Rose Questionnaire

•We will add English units for HT, WT, waist, and hip.

•Make sure to write down both Metric & English units.

•  Check all the questions in the forms are appropriately filled in before participant leaves clinic.

•  No blank allowed. Must be missing, refusal, or unknown.

Physical Examination

Sample Checklist

• QC ID

CBC Results

• Locally done, not all the same.

•  Be sure the units match those on the form

Psychosocial

• May be self-administered.

•  Check for completion when participant is in the clinic.

•  If refused, mark appropriately

Medication

• We are using CHS/MESA system.

•  Can not change the form

•  Use drop-down list to choose the medication

•  If the dosage is not in the drop-down list, use the closest one

•  If the medication does not have a match, write it in the COMMENTS field and inform Debra

FFQ

• May be self-administered. Coach the participant how to answer.

•  Use PENCIL

•  Do not change SHS ID for other participant and re-send to Block

•  When requested for correction by Block, do not change ID

Pedometer

• Coach participant how to wear and re-set

•  Need at least 3 days

ECG Transmission

• Make sure SHS ID is enter correctly

•  Set cart # correctly

•  If wrong ID discovered, inform BOTH Cornell and CC

Echocardiogram, Carotid Ultrasound, & Popliteal

Ultrasound

• Make sure SHS ID is enter correctly

•   If wrong ID is discovered, inform BOTH Cornell and CC

Blood and Urine Samples

• Make sure the printed label package belongs to the participant being examined

•  Serum, plasma, whole blood, DNA

• If you need to re-draw the samples, ask the participant to come back fasting. BE SURE to collect ALL of the blood & urine samples.

Data Entry

• Designate one person to check the completion of ALL the forms to be entered

•   At the end of each page, check the monitor screen for correct entry.

•  If errors are discovered after the data are swabbed, contact Debra

New family member examined:

• Use the SHS-IV Personal Interview Form I, (instead of SHS-V, PI-I), and,

•  MAIL/FAX it to the CC

Mortality and Morbidity Surveillance

• Continue cohort M & M

• Continue Family mortality surveillance

• Start Family morbidity surveillance by abstracting self-reported CVD event from Phase IV medical history
