The Queen’s University Faculty of Health Sciences’ Office of Continuing Professional Development and Palliative Care Medicine, in cooperation with the Department of Family Medicine invite you to participate in the small-group CPD course.

A FREE COURSE!
This course is funded by the Ministry of Health, Kingston Area Long-term Care Division. There is no registration fee, only a $100 deposit which will be returned when you complete the course.
ELIGIBILIT Y
Family physicians living within LHIN 9 and LHIN 10 are eligible to participate in the program.
FINANCIAL SUPPORT
You may claim up to a maximum of $600 for expenses related to attending this course, such as travel, accommodation and the cost of a locum to cover your practice. If you are a rural physician, you may be eligible for additional funds from the OMA (to which you must apply directly) to offset additional expenses.
REGISTR ATION
Registration is limited to 10. Registrations are accepted based on the order in which they are received and on distribution of positions among the eligible counties. Your registration will be confirmed. A deposit of $100 is required. The deposit will be returned if your registration is not accepted, or at the end of the course.
CREDITS
This Group Learning program meets the certification criteria of the College of Family Physicians of Canada and has been certified by Queen’s University for up to 50 Mainpro+ credits.

PRE- AND POST-PROGR AM REQUIREMENTS
Participants must complete a needs assessment before the program and a reflective evaluation and a preceptorship (approximately a one-day commitment) following the program.
