordinating Investigator or the qualified person shall provide the sponsor with all information
about the enrolment of participants on the study.
Any amendments to the protocol must be submitted to the sponsor.
Assistance Publique-Hôpitaux de Paris (AP-HP) reserves the right to terminate the study at any time
for administrative reasons.
5. Data confidentiality and declaration to the CNIL

As the processing of personal data for this study is not governed by the MR 003 Reference Method,
the sponsor must obtain approval from the CNIL.
The investigator must keep the list of patients screened / included in the study up to date, by an
initials system, in the same way as in the case report form. In order to respect patient anonymity,
an anonymous code, made up of the number of the investigation centre and the patient’s study
entry number, will be allocated to patients per centre, according to their order of inclusion in the
protocol. Only the first letter of the patient’s last name and the initial of the first name will feature
on the case report form.
6. Computerised data file
The computer file containing the personal data collected for the research must be declared before
the effective start date of the research, according to law.
A specific reference method for personal data processing in biomedical research studies, set out by
the law 2004-806 of 9 August 2004, as entering into the scope of sections L.1121-1 and following of
the French Code of Public Health, was established by the CNIL in January 2006. This method
provides for a simplified declaration procedure where the type of data collected in the research is
compatible with the list in the CNIL’s reference document.
7. Non-anonymous file
The computer file featuring the full names and addresses of the patients included in the study is
currently stored at the URC Paris-Centre only. An authorisation application was submitted to the
CNIL for the file as a whole and single entity.
8. Information leaflet and informed consent form
The information given to the participant will be recorded in his/her medical file. The participant
must be given an information sheet.
Patients attending the M120 visits (i.e. last visit of the first part of DESIR) will sign a consent form,
accepting to continue in the cohort for at least 10 more years, and attend the center visits,
including the imaging exams every 5 years.

Patients must read and examine the content of the form before signing and dating it. A signed copy
will also be given to them for them to keep. Patients cannot take part in the study without having
given their informed consent.
Patients may withdraw their consent at any time, without this having an effect on the quality of
their treatment.
Participation in the study is voluntary. Patients may only take part in the study if they have given
their free and informed written consent.
Patient anonymity shall be observed during the study and in publications where applicable.
Three copies of the consent form shall be initialled and signed by the patients and the investigator.
A copy of the document will be given the patient; the investigator shall keep the second copy in
their archives for at least 15 years after the end of the study; the third copy will be submitted to the
sponsor in a sealed envelope at the end of the study.
All data collected will be processed so as to guarantee the highest level of confidentiality. Patients
will be informed that the data collected during the study will be computer processed in accordance
with law 78-17 of 6 January 1978 (French data protection law, amended by law 94-548 of 1 July
1994).
Patients will also be informed that the clinical research assistants designated by the Paris Centre
Clinical Research Centre, health authority inspectors and members of the ethics committee, may
request access to source clinical data in order to verify the data in the case report form. The
informed consent form should specify this point in an explicit manner.
9. Data processing and archiving of research documents and data
Research documents subject to the law studies involving humans, must be archived by all parties
for 15 years after the end of the research.
The following will be archived:
• Copies of the letter of authorisation from the ethics committee
• Successive protocol versions (identified by the version no. and date),
• Patients’ signed consent forms in a sealed envelope with the matching inclusion list or
register,
• All appendices specific to the study,

8. Final study report
The report will be produced within 12 months from the last follow-up visit of the last participant.
11. PUBLICATION RULES
The sponsor, AP-HP, is the owner of the data. The data cannot be used or disclosed to a third party
without its prior permission.
The signatories listed first in any publications will be the individuals who have actually participated
in the development and implementation of the protocol and the preparation of its results.
Assistance Publique-Hôpitaux de Paris (AP-HP) must be named as the sponsor of the study, and the

1. Distance occiput – mur Le patient doit se tenir les talons, et si possible le dos, contre le mur. On lui demande de faire un effort afin que sa tête touche le mur sans que le menton soit au-dessus de sa position normale. Le meilleur de deux essais est à retenir 2. Indice de Schöber modifié Le patient doit se tenir debout. Ce test consiste d’abord à marquer un point en regard de l’apophyse épineuse de L5 (première épineuse se situant au-dessous d’une ligne rejoignant les deux crêtes iliaques). Le deuxième point est mesuré à 10 cm directement au-dessus du premier alors que le patient est en position neutre (sans effort d’hyper extension ni de flexion). On demande alors aux patients de se pencher en avant aussi loin que possible sans plier les genoux. La distance entre les deux points est alors mesurée. 3. Distance tragus - mur Le patient doit se tenir les talons, et si possible le dos, contre le mur. On lui demande de faire un effort afin que sa tête touche le mur. La distance tragus-mur est mesurée à droite et à gauche. La moyenne des mesures droite et gauche est retenue. 4. Flexion latérale du rachis Le patient doit être aussi proche du mur que possible avec les épaules au même niveau, bras le long du corps. L’investigateur mesure la distance entre l’extrémité du majeur et le sol (distance A) Puis, l’investigateur demande au patient de se pencher d’un côté (par exemple gauche) tout en maintenant les épaules collées le plus possible contre le mur. Par ailleurs, il est vérifié que les chevilles et les genoux restent dans la même position. A ce moment (fin de la flexion latérale), l’investigateur mesure la distance entre l’extrémité du majeur et le sol (distance B). La flexion latérale du rachis est la DIFFERENCE ENTRE LES MESURES A ET B. Il faut répéter cette mesure pour chaque côté et REPORTER DANS LE CAHIER la meilleure des 2 mesures. 5. Ampliation thoracique Elle sera mesurée comme la différence en centimètres entre l’expiration et l’inspiration profonde mesurée sur la ligne mamelonnaire. Le meilleur des deux essais sera retenu. 6. Rotation cervicale La rotation cervicale est mesurée grâce à un goniomètre. Le patient est en décubitus dorsal, en position neutre, et le goniomètre est placé sur son front. Il est ensuite demandé au patient de tourner la tête aussi loin que possible à droite, puis à gauche. Le meilleur des deux essais est retenu pour chaque mesure (droite et gauche). La moyenne des mesures droites et gauches donne le résultat final pour la rotation cervicale en degrés. 7. Distance inter-malléolaire La distance inter-malléolaire est mesurée sur un patient en décubitus dorsal, les genoux en extension et les pieds en position neutre. Il est demandé au patient d’écarter les genoux au

maximum et la distance entre les deux malléoles est mesurée. Le meilleur de deux essais est recueilli. 8. Distance doigt - sol Le sujet doit se tenir debout, pieds nus, les pieds rapprochés. Ensuite, il se penche vers l’avant le plus possible en essayant d’atteindre le sol avec les doigts sans plier les genoux. La distance entre l’extrémité du majeur droit et le sol est alors mesurée avec un centimètre gradué à 0,1 cm.

o ANNEX III: Methods for the acquisition of radiological data
Methods for the acquisition of radiological data, and for transmission and verification
of the quality of centralised data, and for analysis of non-centralised data
1. Acquisition methods
Thoracic spine, side-on
35 x 43 cm x-ray film.
The x-ray film is centred on the 7th thoracic vertebra.
As landmark, the upper part of the film must be 4 to 6 cm above the upper part
of the shoulders.
The subject identification number must feature at the bottom of the film.
The patient is placed lying down on their left side, with the coronal plane of the
spine in line with the middle of the table (if for any reason this image must be
taken with the patient lying on their right side, please explain why on the data
transfer form, and use the same position for all images).
The patient’s arm must be placed at a right angle to the body (in order to avoid
superimposition with the vertebrae).
The knees and hips must be bent (to avoid rotation of the pelvis, and the spine
must be kept as straight as possible).
Adjust focal-film distance (FFD) to 100 - 120 cm.
Direct the central ray to the spine, for beam to penetrate approximately 4 to 6
cm below T7.
Right or left markers should be used while avoiding superimposition with
relevant anatomical elements.
Bone structures should be visible from at least T3 to L2, and T3 and L2 vertebral
bodies should be visible
The patient must breathe in to maximum inhalation and must not move. 4.
Lumbar spine, side-on

The patient is placed in the same position as for the thoracic spine side-on
The x-ray beam must penetrate at iliac crest level (L4)
A 35 x 43 cm film must be used and placed lengthwise, at iliac crest level, label
facing downwards
The focus-film distance is the same as for the thoracic spine side-on
Breathing must be blocked at maximum exhalation.
Bone structures must be visible from T12 to the lumbosacral junction, including
the upper third of the sacrum, on a single image. Images centred on L5-S1 should
not be used.
Lumbar spine, from front, anterior posterior
The patient must be placed in anterior posterior supine position with the
median line of the body in the sagittal plane, aligned with the middle of the x-ray
table.
The patient’s arms should be placed either side of the body or resting on the
chest, so as not to appear in the image.
Bend the knees so as to place the lumbar spine flat against the table, thus
reducing lumbar lordosis.
Direct the central ray perpendicularly to the spine. The beam must penetrate at
the median line at L3 (usually the lower part of the costal margins).
A 35 x 43 cm film must be used and placed lengthwise, label facing downwards.
Focal distance must be 100 to 120 cm.
Breathing must be blocked at maximum exhalation.
Bone structures must be visible from T11 to the lumbosacral junction, including
the upper third part of the sacrum.
Cervical spine, side-on
The patient is placed side-on to the left, either sitting or standing, with the
shoulders in contact with the film, to prevent rotation. The same profile must be
used for all images taken during monitoring (if images side-on from the right must
be used).

Ask the patient to relax and to relax the shoulders as much as possible (it must
be noted that C1 to T1 must be visible to be able to allocate a score to the images,
and it may be necessary to ask the subject to hold weights to view T1).
Slightly raise the chin so as to avoid superimposition of the mandible and the
upper cervical spine.
Align the central ray perpendicularly to the film, with the beam directed
horizontally to C4-C5.
A 24 x 30 cm film must be used and placed lengthwise, landmarks being placed
at the anterior and superior parts of the cervical spine and centred with respect to
the beam.
Focal distance must be 183 cm.
The entire cervical spine must be visible from C1 to T1, without rotation. The
mandibular nerves must not be seen over the upper cervical spine. Optimal
exposure is achieved when the soft parts and margins of the upper spine are visible
along with the bone framework of the entire cervical spine.
Pelvic x-ray, from front
The patient must be placed in supine position, with the median line of the body
in line with the middle of the x-ray table. Pelvic rotation should be avoided.
The patient’s arms should be placed either side of the body or resting on the
chest, so as not to appear in the image.
The knees and hips must be bent (to avoid rotation of the pelvis, and the spine
must be kept as straight as possible).
Direct the central ray to a point on the median line and at an equal distance
from the anterior superior iliac spines and symphysis pubis.
A 35 x 40 cm film must be used and placed width wise, with the identification
label facing downwards and centred with respect to the median ray.
Focal distance must be 100 to 120 cm.
The film must include the soft parts at the sides.
Breathing must be blocked at maximum inhalation.

The entire pelvis and proximal part of the femurs must be included in the image.
Optimal exposure is achieved where the margins of the sacroiliac joint and the
entire lumbosacral pelvic junction are visible.
2. Methods for the collection and transfer of non-centralised radiological data
The sacroiliac joints, hips and spine will be evaluated and noted directly in the case
report form by the investigator, according to New York, BASRI and mSASSS scores
respectively.

o ANNEX IV: Methods for the acquisition of MRI images
The magnetic field applied should be between 1 and 1.5 Tesla. Regular calibration by
the manufacturer is required.
Three body areas will be monitored in the MRI examination: upper spine (C2 to T10),
lower spine (T8 to S1), and the sacroiliac joints. It must be ensured, by repositioning
the patient on the table, that the two spinal image acquisition levels enable
overlapping of at least 2 vertebrae. The sequences shall be acquired in the sagittal
plane, except for the sacroiliac joints, which will be studied on semi-coronal slices,
parallel to the long axis of the sacrum.
Two types of acquisition will apply for each of the three levels of exploration: T1-FSE
and STIR (see table). A contrast product will not be used.

The clinical investigation centre shall provide the MRI team with a copy of the page
from the case report form concerning the proximate MRI analysis for each patient (see
below).
Within the next 7 days, the MRI centre shall send the images, along with a standard
report and test results on the case report form page back to the investigation centre.
The clinical investigator centre will then include the MRI data in the case report form.
