Subject Material Test Result Findings from supplier: Carmel Forge Applicability All P&WC Engine Models In the coming days, we expect media coverage related to a United Technologies-owned company that supplies forgings to Pratt & Whitney Canada. The report involves past metallurgical testing data practices. We wanted to provide you with some background information in advance. We can assure you that this matter was extensively investigated at the time, corrective actions taken, and it was determined that all engines and parts delivered to our customers during the period met our performance, integrity, durability and safety requirements. Carmel Forge, a United Technologies-owned company located in Israel, produces forgings for disks, seals and cases for a variety of engines and land-based turbines. Carmel Forge is a self-contained forging shop, performing all processes including raw material preparation, heat treating, semi-finished machining and testing. In June 2011, an extensive investigation was immediately launched by Carmel Forge management after being alerted through the UTC Ombudsman program that some test data had been adjusted by certain employees in the past. This was done to avoid additional statistical process control testing, with the belief that adjustments to data were within an acceptable margin of error. A team of Pratt & Whitney investigators and Quality engineers were dispatched to Carmel Forge to investigate the allegation. The investigation included additional metallurgical testing, modeling and validation from which it was confirmed that there was no impact on product performance, integrity, durability or safety. The investigation was shared with the appropriate regulatory authorities, including the Federal Aviation Administration (FAA), Defense Contract Management Agency (DCMA) and Transport Canada, and no subsequent actions (airworthiness directives or service bulletins) were required for delivered components. Carmel Forge has implemented a variety of corrective actions, including software controls, changes to the reporting structure, retraining employees, and personnel changes. We recognize this may raise concerns for you and your customers and we want to reassure you of our commitment to the highest standards of business conduct. Rest assured that we have taken all necessary measures to ensure the integrity of the parts manufactured during that period, and corrective actions were implemented to prevent reoccurrence. P&WC has recently been informed that, in some cases, operators may experience difficulty when inserting an O-ring in oil filters P/N 3059256-01, 3059257-01, 3059258-01, or 3070975-01 that were procured between September 2010 and October 2011 (Figure 1).

Operators experiencing difficulty should return such oil filters to the entity that sold the part in order to obtain credit consideration.
