INFORMATION NEEDED WITH APPLICATION
Personal financial information shared with the CHIP staff is personal and confidential and shall be so
designated. The personal and confidential financial files will not be available for public inspection.
THE FOLLOWING INFORMATION BELOW WILL BE REQUIRED FOR INCOME VERIFICATION (PLEASE NOTE THAT NOT ALL THE INFORMATION WILL PERTAIN TO YOUR CURRENT INCOME SITUATION):
Federal Filed Income Taxes – A copy of your most recent Federal Filed Income Taxes. If you are self-employed, forward copies of the last two years.
Employment – Copies of three (3) weeks’ pay stubs reflecting year to date total.
Unemployment – A statement from the agency verifying the amount of income received.
Pension – A copy of a check receipt reflecting the monthly amount received or a copy of one of the monthly checks.
Social Security, SSI, Disability – A copy of the entitlement letter or a printout from the agency. Income must reflect this year, not the previous year income.
Child Support – A printout from the agency reflecting the monthly amount received.
Bank Statements – Copies of the last three (3) months statements (savings, checking, CD, stocks, bonds, etc.).
Other Verifications Needed:Homeowner’s Insurance – A copy of a statement or invoice that reflects the premium amount you pay for homeowner’s insurance.

HOME REPAIR PROGRAM

PROGRAM
Miami County has received federal funding from the Ohio Development Services Agency to provide Home Repair Grants to County residents.
PURPOSE
The purpose of the County’s Home Repair Grant Program is to repair housing conditions that pose a threat to the health and safety of the occupants, or pose an ongoing threat to the structural integrity of the home.
GRANT AMOUNT
Maximum assistance per applicant is $8,000, and the minimum assistance is $200 per applicant. Applicants are not entitled to the maximum.
PROCESS
Miami County Department of Development inspector will conduct an initial assessment of the home and determine financial eligibility of applicants seeking assistance under the Home Repair Grant Program. Eligible applicants will then be scored on a point system. Applicants scoring the highest point totals will be offered assistance until funds are depleted.
WHEN
Persons requesting assistance will be placed on the waiting list.
WHERE
The Home Repair Grant Program is available to homeowners throughout Miami County, with the exception of residents of the City of Piqua. Piqua residents are encouraged to contact the City of Piqua for information on its housing repair programs.
ELIGIBLE REPAIRS
Improvements may include such items as: roofs, gutters and downspouts, electrical, plumbing, heating, insulation, foundations, bathroom upgrades, water heaters, handicap accessibility, water and sewer tap-ins, water well installation, lead-based paint hazard controls, etc.
Miami County staff will conduct the home inspection, prepare bid specifications, and prepare a cost estimate on behalf of the homeowner. The homeowner will be responsible for obtaining bids from contractors and signing a repair contract with the selected contractor.
APPLICANT QUALIFICATIONS
To be eligible for a Home Repair Grant, at a minimum:
1. You must be the owner of the home (i.e., not a renter).
2. You must live in the home as your permanent residence.
3. Your home must be in need of health, safety or emergency repairs.
4. You must be financially eligible as determined by the “Income Limits” below.
