The National Conference On "Integration of Yoga(specially meditation) into Medical Science" was organised by Dr. Chhajer on 18th and 19th October,2014 and it was a great event. MBBS, MD and DM doctors sat with the Yoga experts in this meet to find out how the medical science would benefit by combining Yoga to fight lifestyle diseases especially Coronary Heart disease. Dr.Chhajer's Hindi book titled "Preksha meditation se Hriday Rog Ka Upchar" was also released on this occasion by Mr. N Sanyal, IAS, the Health Secretary (Ayush) Govt. of India. The second important thing that happened in the last one month was the Inauguration of Saaol's new center in Kerala. India Hospital in Trivandrum, the capital of Kerala has opened a unit of Saaol in their hospital. On the 31st October, 2014 Dr.Chhajer inaugurated this Center. Dr.Saroja Nair, who is the owner of this hospital and Mr. Ram Kamal - the

co-ordinator organised a talk on this occasion where Dr.Chhajer's book on "Reversal of heart Disease" in the local Malayali language was also released. Saaol camps were held in Bangalore, Delhi and HCC camps were held as per our announcement. Now we have a full fledged head office in Delhi in Chattarpur, which was also inaugurated on 8th November, 2014. In the next six months we have tied up to open new centers in Bikaner, Bhubneshwar, Nagpur, Thane, Anand and Bilaspur.

Every heart patient knows that it is the fats that create the blockages - but confusion prevails about the names. People are looking for a clear cut knowledge but the medical doctors do not have time to explain the terms like Cholesterol, Triglycerides, LDL, HDL, VLDL, Lipids, lipoproteins, trans fats, saturated oil, mono unsaturated oil ...... and so on. The oil companies also give misleading advertisements to sell their products. The medical scientists also have differences of opinion regarding role of each in the creation of the blockages. New research results are coming every day. The ultimate result is a complete confusion in the mind of heart patients. In this issue of Saaol times we want you to clearly know about the most important of all the fats - the Cholesterol. The second important fat that matters is Triglycerides. Forget the other jargons like LDL, VLDL and to some extent the HDL. Remember our food do not contain HDL, LDL. Our food c o n t a i n s C h o l e s t e r o l a n d Triglycerides. These fats when combined with proteins make LDL, VLDL and so on. Our foods do not contain any LDL, HDL or VLDL. So, what the patients can do in diet is s i m p l y c u t C h o l e s t e r o l a n d Triglycerides. So, for the sake of practicality a heart patient must remember they need to cut down all animal foods as they contain Cholesterol and Oils/ nuts which are full of Triglycerides. The oil companies sell litre of Triglycerides and

write "Cholesterol FREE" on the container. These are completely two different items. Remember Cholesterol is very close to LDL and Triglycerides are very close of VLDL. When the Cholesterol increases the LDL will also increase and when the Cholesterol is decreased LDL will also decrease. The same applies to Triglycerides and VLDL. HDL is another item - which matters next to Cholesterol and Triglycerides. It is mainly a protein combined with a harmless fat called Phospho lipid. When it passes in the blood it has capacity to pull out Cholesterol and Triglycerides and take them inside. That is why it is called Good Cholesterol. This HDL can not be bought from the market but it is produced in the body. To increase HDL one must do exercise, consume green vegetables and walk daily. HDL should not be less and should be atleast 25% of Cholesterol in the blood. This is called Cholesterol HDL Ratio. It should be less than 4 (ie. HDL should be more than 25%). One must not have any confusion about the blood levels of these fats. For a heart patients we need to have the best. It should not be middle or highest in the range but it should be lowest of the range. Saaol wants Cholesterol at below 130, Triglycerides below 100 and HDL ratio less than 4. These are good enough to protect you from a heart attack.

What is Cholesterol?

Cholesterol is the most known name in the field of heart care. Ask anyone "What is the major cause of heart disease?" - the answer will be "Cholesterol". It is a kind of fat that is present in our blood - and if in excess - would lead to formation of blockages in the coronary arteries. Since heart disease is the most common disease of the mankind - a huge amount of research has been done on this fat and at least three Nobel prizes have been awarded on this subject in the past years. C h e m i c a l l y s p e a k i n g Cholesterol is a long chain fatty acid consisting of 27 carbon atoms, 46 hydrogen atoms and 1 oxygen atom. It is supposed to be the highly decorated small molecules in biology. This tiny molecule plays a major role in the heart disease . It has indirectly proved more fatal then the most powerful atom bombs. When there is too much cholesterol in your blood, it builds up in the walls of your arteries, causing a process called atherosclerosis, which leads to heart disease. The arteries become narrowed and blood flow to the heart muscle is slowed down or blocked.

Cholesterol is waxy type of substance found in the cells of the body. The body needs cholesterol to make Vitamin D hormones and substances that help in the digestion of food. The normal blood level (scientifically speaking serum level) of Cholesterol is 130 to 200mg per 100 ml. There are two kinds of sources of Cholesterol in our blood - food and manufacturing inside the body, mainly in the liver. In the food only source of Cholesterol is Animal food - that is Meats, fishes, eggs, milk and milk products. One must also know one more fact that " No plant food contains any Cholesterol". It is thus obvious that all kinds of animal foods must be avoided by the heart patients and those who want to avoid heart disease in future. For most of the vegetarians about 70-80% of the Cholesterol requirement comes from our Liver's manufacturing. Some cholesterol in these vegetarians come from Milk and milk products. White of egg and skimmed milk ( which still contains 2-3mg Cholesterol per 100 ml ) are the only two animal products that Saaol allows to the heart patients.

Hormone production. Cholesterol plays a part in producing hormones such as estrogen, t e s t o s t e r o n e , p r o g e s t e r o n e , aldosterone and cortisone.Vitamin D production. Vitamin D is produced when the sun's ultraviolet rays reach the human skin surface.

2. What are the functions of Cholesterol?Bile production. Cholesterol produces bile acids which aid in digestion and vitamin absorption.Cell membrane support.Cholesterol plays a very important role in the creation and maintenance of human cell membrane.

3. Sources of Cholesterol in the food

FOODS WHICH HAVE CHOLESTEROL (Values per 100gm Edible Portion)

In the liver- Cholesterol is manufactured from Acetyl Co A , which in turn is converted into HMG (Hydroxy Methyl Glutaryl) Co A. This HMG Co A is then converted into Mevalonic Acid by an enzyme called HMG Co A Reductase. Mevalonic acid is then converted into Cholesterol. The body gets the basic source material of Cholesterol

- the acetyl Co- A from oils and other foods. Mostly the liver produces Cholesterol in the same amount required by our body. But in some people this manufacturing capacity is in excess. This is mainly due to their genes - which also runs in family. This is why you will find many people who do not consume any

animal foods but s t i l l the i r Cholesterol is very high. Such people are also in high risk of getting heart disease. That is why it is said that one must stop all animal foods for few days and then they should

check the serum Cholesterol . If this is higher than 150 mg/dl or 100ml - the liver is the main culprit. It is not under working Liver but it is a over working liver.

5. LDL and HDL - Composition

Cholesterol travels alone and also in small packages where it combines with proteins and Phospholipids. That is why these are called lipoproteins. These packages are made of fat (lipid) on the inside and proteins on the outside. Two kinds of lipoproteins carry cholesterol throughout your body: low-density lipoproteins (LDL) and high-density lipoproteins (HDL). Having healthy levels of both types of lipoproteins is important. Have a look of the composition of Lipoproteins in the box. One must also know that

HDL (called the good Lipoproteins) has very little Cholesterol and more proteins and can pick up some cholesterol from the blockages. That is why it is called "Good Cholesterol Lipoprotein". On the other hand LDL has plenty of Cholesterol and it can lead to more blockages - that is why it is called Bad Cholesterol Lipoproteins. LDL s t a n d s f o r L o w D e n s i t y Lipoproteins and HDL stands for High Density Lipoproteins. Most of the laboratories do not measure LDL, but calculate it with a formula " LDL = Cholesterol - Triglycerides/5 - HDL". If you have your report in your hand please see if this formula fits in your case also. So, for measuring LDL in the serum we need only three figures - Cholesterol, Triglycerides and HDL.

6. What are Triglycerides and VLDL?

Triglycerides (TG or TRIG) are entirely different fats from Cholesterol. Cholesterols and Triglycerides are the main two culprits which lead to the blockages which kill millions in this modern world. The irony is that the whole lot of cardiologists and doctors are not interested to stop these two kinds of fats.

The name of Triglycerides is a recent entry in medical sciences - may be three to four decades. Before this only Cholesterol and LDL was known to create the blockages. This is why Cholesterol is much more popular name amongst the medical doctors and heart patients. After 1960s so many clinical research studies showed that Triglycerides

also are equally important in causing the blockages. Triglycerides also have carbon, hydrogen and oxygen in their structure. The base molecule is Glycerol. Three fatty acid chains are attached to this Glycerol molecule. That is why it is called Triglycerides. They also travel in the blood separately and also in combination

of proteins. Though Triglycerides are present in LDL and HDL it is also carried in the form of Triglycerides packed lipoproteins called VLDL ( Very low density lipoprotein) VLDL is very close to Triglycerides and is again calculated as Triglycerides divided by five. This means if one has TG of 100 - the VLDL will be 20.

The process deposition of the two fats - mainly the Cholesterol and Triglycerides and their subordinates called LDL and VLDL is called Atherosclerosis in medical science. It is a very complicated and still not c o m p l e t e l y u n d e r s t o o d phenomenon. Few hundred pages of explanation can be written on this process of block formation - which takes 20-50 years. The basic mechanism is that these fats - Chol, TG , LDL, VLDL - if in excess get deposited on the inner wall of the coronary arteries, which take blood to the heart muscles. Thinking them to be foreign bodies the white blood cells(WBC) in these walls attack them

7. How Cholesterol and Triglycerides leads to Blockages?

and take them inside the cell and the ultimately the walls. These white blood cells when get filled with excess fats are called foam cells. With time these fats increase and increase - creating the blockages.When these fats lead to more than 80% blockage it leads to Angina. And when these blockages break the covering membrane it leads to clot leading to heart attacks. Cells can use cholesterol as an insulator to stay functional in cold conditions, whereas triglycerides are simply stored in fat cells until they are needed for extra energy. When you work out, you are burning off extra triglycerides, not cholesterol.

8. Oils and Nuts are Cholesterol free. Can they increase Cholesterol?

Yes, all vegetable oils are cholesterol free because the fact is that cholesterol is present only in animal products. And since all these oils come from plant products, there is no question of cholesterol being present in them. The oil companies highlight the "zero cholesterol" slogan in order to promote their product and the consumer gets

misleaded. No oil made from vegetable or plant origin contains cholesterol. Consumption of oil does not increase Cholesterol level , but it can increase Triglycerides level in blood. Even the nuts like Almond, Cashew nut, Wall nuts and Ground nuts are free of Cholesterol but loaded with Triglycerides.

The most important drug for the heart patients are now the drugs which reduce Cholesterol and Triglycerides. A group of drugs called Statins can lower the blood levels of Cholesterol by working on the liver by slowing down the production of Cho les te ro l . But it can not neutralize the C h o l e s t e r o l b e i n g consumed in diet. So, before s ta r t ing the Statins all the p e o p l e m u s t s t o p t h e consumption of Animal food which contain Cholesterol. Statins actually inhibit or antagonize HMG Co A Reductase enzyme which is mainly responsible in the production of Cholesterol in the liver.

9. Cholesterol lowering drugs.

Another group of drug is called Ezetimibe which can cut down the absorption of the Cholesterol from the intestine. To digest food, our liver makes bile which has plenty of Cholesterol. This Cholesterol is reabsorbed in the intestine and

lower Cholesterol to a small extent but they can mainly lower Triglycerides.The examples of all these drugs are given in the box. Combination of these drugs are also available.

Inauguration of the New Head office of Saaol in Chattarpur.

We are very glad to inform you that the new head office of Saaol was officially inaugurated on the 8th of November, 2014. All our centers spread across India and abroad will now be governed by this new office and mostly Dr. Chhajer will be consulting our patients in this office only.This inauguration was unique in the sense that it was not inaugurated by any VIP but it was opened by Saaol's first patient Mr. V L Tuli, who was the first not only to consult Dr. Chhajer 19 years back but also was the first on the list of patients joining Saaol in its first ever camp after Dr.Chhajer resigned from AIIMS. He was advised for a second bypass surgery by Dr.Trehan at the Escorts heart Institute after his first Bypass failed in 1995. His Bypass surgery was done in 1986 but after 7-8 years he started having angina again. He reached AIIMS for joining the Reversal camp and met Dr.Chhajer. When he was told that Dr. Chhajer Is resigning from 1st September, 1995 to start his own clinic in Lajpat Nagar - he came for the inauguration of our first clinic and consulted Dr.Chhajer on that day. He followed Saaol completely with the help of his wife and today even after 19 years he is hail and hearty. He can walk 4-5 km per day and is consuming minimum medicines. His

Bypass is now 28 years and no second bypass was required. Other members of the same batch also attended this inauguration. Hundreds of old patients and Saaol members who joined Saaol more than 10-15 years back came to congratulate Dr.Chhajer on this occasion. We now plan to s tar t Video conferencing with Dr.Chhajer from this center - so that you can meet him in video from any of our centers.
