(DSO) Ministry as responsible for serving as the single point of access for adult developmental services and supports funded under SIPDDA in that geographic area. Functions include providing information, intake support, confirming eligibility, assessing service/support needs, and supporting the referral process.

APPLICATION FOR DEVELOPMENTAL SERVICES & SUPPORTS (ADSS)

The ADSS gathers intake and demographic information about the persons’ current situation, and circumstances in order to understand the persons’ goals and priorities. It also gathers information related to current services and supports, service requests, care concerns, and unpaid caregiver information as well as medical and behavioral concerns. This is one part of the DS Application Package (along with the Supports Intensity Scale (SIS)).

ATTACHMENT An attachment is a supplemental file, e.g., a text document that is attached to a case. The DSCIS user can attach scanned documents that provide information in support of a case such as a birth certificate, bank statement, or psychological assessment.

A range of file types are supported including Word, Excel, and PDF.

AVAILABILITY END DATE

Date when a resource is no longer available.In the context of ‘Residential’, this date is defaulted to the end of the fiscal year if funding is FISCAL.This date is not relevant if funding is ANNUALIZED.

BOOKMARKS Placeholders in the DSCIS system for users to mark cases in which there is a special interest. A list of bookmarked cases is automatically maintained. Users can add and remove cases from this list through the home page or shortcuts panel (on the cases tab).

CASE A case represents all the information to support a person and his/her social network (e.g. family members, neighbors, professionals).

CASE MEMBER The person whose circumstances gave rise to the request for adult developmental services.

CASE TYPE Typically labelled as ‘Type’. Possible values are: Developmental Services and Supports, DSCIS Eligibility, Development Services and Support Plan, Eligibility Stage 1, Eligibility Stage 2, Eligibility Stage 3

CASE

CIRCUMSTANCE Represents information about a person receiving developmental services. The information is used to assess the level of supports needed and to design an efficient service support profile.

A circumstance (income, asset, expense, employment barrier, medical condition, etc.) is a condition of fact that is used to assess needs and determine eligibility. For example circumstances can be that a person is disabled (requiring social adapted assistance) or a person is francophone (requiring services in French) etc.

In some situations, a circumstance may be determined by another circumstance (e.g. a person experiencing an employability barrier may be determined by the level of education and skill, the assessed person need may be training).

Usually a circumstance needs to be verified and the verification result is captured in a verification code. That result may influence the status and status reason.

PROSPECT PERSON A person who:- has supplied insufficient information to be

registered; or- the organization does not wish to register in DSCIS

The prospect person allows the organization to fully interact with the person without them being fully registered in DSCIS.

PROVIDER MEMBER The user associated with the Service Agency and is involved in declaring resources.

PROVIDER RESPONSE Agency's decision to accept or reject the person as a match for their service.
