Dear Students and Parents,

It’s that time of year again to begin preparations for our 38th annual recital. Participation in this production is NOT MANDATORY! It is your choice to participate. Ticket price for all seats: $20 which includes a $2.00 service fee per ticket. Tickets shall be purchased and ordered online. Ordering information and procedures will be released along with your scheduled recital show dates in January 2016. Dance participants DO NOT NEED TICKETS, unless occupying a seat for the majority of time during the performance. This admission pays for the rental of the auditorium and all other expenses incurred in putting on the production. The recital will be held at The Performing Arts Center in Lake Zurich, at 300 Church Street. In order to provide an entertaining production that is not lengthy, the scheduling format is as follows: Most classes perform in only one or two shows. Level 4/5 classes will be scheduled to perform in ALL SHOWS.

Please note: every effort will be made to accommodate siblings and relatives to perform in the same show. However this is not always possible. Students who are enrolled in multiple classes, to avoid a lengthy recital, there is no guarantee that all dances will be performed in one show, some routines may be performed throughout the performance week which would mean purchasing additional tickets for multiple shows. However, please be assured that every effort will be made on your behalf to accommodate needs when possible.

Costume Information For Combination Classes: You are only required to purchase ONE COSTUME per class. There may be an additional charge for accessories not to exceed $25.00. Costume Prices

All Recital Fees and signed forms are due by Saturday December 19, 2015

By this requirement, we hope to avoid “late order” fees, recital participation verification phone calls due to misplacing, or non-delivery to parent, of recital information. Deadline also guarantees all registered students are accounted for.

Beginning on Monday January 4, 2016 there will be a $30 late fee added (per costume). The costume companies levy this charge. All costume orders received after the due date must be filled as a separate order. These costumes will arrive AFTER we receive our initial shipment.

Students requiring three or more costumes, financing is available. Please see the office for more information. Costumes will not be ordered unless payment is in good standing. Please try to make the deadlines so as to avoid any late charges or penalties.

Other Services Offered at the Recital

Video by Laura Lockwood Productions ~ Look for order forms in the Spring. Videotaping with phones and other personal recording devices is not permitted during the performance.

Portraits by Mitchell Studios will be photographing recital portraits during the combination classes’ dress rehearsal. Scheduling and more information TBA

Flowers and gift items available at all performances.

Volunteers If you are interested in volunteering for the 2016 recital, please contact Sasha Vargas at sashvargas@aol.com. We need volunteers for ushering, backstage supervision and other areas as needed. Thank you!

Denise Sabala Dance Studios Agreement to Participate in the 2016 Recital

Please read the following information carefully before signing the agreement, which is required by Saturday December 19, 2015

1) I understand once I purchase a costume or costumes, there are NO REFUNDS issued once the order has been placed.

2) I understand that is I choose to cancel my costume order, there is a $50 cancellation fee if the costume has already been ordered.

3) I understand that if I have ordered the wrong size costume and it needs to be exchanged for a smaller or larger size, there is a exchange fee involved to do so of $35.00 in addition to a balance or refund depending on the size exchanged.

4) I understand beginning January 4, 2016 there will be a $30.00 late fee added (per costume). All costume orders received after the due date must be filled as a separate order.

5) I understand that my costume may not be picked up until my tuition account is in good standing along with any other outstanding studio charges that remain.

6) I understand that if I ordered my costume late I will not hold the studio responsible for any delays and will be responsible for shipping or overnight costs if needed.

7) I understand a staff member has been assigned to backstage operations during the performance, to ensure the safety, protection and welfare of my children. I agree to abide by the rules of the backstage procedures so as to not jeopardize the safety of the performers.

8) I understand that the studio will make every effort to schedule siblings to perform in the same show, however due to the amount of studio families this can not be guaranteed.
