I, Mst. MUMTAZ widow of MUHAMMAD MASKIN, Resident of Nawai Waqt

House, Sector G-7/1, Islamabad, do hereby solemnly affirm and declare as under

:-

1. That my date of birth is 21/05/1953

2. That I am legal widow of deceased Muhammad Maskin who was

died on __________________

3. That I am only widow of my deceased husband and there is no

other widow of my deceased husband.

4. That after the death of my deceased husband I am leading my life

as the widow of my deceased husband and did not contract the

second marriage.

5. That it is therefore requested the Benevolent Fund may kindly bee

granted to me.

AFFIDAVIT.

I, MR. MUHAMMAD FAROOQ RAZA S/o Muhammad Jahangir Resident of

H.NO.DD-692, ST.NO.15, JHANDA CHICHI, RAWALPINDI CANTT, do hereby

solemnly affirm and declare as under :-

1. That my mother Mst. Sabiha Khatoon Wd/o Muhamamd Jahangir

has died on 19/04/2011.

2. That my mother was maintaining an account in NBP Rawat Branch

and left an amount of Rs. ________________ behind her in the

said account.

3. That all legal heirs of my deceased mother have permission/NOC

me to get the all dues/balance amount from the said bank account.

4. That I hereby undertake and declare that if there appears any other

legal heir except my said legal heirs (list of disclaimers attached) I

will be liable to pay all the bank dues/losses if any.
