Celebration of Life
Signature HealthCARE of ClarksvilleClarksville, TN
Introduction
The event was a memorial service for residents who had passed away during the previous quarter. It provided an opportunity for stakeholders, residents, and family members to reflect and reminisce about those who were being honored. It was also a time where some closure could be achieved over the loss of a loved one.
Objectives
• To provide closure and honor the
memories of the residents who were part
of the Signature family.
Approach
• The event was attended by residents who were highly
cognitive.
• A stakeholder sang, “What a Day that Will Be” and
another sang, “Amazing Grace”
• Cake, cookies and punch were served to the guests in
attendance
Development and Planning• There was no theme. The decorations consisted of a large table
covered by a white table cloth. On each side of the table was a large
vase of seasonal flowers. The center of the table was highlighted by
a large vase of white flowers. The table was strewn with white rose
petals and lit candles. There were 4 candles situated at the front of
the table to symbolize the honorees. Name cards were placed in
front of the candles to represent the individuals.

Challenges
• If a resident is discharged to the hospital and passes,
there is no record. We have started a spreadsheet that
allows us to keep up with all of the resident deaths.
Outcomes
• It gives everyone an opportunity to speak about the
resident.
• The event also fosters a climate of closure for the
families of the residents.
