PACT---Patient Aligned Care Teams 3 Teams consist of Physician Registered Nurse Licensed Practical Nurse Medical Support Assistant

Providing comprehensive physical examinations, including women’s health exams, and follow up treatment for chronic diseases and conditions, using a team approach with the Veteran at the center of the treatment plan.

Optometrist in clinic 3 days per week,

Monday, Tuesday, and Wednesday. Outpatient Mental Health in clinic

daily. Podiatry in clinic 2 days per week,

Monday and Thursday. Laboratory on site daily. Social Work on site daily.

Addition of 4th PACT Team Audiology services Physical therapy services Increase in Telehealth services Change in care delivery for face to face visits Addition of group appointments
