Rapidly changing world of

Predictions
The ability to foretell what is going to
happen tomorrow, next week, next month,
next year.
And to have the ability afterwards to ex-
plain why it didn't happen.
Winston Churchill
Rapid growth of cardiac surgery in seventies and
eighties in Zurich was almost exclusively due to
increase of CABG
In early eighties, with advent of cardioplegia, we
truly believed that CABG mortality was
approaching zero
Am Anfang war die koronare Dilatation ein
risikoreicher Eingriff: bei ersten 95 Patienten
mussten wir bei 10 % der Fälle wegen
Komplikationen (Gefässverschluss oder
Dissektion) eine notfallmässige chirurgische
Revaskularisation vornehmen, zum Teil unter
fortlaufender Reanimation (ohne Mortalität).

when.

Stenting belongs to one of the bleakest
chapters in the history of Western
medicine. Cardiologists are marching on
because the interventional cardiology
industry has a cash flow comparable to
the GDP of many countries and doesn’t
want to lose it.

North Carolina at Chapel Hill, quoted in Wall Street

Aortenklappenersatz in Deutschland, 2014
Conclusion: Although patients undergoing TAVI at hospitals
without on-site CS department were older and at higher predicted
perioperative death risk, major complications, and in-hospital
mortality were not statistically different, suggesting the feasibility
and safety of Heart Team-based TAVI at non-CS sites. These
findings need confirmation in future randomized study.

Arrhythmie Chirurgie
Zuerst grosser Erfolg der intellektuell
anspruchsvollen operativen Technik (WPW
Chirurgie, LV und RV Ablation), später
durch Katheter Technik und ICD
vollkommen abgelöst.
WPW Chirurgie in achtziger Jahren
Arrhythmie Chirurgie später
Renaissance als Chirurgie des
Vorhofflimmerns (“Maze Procedure”), heute
meistens abgelöst durch verschiedene
Katheter Techniken. Kommt nur langsam
zurück zur Chirurgie durch minimalinvasive,
endoskopische Operationen.

Hohe Kosten der fortgeschrittenen Therapien
müssen mit anderen, allgemein akzeptierten
Ausgaben verglichen werden
• Quality-adjusted life year (QALY) cost after
LVAD implantation ~ 140’000 €
• Annual cost of keeping a convicted criminal
in jail in Western Europe ~ 100’000 € (Zurich
~ 160’000 CHF)
Destination Therapy is becoming main
indication for LVAD
“Total artificial heart” wird z.Z. kaum verwendet
Überlebensrate von LVAD wird langsam
akzeptabel: 58% @ 3 Jahre und 48% @ 4 Jahre
LVAD hat viel bessere Überlebensrate als
BiVAD!

Disappearing and Shrinking Areas Of Cardiac
Surgery In Western World
• Substantial proportion of CABG is replaced by PCI
• AVR is being replaced by TAVI
• Mitral repair and replacement by catheter techniques
• ASD and PDA closure
• Coarctatio: balloon dilatation and covered stents
• Mitral and pulmonary stenosis: catheter dilatation
• Arrhythmia surgery: catheter procedures and AICD
• Pulmonary valve replacement: catheter technique
• Aortic aneurysms: TEVAR and EVAR
beherrschen:
• Katheter Technik für transvaskuläre
Eingriffe
• Bildinterpretation für 2-D und 3-D Echo,
CT und MRI
• Endoskopische Chirurgie für
minimalinvasive Operationen (Klappen,
ACBP, Arrhythmie)
Klassische Herzchirurgie zerfällt in mehrere
Spezialgebiete:
• Kongenitale Herzchirurgie ist schon jetzt ein
anerkannter Spezialfach
• Aorten Chirurgie wird von Hybridspezialisten
beherrscht
• Komplexe Koronarchirurgie verlangt spezielle
Weiterbildung (Arterielle Revaskularisation, etc.)
• Fortgeschrittene Klappenchirurgie
(Rekonstruktionen, endoskopische Eingriffe)
• Spezialisten erzielen hervorragende Ergebnisse
in der Arrhythmie Chirurgie
• GUCH bleibt ein Waisenkind
Herzchirurgie muss sich wieder der
Forschung widmen!
In allen hochentwickelten Länder
verlangen Patienten eine kurze,
schmerzfreie Behandlung.
Diese soll in Zusammenarbeit mit
Kardiologen und Radiologen entwickelt:
Herz- und Gefässteam werden obligat!
Zukunft der Herzchirurgie liegt in
Neuentwicklungen, nicht in der
Verteidigung überholter Positionen.
