Exactly as it is on birth certificate

Official Transcripts (Sealed and usually mailed directly from THS)

• Complete a “Transcript Request Form”

• Provide a college addressed, stamped envelope to the Counseling Center for each official transcript

Unofficial Transcripts

• May be photocopied and sent by student

• Usually acceptable for scholarship applications

• See Ms. Cortez in the Counseling Center for an updated copy

Letters of Recommendation

• Completed Data Sheets

• Providing adequate time is crucial!

• Provide an addressed, stamped envelope or arrange to pick up in person
