Guideline in Diabetic Patients ที่จัดขึ้นเมื่อวันที่ 21-22 มกราคม 2556 ที่อัมพวา
จ.สมุทรสงคราม และเพื่อนๆ คงจะได้รับข้อมูลที่สรุปได้จากการประชุมเร็วๆ นี้ค่ะ

ปริทันต์ทันโลก
สำาหรับปริทันต์ทันโลกฉบับนี้ เราจะมาต่อกันเรื่องของ Furation Management to Save or Extract ? กันตามที่สัญญาไว้ในเล่มก่อนหน้านี้นะคะ หากท่านสมาชิกปริทันต์ท่านใดลืมว่าเราพูดค้างไว้ถึงตอนใดบ้าง เราได้หยิบยกตัวอย่างในตอนท้ายมาให้อ่านเล็กน้อยค่ะ
post treatment complication ของ tunnelling มักเกิดจาก root caries ซึ่งมักพบใน 2 ปีหลัง tx
ส่วน regeneration พบว่า complete bone fill ไม่ได้เป็น predictable outcome สำาหรับ regeneration ในการ tx. FII (Huynh-Ba et al 2009, Laurell L et al 1994) แต่การเปลี่ยนจาก FII มาเป็น FI สามารถเป็น Predictable result ในการ tx. FII ด้วย regeneration ปัจจัยที่ส่งผลต่อผลการรักษา plaque con-trol และ Smoking ทั้งในแง่ Cal gain , V-bone gain, H-bone gain
แต่โดยสรุปแล้วจาก Hugnh-Ba G, et al 2009 ก็พบว่า ยังไม่มี Scientific evidence ใดที่จะบอกได้ว่า treatment ใดที่ดีที่สุด
โดยสรุป Root resection 5 yr. Survival rate is good (~80-90%) 10 yr. Survival rate is fair to good (~60-90%)และ Causes of Failures มาจาก 1. Post treatment Complication ได้แก่ - Fracture - Periodontal breakdown - endodontic Failure / Combination 2. Smoking 3. Occlusal load มักพบในกรณี มีฟัน ≤ 20 ซี่ในปาก parafunctional habits (bruxism,clenching) 4. Remaining bone support (กรณีระยะ bone ถึง CEJ มากกว่า 6มม.)

Conclusion Degree I furcation can be successfully treated non-surgically or surgically สำาหรับ Degree II & III จะให้ favorable outcome ตราบเท่าที่จะสามารถดูแล Plaque controls ได้ดี และมารับการ maintehance ตรงเวลา inadequate indication for extraction is a common reason for poor long-term prognosis of treated furcation involved molars the decision to save or extract furcation involved molars during initial therapy is the key to successful tx.
Degree II & III furcation have favourable outcome, as long as the maintenance & pt’s plaque control are adequate.
