Instalment payment plan: Members will have the option to pay their membership over 9 equal monthly instalments. The instalment payment plan can only be paid by credit card (Visa or MasterCard accepted). Your credit card statement will display ‘IMG STG Melbourne’. IMG are the Cowboys’ contracted e-commerce partner. There will be an account transaction fee of 4.5% per transaction which will be deducted with each instalment. In the event that a payment is dishonoured, your tickets will be cancelled.

Instalment payment process:1. When selecting to pay your membership via instalments, you will be required to pay the initial instalment upon the processing of your membership application.2. The remaining balance will be deducted over eight (8) equal monthly instalments, commencing on 15 December 2011 and concluding on 15 July 2012. 3. If you renew/join after 15 December 2011 using the monthly instalment payment plan, your first deduction will equal the sum of months passed to

bring you into line with the current month.

Deductions will occur on the 15th day of each month, with final payment on 15 July 2012.

BPAY: A biller code and reference number will be supplied upon receipt of this form. BPAY telephone & internet banking – pay as little or as many payments as you like, as long as the membership is paid in full by 4pm Friday 30 March 2012. Call your bank, credit union or building society to make this payment or to set up a recurring payment from your cheque or savings account or credit card.
