Benefits Overview

During Open Enrollment you can update or change your elections for:

• Medical, Dental or vision- changes (submit form) or no changes (do not submit form)

• Waiver of Insurance-must recertify annually if you wish to have waiver dollars

• Spousal Surcharge- all employees must recertify annually regardless of marital status

• Flexible Spending Account (FSA)- must elect annually if you wish to have this benefit

• Life & 403(b) beneficiary updates- It is important to update your beneficiaries on file as needed, during this OE you will need to update your beneficiary information.

Qualifying Life Events

If you experience a qualifying event and desire to make changes to your benefits enrollment, you have 31-days from qualifying event date to do so.

Employee and Dependent Benefits Eligibility

• Employees working at least 30 hours per week who have met District requirements and the obligations of their Collective Bargaining Agreement

• Legally married spouses

• Medical- Children up to age 26 and children who are over 26 but are permanently disabled

• Dental & Vision- Children up to age 19 or up to age 23 if full-time student

Flexible Spending Account (FSA)

The District offers both a Health Care and Dependent Care FSA.

• 2017 FSA maximum/minimum contribution limits:• Health Care- $2,600/$120• Dependent Care- $5,000/no minimum

• Carryover provision• Allows you to carryover amounts from $60 to $500 of unused

funds from your health care FSA at the end of each plan year. The carryover does NOT apply to the Dependent Care FSA.

• 60-day run-out period no longer offered for submitting claims.

Who needs to take action?

Eligible employees who wish to enroll in or make changes to current benefits enrollments, employees who wish to waive insurance coverage and those electing

or re-electing the FSA.

ALL EMPLOYEES MUST submit a spousal surcharge form regardless of marital status and update life beneficiaries
