Individual SolicitationDefinition of Individual

A person who is a parent, guardian

or immediate relative of a patientsuffering from chronic diseases thatrequires long term and/or expensivemedication. He/she shall becertified by the Social Worker of theLocal Social Welfare andDevelopment Office of his/herlocality to be eligible to seek furtherassistance in behalf of the patient.

Discuss with the applicant the reasonsfor the disapproval and provide the

Responsibility of the SWDAs

Accountable and liable for anyactions of the individual solicitor tothe issued permit

Keep/file all documents for pre-application, during and postprocesses

Provide information on mattersrelated to the solicitation activitywhile respecting the privacy ofdonors.

Validity of the PermitTreated as local fund campaign

Valid only in the approved area/s

within the region for a definite periodwhich in no case shall exceed sixmonths

The name in the permit to be issued

should be the DSWD registered andlicensed SWDA or the CIU of theDSWD or LGU that has jurisdictionover the applicants fund driveactivities with the name of the

applicant also included.

Post Facilitation ProcessIndividual solicitor shall inform theconcerned DSWD FO on the scheduleof inventory/accounting of solicitedfunds (atleast 1 month before the expiry

date)Within sixty (60) days after the expirydate of the permit, whetherconsummated or not, surrender the

authority together with the followingreports:

Fund utilization reportverified/certified by the auditor ofDSWD registered and licensedSWDA or the CIU of the DSWD/LGU

Post Facilitation Process List of donors and their

corresponding addresses and contactnumbers, if applicable

Expired original certificate authorizingthe conduct of local fund campaign.

If permit is unused, a justification fornot continuing the fund campaign

activity.If permit not yet fully disbursed orutilized submit a status report of thefunds within the 60 days. A report shallbe submitted monthly thereafter until allthe solicited funds are utilized

Post Facilitation Process

The DSWD FO where theapplication was filed, whether

announced or unannounced, shallmonitor/visit the conduct of fund-raising activities of the individual towhom the permit was issued to

determine whether these are inaccordance with the permit issued.

The solicitation paraphernalia shouldprotect the beneficiaries identity, privacyand confidentiality and shall not portray adehumanizing picture or situation of theintended beneficiary/ies.

If actual pictures of the beneficiaries areto be used, they should not show theirfaces but rather their activities while atthe agency. No information on the

beneficiary shall be uploaded/published.Solicitation paraphernalia shouldhighlight the agencys thrust, activitiesand success stories rather than the life of

the beneficiaries.
