The purpose of the activity are the following: a.) to monitor Division implementation and fund utilization of the School-Based Feeding Program (SBFP); b.) to monitor the Division implementation of Oplan Kalusugan sa DepED; c.) to identify SBFP beneficiaries for the Oral Urgent Treatment and d.)to validate (2) schools implementing best practices on SBFP and OK sa DepED Programs. 3. The schedule of monitoring and evaluation will be on September 12, 2019. 4. All schools are required to have the following in the feeding center, such as: • SBFP Forms 1,2, and 3 • WFP, PPMP, APP and attachments • Gantt Chart • Attendance logbooks (PTA meetings, parent volunteers, feeding beneficiaries) • Minutes of any meetings conducted prior and during implementation • 20 Days Cycle Menu tarpaulin • 2019 launching documentation • Partners and stakeholders lists and documentation if there's any • A copy of DO No. 39, s. 2017 5.
