    A letter giving permission from the person named on the certificate or if deceased, the appropriateauthorised person (as stated in the Registrys Certificate Access Policy).

    Identification for applicant from Lists 1, 2 or 3; (see over for table); and

    Identification of the person from Lists 1, 2 or 3 who is entitled to the document (see over for table)(as stated in the Registrys Certificate Access Policy)

    IDENTIFICATION REQUIREMENTS *

    Correct as at 1 July 2009

    When applying for a Western Australian birth, death, marriage or change of name certificate on behalf of anotherperson, identification for the applicant from Lists 1, 2 or 3 and identification of the person from Lists 1, 2 or 3 whois entitled to the document must be provided:

    ONE form of current identification from List 1; OR

    TWO forms of current identification from List 2; OR

    THREE forms of current identification from List 3PLUS

    ONE form of current identification from List 2
