TONISCHE FASE
Klinisch belang parasympatische fase?
 Controversieel
 Asystole niet steeds aanwezig
Afh locatie electrodes: unilateraal vs bifrontaal
Geen invloed op cardiale functie ( Shettar et al)
CLONISCHE FASE
Sympathische overactivatiteit: catecholamine surge
Duur tachycardie + AHT = duur aanval
Invloed plaats elektrodes beperkt (Gangadhar et al)
 Rate-pressure product
Retrospectieve review: Rivera, Lapid, Sampson
35 patiënten met hartfalen en LV dysfunctie (513 ECT behandelingen)
 Goede tolerantie ECT
Geen HDC, MI, ischemie gedurende of 24u na ECT sessie
Retrospectieve review: Gerring, Shields
40 patiënten
12/17 patiënten met onderliggend hartlijden ontwikkelden cardiale complicaties na ECT
Meestal benigne, zelf-limiterend: atriale en ventriculaire ectopie en VT
 Ischemie bij 2 patiënten
 1 hartstilstand
Retrospectieve review: Gerring, Shields
70% complicaties
 Onderliggend hartlijden
 >50j
Transiënte daling EF bij gezonde personen
ECT bij 53 patiënten ( McCully et al)
 7 ontwikkelden nieuwe LV dysfunctie
 8 ontwikkelden ‘wall motion abnormalities’
Conclusie:
Goede tolerantie ECT
Belangrijke cardiale complicaties zeldzaam
 Cave: ouderen, onderliggende hartziekten
 Wateroplosbaar, synthetisch anticholinergicum
Quaternaire ammoniumstructuur: trage penetratie biologische membranen (BBB, placenta)
 PO, IM, IV
 Volledig geïoniseerd bij fysiologische pH
Plasmaconcentraties na injectie glycopyrrolaat 5µg/kg
Snelle distributiefase: t1/2α = 2.22 ± 1.26 minTrage eliminatiefase: t1/2 = 0.83 ± 0,27 hr
Distributievolume: 0.42 ± 0.22L/kg
Eliminatie:
Precieze metabole pathways niet gekend
Dierenexperimenten:
 hydroxylatie cyclopentylring
 Oxidatie hydroxylgroep
85% excretie via urine in 48 uur
Werkingsmechanisme:
Competitief antagonisme Ach thv de 5 muscarinereceptoren (M1-M5)
 Geen effect op nicotine bindingssite
BLOKKADE PARASYMPATISCHE OUTFLOW
↓ secreties (M1, M3)
 speekselklieren
 farynx
 trachea
 bronhi
↑ hartfrequentie (M2)
↑ pH maag (M1, M3)
↓ volume maag
Bronchodilatatie (M3)
 Duizeligheid
 Slaperigheid
 Visusstoornissen
 Droge ogen
 Droge mond
 Obstipatie

ECT of Robinul????
Enquête BA Kramer:
50/58 ziekenhuizen gebruiken routinematig anticholinerge premedicatie
 26 enkel Atropine
 10 enkel glycopyrrolaat
 14 beiden
Populariteit omwille van antisialologe en vagolytische eigenschappen
Standaard dosis: 0,8 mg atropine of 0,2 mg glp
Atropine en Glp bereiken beide adequate mucosale droogte en preventie overmatige secreties (BA Kramer)
‘silent aspiration’:
 passief transfer maag-orofarynx
 Passief transfer orofarynx-tracheobronchiaal
Vagale reacties in 30% van 127 ptn, 4% klinisch ernstig (Bankhead et al)
Geen ernstige bradycardiën of overdreven sialloree in retrospectieve studie 6000 ECT’s zonder anticholinergica (Verheecke & Troch)
 1 sterfgeval door asystolie (Rubin)
Post-stimulus pause (PSP) bij 80% van ptn zonder anticholinerge premedicatie
MAAR geen impact asystole op hartfunctie geobserveerd (Shettar et al)
Gebruik anticholinergica aanbevolen door meeste ECT guidelines
Geen studies die impact anticholinergica op outcome kunnen tonen.
Vele studies excluderen ptn met voorbestaand hartlijden
 Interferentie door psychotropica
Atropine
 SVT
 onregelmatige pols: premature ventriculaire contracties
Glycopyrrolaat
Hartritme meer stabiel ( Greenan et al)
Atropine
 Goedkoper
 Beter gekend
Glycopyrrolaat
 Meer ventriculaire aritmiën
 Nausea, braken
Shettar et al
1e ECT behandeling zonder anticholinergicum indien niet cardiovasculair belast.
Anticholinergicum indien verlengde asystole of hogere intensiteit elektrische stimulus
Abrams
 Toedienen bij ‘hypodynamic cardiac states’
 Vermijden bij ‘hyperdynamic cardiac states’
Abrams
Geen evidentie voor cardiale events bij toedienen anticholinergicum
Geen evidentie dat ECT-geïnduceerde asystolen zonder risico zijn
Controversieel
Tegenstrijdige bevindingen
Studies inconclusief
Weinig impact op clinical outcome
