Outside catering company must provide hotel with proof of liability insurance of not less than $5,000,000.00 in coverage.

Outside catering company must be in contact with the hotel event manager at least 2 weeks prior to the event.

Use of hotel kitchen facility is prohibited by any outside catering company and/or vendor. No exceptions.

Hotel agrees to provide the appropriate number of chafing dish inserts, serving utensils and other miscellaneous items used to display a full buffet presentation.

Outside catering company agrees to provide platters to be used for any cold item food displays.

All food provided by outside catering company should arrive to Hotel complete and prepped. Hotel agrees to provide outside catering company with appropriate number of hot boxes to maintain the food until served

Outside catering company must provide Hotel with the full menu at least 1 week priorto the event.

Outside catering company will be informed of hotel loading dock procedures by the event management team. Upon arrival, all members of the outside catering company will need to report to the Loss Prevention department to receive a visitors badge.

Associates from the outside catering company should always wear a visitors badge while on property. Failure to abide with this policy will result in removal from the property.

Associates from the outside catering company are not permitted to use the Hotel associate cafeteria.

Associates from the outside catering company are permitted to use the associate restrooms. Under no circumstances are members from the outside catering company permitted to be in the front of the house.

Catering Company’s Acknowledgement

The signature below indicates that patron and catering company have read and understand these regulations and agree

to it’s contents. Failure to abide by any of the above listed regulations will result in immediate removal from the Hotel

premises.

HOTEL POLICY

It is the policy of this Hotel to discourage Hotel patrons from purchasing food and from using the Hotel facilities to prepare

or to prepare and serve food to Hotel guests. Improper handling of food may lead to food poisoning and other health

hazards. However, the Hotel recognizes that for religious or other special reasons a Hotel patron may have , it may be

necessary to permit such preparation and service. In the event that a patron insists on the purchase, preparation or

serving of food by persons other than Hotel employees, the Hotel may allow such activities only if such patron

acknowledges, by its signature below, its agreement to accept the responsibility and abide by the terms set forth in this

agreement.

WAIVER

The undersigned patron (“Patron”) agrees to waive any claim for damages of any nature whatsoever and to release the

Hotel, the Hotel owner, the Hotel Manager, Crowne Plaza International, Inc. and each of their respective subsidiaries,

affiliates, officers, directors, employees and agents from any liability or responsibility whatsoever for any ill-effect, injury, or

Loss incurred by Patron or any third party including, but not limited to, all manner of actions, causes of action, suits, debts,

damages, claims, demands, costs, losses and expenses of any type or kind whatsoever, arising from, connected with or

related to the purchase of food and the preparation of serving of food to Hotel guests by persons other than Hotel

employees.

INDEMNIFICATION

Patron agrees to indemnify, defend and hold harmless the Hotel, the Hotel owner, the Hotel Manager, Crowne Plaza

International, Inc. and each of their respective subsidiaries, affiliates, officers, directors, employees and agents from and

against any liability, claims, actions, causes of action, suits, demands, damages, judgments, costs, losses and expenses,

including reasonable attorney’s fees, to which any of the above-named parties may be subject, including, but not

limited to, any claim for any injury to or the sickness or death of any person or persons, or for damages to property or

otherwise, arising from, connected with or related to the purchase of food and the preparation of serving of food to

Hotel guests by persons other than HotelEmployees.
