Special devices passed through a scope allow physicians to place sutures along the outlet.

Physicians place sutures until the outlet is back down to around 10 mm in diameter.

Sutures can also be added around the pouch to reduce its volume.

Have you started to regain weight after your initial success from gastric bypass? For many gastric bypass patients, over time, the small stomach pouch and the outlet that connects it to the small intestine stretch out. As a result, patients begin to eat more food before they feel full. We offer a safe and effective endoscopic procedure to repair your enlarged pouch and outlet and return them to their original post-gastric bypass proportions.

WHY WOULD I REGAIN WEIGHT AFTER GASTRIC BYPASS?

Several studies suggest that the majority of patients regain significant weight after their initial gastric bypass. Common causes include gradual stretching of the stomach pouch and outlet. When the standard-of-care Roux-en-Y procedure, is performed, the stomach and outlet are made very small which reduces the amount of food that can be consumed and slows the passage of food through the digestive tract. Together, this helps people feel full after eating just a small amount of food. As a result, weight loss occurs from a dramatic decrease in calories. Many investigators have shown that when the stomach pouch and/or stoma gradually stretch, it takes longer for the patient to feel full. As a result, patients start eating larger meals and may begin to regain weight.

ENDOSCOPIC OUTLET AND POUCH REPAIR

We now offer qualified patients an endoscopic procedure to reduce the volume of an enlarged pouch and the diameter of the outlet. Through an endoscope passed through the patient’s mouth, a physician can safely place the same type of sutures used during traditional abdominal surgery. Instead of being admitted to the hospital and facing the risks associated with abdominal surgery, this minimally invasive endoscopic procedure can be performed in an outpatient setting and leaves no external scars.

WHAT ARE THE BENEFITS OF THE ENDOSCOPIC PROCEDURE?

An endoscopic repair eliminates the need for an open or laparoscopic procedure. With traditional abdominal surgery, scarring and adhesions from the initial bypass procedure often make open or laparoscopic revision surgery very challenging. Abdominal revision surgery usually takes longer than the original gastric bypass and patients are three times more likely to develop complications.

With the endoscopic approach, patients reduce their risks to major abdominal surgery, have less post-operative pain, recover faster and have no abdominal scars. Typically, patients go home the same day, but each patient’s discharge will vary based on his/her physician’s recommendation. Please discuss your options for enlarged outlet and pouch repair with your physician or surgeon.

WHAT ARE THE SIDE EFFECTS?

It is anticipated that patients will feel little or no discomfort from the endoscopic procedure. Minor side effects may include nausea, a temporary sore throat, swollen tongue, and lip pain from the insertion of the endoscope into the mouth.

WHAT IS THE RECOVERY PROCESS?

Typically, patients return to their normal routine within 48 hours. Following endoscopic suturing repair, patients should follow the same diet and exercise regimen they did after their initial bypass surgery.
